BACKGROUND: The p53 gene mutations have been found in a variety of human skin cancers. To date most studies concerning p53 gene mutations in skin cancers focused on squamous cell carcinoma(SCC) and its precancerous lesions such as actinic keratosis and Bowen's disease, especially related to ultraviolet light, but few on basal cell carcinoma(BCC). OBJECTIVE: This study was aimed to investigate the aberrant expression rates on p53 mutant gene in BCCs as well as in SCCs and their changes depending upon their clinicopathologic characteristics. METHODS: An immunohistochemical study was employed using the mouse monoclonal antibody raised against recombinant human p53 in 30 BCCs and 16 SCCs. Also, clinical and histopathologic analyses for age, sex, site, duration and histopathologic classification were performed. RESULTS: p53 immunoreactivity was observed in 57% of BCCs and 75% of SCCs. In BCCs P53 immnoreacticvity was increased by up to 61% in cases(28/30) developed on the sun-exposed area. In SCCs p53 immunoreactivity(89%) on non-sun-exposed area was unexpectantly higher than that(57%) on sun-exposed area. No significant correlation between p53 imunoreactivity and clinicopathologic characteristics in BCCs and SCCs was noted. CONCLUSION: Mutations of p53 may play an important role of the pathogenetic sequence in a large part of the pathogenetic sequence in a large part of BCCs as well as SCCs.

Open carpal tunnel release has been the standard method of sumical treatment of carpal tunnel syndrome. Recently endoscopic carpal tunnel release has been introduced and is heing used by many authors. The advantages of this new technique are less postoperative pain, rapid restoration of power and rapid return-to-work. However many considerate authors, in spite of these advantages. insist that the inevitahle risk of neurovascular injury during the endoscopic procedure should not he underestimated. The purpose of our study is to compare the clinical results of endoscopic carpal tunnel release with those of open release. 20 open carpal tunnel releases in 16 patients and 15 endoscopic carpal tunnel reieases (single-portal technique) in 11 patients were performed hy the first author. Preoperative conditions of both groups are not different. Authors compared the clinical results between the two groups with some parameters. The overall clinical results were not different significantly hetween two groups. Rapid return-to-work(36 days in endoscopic group versus 60 days in open group) and less postoperative scar and pillar pain in endoscopic group were demonstrated. However, the major complication of one median nerve injury in endoscopic group seemed to overweigh these some benefits. We suggest that the standard operative technique for carpal tunnel syndrome should be open carpal tunnel release and more considerations should be takcn in choosing endoscopic method because of its inherent risk.

Surgical and anesthetic management in patients with complex vascular lesions is often challenging. Cardiopulmonary bypass followed by total circulatory arrest, deep hypothermia and barbiturate cerebral protection are efficacious adjuncts in the surgical treatment of giant basilar artery aneurysm. These techniques were utillized in two large basilar artery aneurysm patients with good results, and the surgical and anesthetic considerations are reviewed.

Diarrhea is a common complication of antibiotics usage. The diarrhea ascribed to antibiotics usually consist of loose or watery stools, sometimes containing mucus but rarely grossly evident blood. Almost every antibiotics has been implicated, although cases related to the use of vancomycin or aminoglycosides are rare. Although the pathogenesis of antibiotics-associated diarrhea is not confirmed, recent studies have shown that a toxin of clostridium difficile is implicated as a cause of it. Diagnosis is made by detecting toxin in the stool or stool culture for C.difficile. Treatment is either directed at binding the toxin with anion exchange resins such as cholestyramine in mild case, or at eradicating the C.difficile organism with vancomycin, metronidazole or bacitracin for more severe cases. Since shoulder have a higher proportion of muscle which would have low electrical resistance, there could be more electrical damage to the rotator cuff muscles. We present a patient with acute rotator cuff tear by sudden uncontrolled jerking contractions caused by an electrical shock. A case of 42-year-old man with acute rotator cuff tear due to electrical injury to the shoulder was presented. Magnetic resonance imaging showed a full thickness tear and an undulating appearance of the peripheral end of the torn supraspinatus and infraspinatus muscle, suggesting an acute complete rupture. By arthroscopic surgery, the torn rotator cuff tendons were repaired with a suture bridge technique. At the final follow-up, the patient had a full, pain-free range of motion and had fully recovered shoulder muscle power.

Anisakiasis refers to the infestation of humans by species of marine nematode larvae belonging to the subfamily Anisakiae. An acute abdominal symptom may occur after ingestion of raw marine fish containing nematode larvae of the genus Anisakis. Migration of the parasite into the wall of gtomach, small intestine or, less commomly, the colon elicits a striking eosinophilic granulomatous tissue response. A 39 year old man was visited St. Vincent Hospital because of abdominal distension and tenderness. The plain abdominal X-ray showed ileus pattern suggesting intestinal obstruction. Hence we presented a case of intestinal Anisakiasis combined with intstinal obstruction.

Pilomatricoma(calcifying epithelioma of Malherbe) is a rare benign turner of the skin which develops from immature hair matrix cells. The diagnosis of the calcifying epithelioma of Malherbe is only rarely made on clinical ground but is, as a rule, easily made by histologic examination. We briefly report 2 cases of pilomatricoma occurring on upper eyelid with review of the literature.
