1.The visual prognostic factors in vitreoretinal surgery for myopic foveoschisis

Objective To observe the clinical effects of vitreoretinal surgery for myopic foveoschisis,evaluate the visual prognostic factors.Methods Twenty-seven eyes of 23 patients with myopic foveoschisis underwent vitreoretinal surgery were analyzed retrospectively. All the patients had undergone the examinations of best corrected visual acuity (BCVA),intraocular pressure,slit lamp microscope,direct ophthalmoscope,binocular indirect ophthalmoscope, A or B ultrasonic scan and optical coherence tomography (OCT).Follow-up duration varied from 6 to 36 months,with the mean of (19.4± 10.03)months.The visual acuity,retinal reattachment and the complications were observed.Logistic regression analysis was performed to predict the prognosis of visual acuity.Results Twenty-one eyes (77.78%) were anatomically reattached, six eyes (22.22%) were not anatomically reattached. Postoperative BCVA improved in 24 eyes (88.89%),reduced or remained unchanged in three eyes (11.11%). No ocular complications such as fundus hemorrhage,low or high intraocular pressure,endophthalmitis were found.The preoperative BCVA (OR =9.11,P =0.007),axial length (OR =0.31, P =0.038) and the photoreceptor inner and outer segment (IS/OS) junction line continuity (OR=4.32,P=0.001) are closely related to visual prognosis.Conclusions Vitreoretinal surgery is an effective approach to treat myopic foveoschisis with both anatomical and visual improvement.The preoperative BCVA,axial length and IS/OS line continuity are closely related to visual prognosis.
2.Clinical Observation of Shexiang Zhenggu Tincture Combined with Huoxue Zhitong Powder in the Treatment of Acute Traumatic Swelling and Pain

OBJECTIVE:To observe the efficacy and safety of Shexiang zhenggu tincture combined with Huoxue zhitong pow-der in the treatment of acute traumatic swelling and pain. METHODS:Totally 160 patients with acute traumatic swelling and pain were randomly divided into group A and group B. Patients in group A were given Shexiangzhenggu tincture for external use,3 times a day;patients in group B were orally added one bag of Huoxue zhitong powder,3 times a day,based on the treatment of control group. 7 d was course and it lasted 2 courses. The clinical efficacy,the scores of visual analogue pain score (VAS) and swelling remission before and after 7 and 14 d treatment and the incidence of ADR were observed. RESULTS:After 7 d treatment, the total effective rate in group A was significantly higher than group B,with significant difference(P<0.05);after 14 d treat-ment,there was no significant difference in the total effective rate between 2 groups (P>0.05). After 7 and 14 d,the scores of VAS and swelling remission were significantly lower than before;14 d treatment was lower than 7 d treatment,and group B was significantly lower than group A after 7 d treatment,with significant differences (P<0.05);there were no significant differences among the scores of VAS and swelling remission in 2 groups after 14 d treatment (P>0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Shexiang zhenggu tincture combined with Huoxue zhitong powder has better efficacy, can relieve the pain and swelling faster,with similar safety.
3.Efficacy Observation of Amlodipine Besylate Combined with Xinkeshu in the Treatment of Hypertension

OBJECTIVE:To observe the efficacy and safety of amlodipine besylate combined with Xinkeshu in the treatment of hypertension. METHODS:100 patients with hypertension were randomly divided into control group (50 cases) and observation group(50 cases). Control group was orally given one Amlodipine besylate tablet,once a day;observation group was additionally given 4 Xinkeshu tablets,3 times a day. The treatment course was 8 weeks. Clinical efficacy,systolic blood pressure(SBP)and di-astolic blood pressure(DBP)before and after treatment in 2 groups were observed,the incidence of adverse reactions was record-ed. RESULTS:The total effective rate in observation group was significantly higher than control group,the incidence of adverse re-actions was significantly lower than control group,the differences were statistically significant (P<0.05). Before treatment,there were no significant differences in the SBP and DBP between 2 groups (P>0.05). After treatment,the SBP and DBP in 2 groups were significantly lower than before,the differences were statistically significant(P<0.05);but there was no significant difference between 2 groups (P>0.05). CONCLUSIONS:The efficacy of amlodipine besylate combined with Xinkeshu is superior to amlo-dipine besylate alone in the treatment of hypertension,with better safety.
4.Influence of Budesonide-Solution Inhaled Therapy on Airway Resistance in Asthmatic Bronchitis Children

Objective To examine the changes of the airway resistance(AR) in asthmatic bronchitis children before and after budeso-nide-solution inhaled therapy.Methods Fifty-six cases of asthmatic bronchitis children were randomly divided into A(regular treatment+budesonide-solution inhaled) and B groups(regular treatment),and 30 normal children at the same age as control group.The AR was eva-luated by Microloop lung function meter with MicoroRint sensor.The changes of AR were compared within above 3 groups.Results AR in asthmatic bronchitis children increased significantly compared with normal children.After 2 weeks treatment,AR decreased significantly in both A and B groups compared with that of 2 groups before therapy,AR in A groups declined to normal control level,but still kept higher level in B groups.Conclusions AR in asthmatic bronchitis children increase significantly.The effect of Budesonide-solution in asthmatic bronchitis children is partly via reducing AR to improve ventilated condition.
5.Effects of long-term,low-dose hormone replacement therapy on serum level of c-reactive protein and homocysteine in postmenopausal women

Objective To investigate the effects of long-term,low-dose hormone replacement therapy(HRT) on serum level of creactive protein(CRP) and homocysteine(Hcy) in postmenopausal women.Methods A total of 141 postmenopausal women were selected from medical staff of the Peking Union Medical College Hospital.Of these,63 subjects have been treated with low-dose sex hormone for over 5 years(HRT group),78 subjects have never received hormone replacement therapy as control group.The serum levels of CRP and Hcy in two groups were evaluated.Results The serum level of estradiol(E_2) in HRT group was significant higher than that in control group(median,inter-quartile range;33.8 ng/L,30.9 vs 21.3 ng/L,33.2)(P0.05).There was no correlation between the serum level of CRP,Hcy and the plasma levels of E_2 and FSH respectively.Conclusion Long-term and low-dose HRT does not increase the serum level of CRP and Hcy in postmenopausal women.
6.Antibiotic Resistance Genes Detection in Meticillin-resistant Coagulase Negative Staphylococcus

OBJECTIVE To study the epidemiology of the drug-resistant genes in meticillin-resistant coagulase negative staphylococcus(MRCNS).METHODS mecA,aac(6′)/aph(2″) and aph(3′)-Ⅲgenes of aminoglycoside-modifying enzyme(AME) and ermA/B/C genes of erythromycin methyltransferase were detected by polymerase chain reaction(PCR) from 40 MRCNS strains.RESULTS Thirty-nine strains carried mecA gene,32 strains with aac(6′)/aph(2″) gene,15 strains with aph(3′)-Ⅲ gene,30 strains with ermA/B/C gene and 2 strains with tetM gene in 40 MRCNS strains.CONCLUSIONS About 65% MRCNS strains carry mecA,aac(6′)/aph(2″),/aph(3′)-Ⅲ and ermA/B/C genes at the same time.
7.Effects of ursolic acid on interleukin-33 expression in HaCaT cells induced by interferon-γ

Objective To evaluate effects of ursolic acid (UA) on interleukin?33 (IL?33) expression in HaCaT cells induced by interferon?γ(IFN?γ), and to explore their mechanism. Methods Some HaCaT cells were treated with UA at different concentrations(0, 0.1, 1, 5, 10, 20, 40 and 80μmol/L)for 24, 48 and 72 hours separately. Then, methyl thiazolyl tetrazolium(MTT)assay was conducted to evaluate cell proliferative activity. A cell model of inflammation was established by culture of HaCaT cells with the presence of 200μg/L IFN?γ. Some HaCaT cells were classified into several groups to be treated with IFN?γ(200μg/L)and UA(10 and 15μmol/L)alone or in combination (firstly treated with IFN?γ followed by UA treatment), and those receiving no treatment served as the blank control group. Reverse transcription PCR (RT?PCR) was performed to detect mRNA expressions of IL?6 and IL?33, and Western?blot analysis to measure IL?33 protein expression after 12?hour culture. The expressions of extracelluar signal?regulated kinase 1/2(ERK1/2)and phosphorylated ERK1/2(p?ERK1/2)were also measured by Western?blot analysis after 5?and 60?minute treatments with IFN?γand UA alone or in combination. Results MTT assay showed that the treatments with 5-20μmol/L UA for 24 hours had no effects on cell proliferative activity, while 40-80μmol/L UA could significantly inhibit it at 24, 48 and 72 hours (all P < 0.05). Thus, 10 and 15 μmol/L were chosen as the concentrations of UA for further study. After the treatment with 200μg/L IFN?γ, there was a significant increase in the expressions of IL?33 mRNA(0.812 ± 0.036 vs. 0.412 ± 0.021), IL?6 mRNA(0.947 ± 0.091 vs. 0.595 ± 0.030)and IL?33 protein(1.317 ± 0.119 vs. 0.147 ± 0.036)in HaCaT cells compared with the blank control group(all P<0.05). Compared with the IFN?γgroup, the IFN?γ+10?μmol/L UA group and IFN?γ+15?μmol/L UA group both showed significantly decreased expressions of IL?33 mRNA(0.447 ± 0.042 and 0.438 ± 0.028 respectively, both P<0.05), IL?6 mRNA(0.437 ± 0.099 and 0.350 ± 0.075 respectively, both P<0.05)and IL?33 protein(0.923 ± 0.058 and 0.564 ± 0.113 respectively, both P<0.05). There were no significant differences in IL?33 mRNA expression between the IFN?γ+10?or 15?μmol/L UA group and blank control group(P>0.05), while IL?33 protein expression was significantly lower in the IFN?γ+15?μmol/L UA group than in the IFN?γ+10?μmol/L UA group(P<0.05). The p?ERK1/2 protein expression significantly increased in HaCaT cells treated with IFN?γ for 5 and 60 minutes compared with the blank control group, but significantly decreased in the IFN?γ+15?μmol/L UA group compared with the IFN?γgroup(0.458 ± 0.053 vs. 0.941 ± 0.042 at 5 minutes, 0.302 ± 0.054 vs. 0.509 ± 0.032 at 60 minutes, both P < 0.05). However, no significant differences were observed in the total ERK1/2 protein expression between the IFN?γ+15?μmol/L UA group and IFN?γgroup at 5 or 60 minutes. Conclusion UA can suppress IL?33 expression in HaCaT cells induced by IFN?γ, likely by regulating expressions of the ERK signaling pathway?related proteins.
8.Effect of Structured Information Provided on Self Care Knowledge, Self Care Performance, and Functional Status of Liver Cirrhosis Patients.

PURPOSE: The purpose of this study was to identify the effects of structured information provided on self care knowledge, self care performance, and functional status. METHOD: The data were collected using self care knowledge and performance assessment tool, and functional status assessment tool from both group hospitalized patients at D hospital in Busan. RESULTS: There was a significant improvement(p=.032) in self care knowledge in experimental group compared to the control group. But there were not improvement in self care performance and functional status in experimental group compared to the control group. But comparing to pretest and posttest in experimental group, There were significant improvement in self care performance (p=.003) and functional status(p=.013). CONCLUSION: Structured information provided showed increased in the degree of knowledge, self care performance, and functional status. But the effect size of program which had been developed in this study was estimated small, so there is needed to modify this program and to research repeatedly.

9.Suppression of COX-2 protein to cell apoptosis in non-small cell lung cancer.

BACKGROUNDOne of mechanisms of carcinogenesis is suppression of cell apoptosis which leads to accumulation of aberrant cells. The aim of this study is to investigate cell apoptosis and COX-2 protein expression in non-small cell lung cancer (NSCLC).
METHODSCell apoptosis, expression of COX-2 and microvessel density (MVD) were detcted in 111 NSCLC samples by TdT-mediated dUTP nick end labeling (TUNEL) technique and immunohistochemical staining.
RESULTSThe positive rate of COX-2 protein expression was 67.6% (75/111), and there were 53 patients with high level cell apoptosis (47.7%). Expression of COX-2 protien was significantly related to TNM stages (P=0.025) and lymph node metastasis (P=0.018). The MVD in NSCLC tissues with positive COX-2 expression was significantly higher than that in negative expression ones (P=0.000). COX model showed that lymph node metastasis (P=0.006) and positive expression of COX-2 protein (P=0.000) were independent prognostic factors of NSCLC.
CONCLUSIONSThe expression of COX-2 protein may suppress cell apoptosis of tumor, and it may serve as a potential marker of prognosis for NSCLC.
10.Clinical Study of Pulmonary Tuberculosis for Admitted Patients at National Masan Tuberculosis Hospital.

OBJECTIVE: Although the prevalence of pulmonary tuberculosis has decreased progressively after the national control program for tuberculosis began, nowadays the number of MDRTB is increasing seriously. MDRTB tends to be poor responsive to current antituberculosis regimens. It is mainly due to poor compliance, high rate of side reaction of secondary drugs, and limitation in number of available drugs. The purpose of present study is to evaluate the clinical features of pulmonary tuberculosis patients admitted in one national tuberculosis hospital and to expose the problems pertaining to current remedies, to increase the treatment efficacy for pulmonary tuberculosis including MDRTB in the end. METHOD: Retrospective analysis of 336 pulmonary tuberculosis patients admitted in National Masan Tuberculosis Hospital was done. Contents of analysis were patients profile, the first diagnosed time and medical institutes, family history, residence, previous treatment history, chief complaints at the time of admission, lesion site on chest X-ray film, combined deseases, side reaction to antibuberculosis drugs, used drugs before admission and the results of drug sensitivity test. RESULTS: The ratio between male and female was 4:1. Age showed relatively even distribution from 3rd to 6 th decades. 64.6% of the patients was diagnosed at public health center. Weight loss was the most common complaint at admission. Bilateral lesions on chest X-ray films were 59.8%. 1 30patients had combined desease, of which DM was the most common(37.7%). 95patients had family history, of which parents were the most common(41.7%). According to the time of first diagnosis, 31 patients were diagnosed before 1980, and after then the number of patients was increased by degrees Residence overwhelmed in pusan and gyung-nam province. 258 patients got previous treatment history, of which 112 patients(43.4%) had more than 3 times and only 133 patients(51.6%) got regular medication. 97 patients used more than other 3 drugs in addition to INH, EMB, RFP and PZA before admission. 154 patients were informed with the results of drug sensitivity test. of which 77 patients had resistance to more than 5 drugs. Gastrointestinal problem was the most common in side reaction to drugs. CONCLUSION: In the case of weigt loss of unknown cause, tuberculosis should be suspected. In first treatment, sufficient and satisfactory explanation for tuberculosis is necessary and treatment period should not be stict to 6 month-short term therapy. In retreatment, new drugs should not be added to used drugs enen though drug sensitivity restlts show sensitivity to some of them. Proper time for surgical intervention should not be delayed.
