1.Acromion Fracture after Hook Plate Fixation for Distal Clavicle Injury: A Report of 2 Cases.

Fractures at the lateral end of the clavicle inevitably require surgical treatment as there is high potential for delayed union or nonunion. Acromioclavicular dislocation also requires stable and solid fixation for healing, and surgical treatment is recommended for the maintenance of joint function. The hook plate maintains the biomechanics of the acromioclavicular joint, enabling early range of motion. Therefore, for the past 10 years, the hook plate has been widely used in distal clavicle fractures and acromioclavicular joint injuries. However, the hook plate is associated with several complications, such as proximal clavicle fractures, widening of the hook hole, rotator cuff tear, subacromial impingement, and often acromial fractures. We report on two unusual cases of acromion fracture after hook plate fixation in patients with distal clavicle fracture and acromioclavicular dislocation alongside a literature review.

2.Point Prevalence of Anxiety and Depression among Cancer Inpatients in a General Hospital: A Preliminary Study for the Development of Distress Management Strategies in Cancer Patients.

OBJECTIVES: This study aimed to examine the point prevalence of anxiety, depression and the associated risk factors in hospitalized patients with cancer. METHODS: A total of 114 patients with cancer who admitted to the Cancer Center in St. Mary's Hospital were enrolled for a day. The patients were asked to complete Hospital Anxiety and Depression Scale(HADS). Sociodemographic and cancer related clinical variables were also evaluated. RESULTS: The point prevalence of anxiety and depression was 10.5% and 34.2%, respectively, as assessed with HADS. Logistic regression models revealed that distant metastasis was significantly associated with anxiety in patients with cancer and previous chemotherapy and Low socioeconomic status were significantly associated with depression. CONCLUSIONS: Our results indicate that more than one third of patients with cancer suffer from either anxiety or depression. In addition, previous chemotherapy as well as low socioeconomic status and distant metastasis were associated risk factors of anxiety and depression, respectively. These findings suggest the need for screening and systematized psychosocial intervention for psychological distress in hospitalized patients with cancer.

6.Femoral Head Fractures

9 Cases of femoral head fractures treated in Seoul National University Hospital were reviewed. They were classified according to Pipkin, and assessed according to Ebstein's criteria. The longest follow-up was 3 years and the shortest 6 months, the average being 1 year and 6 months. l. 8 cases were injured due to auto accident and there was no one who had worn seatbelt. 2. 1 case of intestine rupture and 2 cases of homothorax were accompanied. 3. There were 1 excellent, 1 good, and 1 poor results in 3 cases of Pipkin type 1, 1 fair result in 1 case of Pipkin type 2, 1 good and 1 fair results in 2 cases of Pipkin type 3, 1 good and 2 fair results in 3 cases of Pipkin type 4. 4. The following complications were encountered, 2 cases of sciatic nerve palsy, 1 case of avascular necrosis of femoral head, 1 case of traumatic arthritis.

7.Segmental Spinal Instrumentation in the Treatment of Scoliosis

Since Luque introduced new posterior instrumentation by segmental sublaminar wiring for neuromuscular scoliosis in 1976, preliminary reports have been published regarding its use with Harrington rod or Luque rod for other types of scoliosis, traumatic lesions of the spine, and spondylolisthesis as well as for degenerative disorders and tumors. Its advantage includes rigid internal fixation which often obviates the requirement for postoperative immobilization and the significant correction of deformity. Disadvantages are longer operation time, increased blood loss and the risk of neurologic damage. Segmental spinal instrumentation was carried out using either Harrington rod or Luque rod in 30 cases of scoliosis; 16 in idiopathic scoliosis, 12 in paralytic scoliosis and 1 each in congenital scoliosis and neurofibromatosis, at the Department of Orthopedic Surgery, Seoul National University Hospital, for 3 years from January 1984 to December 1986, Twenty six cases had been followed for more than 1 year and following results were obtained. 1. In idiopathic scoliosis, average preoperative curve was 65.0 degrees and immediate postoperative curve was 27.9 degrees with 57.4% correction. There was 1.2 degrees loss of correction with an average follow-up of 19.5 months (14–28 months). 2. In papalytic scoliosis, average preoperative curve was 108.5 degrees and immediate postoperative curve was 55.5 degrees with 49.5% correction. There was little loss of correction with anaverage follow-up 24.5 months (13–38 months). 3. No neurological complication occurred as a result of sublaminar wiring. 4. Segmental spinal instrumentation is an effective method with an advantage of better correction, greater contouring of the spine to avoid flat back, and less external immobilization in the treatment of idiopathic or paralytic scoliosis without increased complication.

8.Osteocutaneous Free Flap Transfer by Microsurgical Technique

A microvascular osteocutaneous free flap is totally detached from its origin and reattached at a distant site, in one stage, by the use of microvascular anastomosis to maintain its viability. The advantages of microvascular osteocutaneous free flap trsnsfer are more rapid healing, greater resistance to infection, less risk of absorption than conventional bone grafts, and shortened hospitalization. Two cases of microvascular osteocutaaeous free flap transfer were performed at the department of Orthopaedic surgery of Hanyang University Hospital, and satisfactory results were obtained.

9.Lipoma of the Heart: An Autopsy case report.

Lipomas of the heart are benign neoplasms and have rarely been described. Due to the fact that they normally cause no symptoms, diagnosis is often purely accidental. Because of the rarity of these tumors, it seems worthwhile to present an example studied at autopsy. It was associated with the sudden death of a 15-year-old boy. The tumor arose from the wall of the left ventricle and occupied the pericardial cavity, measuring 13x7x6 cm in size. The tumor was whitish-yellow, translucent, and soft. Microscopically, the tumor was composed of mature adipose tissue which extended between muscle fibers. This current case, the giant cardiac lipoma is believed to produce disturbances of the conduction system and distrubances of cardiac filling.
10.Sarcoma-like Mural Nodule in Ovarian Mucinous Cystadenocarcinoma: A case report.

We describe an ovarian mucinous cystadenocarcinoma with a sarcoma-like mural nodule. In the literature, rare cases of ovarian mucinous tumors have been described which contain foci of undifferentiated carcinoma, sarcoma, and sarcoma-like nodules. The distinction between these lesions is important because of poorer prognosis of true sarcoma and anaplastic carcinoma than sarcoma-like mural nodules. This case shows different results of immunohistochemical stain for anaplastic carcinoma.
