1.Clinical and Histopathologic Study of Steroid Acne.

This study was performed to investigate clinical and histopathologic features of steroid acne, which was induced by systemic administration and topical application of corticosteroids. Thirty five cases of steroid acne visited to Department of Dermatology, Hangang Sacred Heart Hospital from September, 1g79 to June, 1984 were analyzed, and the results obtained can be summarized as follows: 1. The peak age of the subjects was third decade(42.9%) with an average age of 30. 1 years, and male to female ratio was l.9: 1,2. The skin lesions had unique clinical features that showed many, uniform sized, erythematous papules and pust;ules. 3 The predilection sites of steroid acne induced by systemic steroid therapy were anterior chest(93.1%), back(44,8%), neck(31.0%), shoulder(31.0%) and face (20.7%) 4 Among thirty five cases of steroid acne, twenty cases were induced by parenteral adrninistration of dexamethasone disodium phosphate(group A), nine cases by oral administration of prednisolone(group B), and six cases by topical application of three kinds of steroid creams(group C). 5. The mean induction time after starting steroid in group A(ll. 3 days) was shorter than those in group B and C(18.9 days and l4.8 days respective)y). The mean total dosage of used steroid in group A was 191. 3mg of dexamethasone disodium phosphate and that in group B was 515. 7mg of prednisolone. On histopathologic findings of twenty two skin biopsy specimens of the three groups, perivascular inflammatory reaction was the most common finding followed by intra-and peri-follicular inflammatory reaction, dermal vascular dilatation, necrosis of follicular epithelium, comedo, intraand periollicular abscess and rupture of follicle.

6.A Case of Intravenous Pyogenic Granuloma.

Cooper at al and Mills et al asserted that the essential pathologic finding of pyogenic granuloma (PG) is a unique lobular organization of capillaries. This pathologic pattern is present not only in conventional cutaneous PG, oral-nasal PG but also rarely in intradermal PG, subcutaneous PG and intravenous PG. A 32-year-old man presented with a two-day history of an asymptomatic, movable, pea-sized nodule on his left forearm. Excision biopsy revealed a pink, oval, cyst-like nodule within the vein of the subcutaneous tissue of the medio-lateral aspect of the forearm. Histopathologic finding of the nodule showed rnultiple lobules of capillaries and the lumens of the capillaries were lined by normal endothelial cells. The nodule was connected with the venous wall by a thin stalk. Each lobule was separated and surrounded by fibrous stroma. There was no evidence of thrombi.

8.A Case of Rectal Adenocarcinoma Metastatic.

Adenocarcinoma metastatic to the skin usually originates in the lung, breast, large intestine or ovary. Cutaneous metastatic tumors from rectum or colon tend to appear in the abdominal or perineal area, but rarely appear in the perianal and genital skin. A 53-year-old female presented multiple nodules in and near the surgical scars of inguinal, pubie, vulvar, perianal areas and normal skin of left thigh two months after surgical removal of rectal adenocarcinoma. Biopsy specimens from the nodules of the perianal area and left thigh showed adenocarcinoma of well differentiated atruetures like the primary rectal carcinoma.

9.Study of pH and gas analysis of umbilical arterial blood and apgar score as indicators of newborn health.

Apgar score is most widely used evaluating indicator of newborn health, but it is very subjective. Umbilical cord blood gas analysis is more objective and effective than Apgar score in evaluation of newborn status. Cord blood gas was changed slightly by processing of time after fetal birth. This study was undertaken to observe objectiveness and effectiveness of umbilical arterial blood gas analysis and effects of time interval of cord clamping to newborn health with 122 pregnant women and their babies. We observed following results: 1. There were poor correlation between Apgar score and umbilical cord arterial blood analysis in evaluating of newborn health (P>0.05). 2. There was no clinical significance of Apgar score or umbilical arterial blood gas analysis as single indicator in evaluating of newborn health (P>0.05). 3. Gas analysis and pH of umbilical arterial blood was more helpful in evaluating of newborn health than Apgar score. 4. There were no significant effects of time interval of umbilical cord clamping to newborn health. 5. If there were no indications of early umbilical cord clamping, cord blood sampling at immediately after birth without cord clamping was more effective to evaluate newborn status.

10.A Case of Epidermal Nevus Syndrome.

Epidermal nevus syndrome is now delineated as a congenitally acquired syndrome consisting of deformities of the skin (various linear epidermal or organoid nevi), central nervous system, skeleton, cardiovascular system, and renal system. A 4-year-old girl had linear epidermal nevus on the right lower extremity, right ear lobe and anterior neck, and multiple skeletal abnorrnalities since birth. And she had an adult fist-sied, soft, nontender mass on the right lower abdomen since 2 years of age. Radiologic examination showed scoliosis of the thoracic spines, deformities of the lumbar spines, focal gigantisms of the left hand and right foot, and other skeletal abnormalities. Histological examination of the linear, verrucous lesion of the right leg showed hyperkeratosis and papillomatosis. The surgically removed abdominal mass revealed the finding of hydrosalpinx.
