1.A Case of Macular Amyloidosis due to Nylon Towel.

Amyloidosis is a disorder of protein metabolism characterized by the extracellular deposition of abnormal protein fibrils. We report a case of macular amyloidosis due to nylon towel on his back. A 61-year-old male presented with asymptomatic hyperpigmented macules on the scapulae for 6 years. He had a habit of scrubbing his back for many years with a rough nylon towel while taking a bath. The histologic examination revealed liquefaction degeneration and homogenous eosinophilic deposition in the papillary dermis. In Congo red, amyloid material stained positively and in anti-keratin antibody staining. He had neither clinical nor laboratory evidence of systemic amyloidosis. A diagnosis of friction amyloidosis was made on histological and immunohistopathological findings.

2.A Case of Generalized Nodular Scabies.

Nodular scabies is a clinical variant of classic scabies with uncertain cause. The cutaneous manifestations consist of firm, erythematous nodules focally affecting genitalia, groin and axillae, and no case of generalized involvement has been reported in the literature. On histopathologic examination, mites and excretas were known to be seldom found in the stratum corneum, but if serial sections are studied, they can be seen in as much as 20% of the cases. The dermis shows dense superficial and deep perivascular mixed cell infiltration composed of lymphocytes, histiocytes, plasma cells, eosinophils, and atypical mononuclear cells, which should be distinguished from many types of lymphomas. We herein report a case of nodular scabies that clinically affected whole body and histopathologically revealed mite and eggshells in the stratum corneum and perivascular mixed cell infiltration throughout the dermis.
3.A Case of Kikuchi's Disease (Histiocytic Necrotizing Lymphadenopathy) Showing Typical Histopathologic Findings in Cutaneous Lesion.

Kikuchi's disease(Histiocytic necrotizing lymphadenitis) is an idiopathic, self-limiting disease typically affecting cervical lymph nodes in young adults. Cutaneous involvement has been reported in 16-40%. We report a case of Kikuchi's disease in 10-year-old female that showed asymptomatic erythematous papules on the face accompanied by cervical lymphadenopathy. The histopathologic examination of the skin revealed diffuse dermal lymphohistiocytic infiltrate, a large amount of nuclear dust with absence of neutrophils, which is characteristic of Kikuchi's disease. In general, histopathologic findings of skin biopsy show a mirror image of that of lymph nodes involved in Kikuchi's disease. Therefore, it is important to recognize cutaneous manifestations of this entity, because we can avoid invasive lymph node biopsy. if skin lesions involved show typical histopathologic findings of Kikuchi's disease.

4.Serum Specific IgE to Thyroid Peroxidase Activates Basophils in Aspirin Intolerant Urticaria.

Thyroid antibodies are frequently observed in urticaria patients, but their roles in urticaria are not clearly elucidated. We investigated the role of serum specific IgE to thyroid peroxidase (TPO) in patients with aspirin intolerant acute urticaria (AIAU) and aspirin intolerant chronic urticaria (AICU). We recruited 59 AIAU and 96 AICU patients with 69 normal controls (NC). Serum specific IgE to TPO was measured by manual direct ELISA, and CD203c expressions on basophil with additions of TPO were measured to prove a direct role of TPO in effector cells. The prevalences of serum specific IgE to TPO were significantly higher in AIAU (15.2%) and AICU groups (7.5%) compared to NC (0%, P=0.018: P=0.013, respectively). Flow cytometry showed CD203c induction in a dose dependent manner with serial additions of TPO in some AIAU and AICU patients having high specific IgE to TPO. Our findings show that the prevalence of serum specific IgE to TPO was significantly higher in both AIAU and AICU patients than in NC. It is suggested that specific IgE to TPO play a pathogenic role in AIAU and AICU.

5.The changes of nociception and the signal molecules expression in the dorsal root ganglia and the spinal cord after cold water swimming stress in mice

Several studies have previously reported that exposure to stress provokes behavioral changes, including antinociception, in rodents. In the present study, we studied the effect of acute cold-water (4°C) swimming stress (CWSS) on nociception and the possible changes in several signal molecules in male ICR mice.Here, we show that 3 min of CWSS was sufficient to produce antinociception in tailflick, hot-plate, von-Frey, writhing, and formalin-induced pain models. Significantly, CWSS strongly reduced nociceptive behavior in the first phase, but not in the second phase, of the formalin-induced pain model. We further examined some signal molecules' expressions in the dorsal root ganglia (DRG) and spinal cord to delineate the possible molecular mechanism involved in the antinociceptive effect under CWSS.CWSS reduced p-ERK, p-AMPKα1, p-AMPKα2, p-Tyk2, and p-STAT3 expression both in the spinal cord and DRG. However, the phosphorylation of mTOR was activated after CWSS in the spinal cord and DRG. Moreover, p-JNK and p-CREB activation were significantly increased by CWSS in the spinal cord, whereas CWSS alleviated JNK and CREB phosphorylation levels in DRG. Our results suggest that the antinociception induced by CWSS may be mediated by several molecules, such as ERK, JNK, CREB, AMPKα1, AMPKα2, mTOR, Tyk2, and STAT3 located in the spinal cord and DRG.

Wheat is known as a causative agent of bakers' asthma, wheat-dependent exercise induced anaphylaxis and food allergies in infants and young children. However, there has been few cases of wheat-induced anaphylaxis in adult age. We report a case of wheat-induced anaphylaxis patient presented in an adult and two IgE-binding components(43 kDa and 48 kDa) were identified.

Angioedema is a non-pitting edema that occurs in the skin and mucus membranes. It is known that major etiologies include hereditary deficiency of C1 esterase inhibitor, temperature extreme, trauma, food sensitivity, and medications such as penicillin, aspirin, NSAIDS and ACE inhibitors. ACE inhibitors are blocking the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor, and increasing local levels of bradykinin, a potent vasodilator. This increased bradykinin has been theorized to cause angioedema and cough in patients on ACE inhibitors. However, there has been very few causes of angioedema induced by angiotensin II receptor blocker. This is the first report of a patient presenting angioedema induced by losartan -angiotensin II receptor blocker- in this century.

9.Buckwheat allergy in adults: comparison of specific IgE between homemade ELISA and CAP system, and identification of IgE-binding components.

BACKGROUND AND OBJECTIVE: Ingestion and inhalation of buckwheat flour can induce IgE-mediated bronchoconstriction and anaphylaxis in sensitized individuals, especially in childhood. The aim of this study was to determine the sensitization rate of buckwheat allergen, measure the level of specific IgE to buckwheat, and identify IgE-binding components in adult patients with various allergic diseases. METHODS: 1,738 allergy patients and 40 healthy controls were enrolled. Skin prick tests were performed using homemade buckwheat extract. The specific IgE level to homemade buckwheat allergen was measured by ELISA, and results were compared to those of the CAP system. ELISA inhibition tests were done to evaluate allergenic relationships with major food allergens and IgE binding components were identified using IgE immunoblot analysis. RESULTS: Among 1,738 patients tested, 60 patients (3.5%) showed more than a 2+ response on skin prick tests to buckwheat. The prevalence of serum specific IgE to buckwheat ranged from 24% in patients with a 2+ response to buckwheat skin prick test, to 50% in patients with a 4+ response. The mean absorbance value increased with skin reactivity although it was not statistically significant. However, CAP results were significantly correlated with skin reactivity (p<0.05). A significant correlation was noted between (the) results by homemade ELISA and CAP. IgE immunoblot demonstrated 20 IgE binding components ranging from 20 to 114 kDa, and 10 components were bound to IgE in more than 50% of the patients tested. CONCLUSION: Natural buckwheat allergens should be considered as one of the causative food allergens in exposed adults. Specific IgE results by homemade ELISA were comparable with those of CAP system. Twenty IgE binding components and 10 major allergens were noted within natural buckwheat allergen. Further studies will be needed to evaluate the allergenic relationships with other food allergens.

10.Specific IgE determination to shrimp (Metapenaeus joyneri) and identification of its allergen.

BACKGROUND: Shrimp is one of the major causative crustacean food allergen. An investigation has been reported that tropomyosin belonged to muscle protein is a major allergen within shrimp. But there have been a few investigations on shrimp allergen in Korea. The aim of this study is to evaluate skin reactivity and specific IgE sensitization to Metapenaeus joyneri which is one of the major shrimp in this country, and to identitify IgE binding components and evaluate allergenic relationship with other species. METHODS: We performed skin prick test with M. joyneri extract in 1,738 patients. ELISA was performed for detection of serum specific IgE antibody. To evaluate the cross allergenecity between M. joyneri and other crustaceans (crab, lobster, crayfish), Dermatophagoides pteronyssinus (Dpt), triton shell, abalone and buckwheat. ELISA inhibition tests were performed with each four patient's sera showing high specific IgE antibody. To identify IgE binding components, SDS-PAGE followed by IgE-Immunoblot were applied. RESULTS: 211 patients (12.2%) showed positive responses (A/H >or=2+) on skin prick test. Serum specific IgE antibodies were detected in 61 patients (37.2%) of 164 sensitzed patients. ELISA inhibition test using four patient's sera showed significant inhibitions by M. joyneri. and other crustaceans including lobster, crab and crayfish, partial inhibitions were noted by Dpt, triton shell, buckwheat and abalone. SDS-PAGE and IgE-imunoblot with patients' individual sera sensitized to M. joyneri showed 12 IgE binding components (31, 32, 38-44, 57, 70, 81 kDa) and two (31, 32 kDa) were bound to IgE in more than 50% of sera tested. Five (43, 44, 57, 70 and 81 kDa) were bound to IgE in more than 25% of sera tested. CONCLUSION: Specific IgE was detected in 37.2% of allergy patients sensitized to M. joyneri. Twelve IgE binding components and two (31, 32 kDa) major allergens were indentified. Cross allergenecity was noted with other crustaceans.
