2.Nutritional Status of Hospitalized Geriatric Patients Using by the Mini Nutritional Assessment.

The prevalence of undernutrition in hospital populations is known to be high. The presence of malnutrition is associated with depression, infections, sarcopaenia, falls, fractures, reduced autonomy and increased mortality. This study specifically examined the prevalence of malnutrition in patients aged 65 or older at the time of admission as determined by the Mini Nutritional Assessment (MNA) which has been a frequently used nutritional risk screening tools in detecting undernutrition in old people. This study was done for one hundred eight hospitalized geriatric patients in Seoul National University Bundang Hospital, Seoul, Korea. On admission baseline history, anthropometrics measurements, laboratory data and nutritional status by MNA were assessed. Length of hospital stay was obtained by reviewing medical charts. We used one-way analysis of variance to compare the differences in variables. Spearman's rank correlation coefficients were calculated for associations between MNA and variables. On admission, 22.3% of patients were malnourished and 40.7% were at risk of malnutrition according to the MNA. Percent of ideal body weight, anthropometrics data, albumin, and hemoglobin were lower in the malnourished patients (p < 0.05). The malnourished patients stayed in the hospital 7.3 days longer, as compared with well nourished patients (p < 0.05). Percent of ideal body weight, albumin, hemoglobin and total cholesterol were correlated inversely with nutritional status according to MNA (p < 0.05). MNA can be used for nutritional assessment in Korean old people, because MNA significantly correlated with other nutritional assessment parameters, such as, anthropometric and laboratory data in hospitalized geriatric patients. The high prevalence of malnutrition in the elderly was observed and the presence of malnutrition on admission predicted a significant increase in the length of hospital stay in this study. Therefore further studies are needed to determine whether nutritional interventions in old people with low MNA scores can improve clinical outcomes during the hospital course.

3.Histopathologic Study of Post-irradiation Specimen.

The number of post-irradiated surgical specimen appears to have risen in recent years due to the increasing utiliz ation of radiotherapy for cancer patients. Radiation therapy damages cells by its effects on the deoxyribonucleic acid and the spectrum of injury ranges from acute self-limited lesion to irreversible chronic lesion. We reviewed 24 cases of post-irradiated specimen and thought that time interval is the main factor influencing the morphologic change. Within six weeks, the individual cytologic changes such as biz arre nuclei, altered nucleus/cytoplasm ratio, amphophilic and vacuolated cytoplasm are noted. Chronic injury resulting from progressive changes in the fibrovascular tissue of the radiated area occur in six months after the initial course of radiation and the vascular changes of intimal thickening and fibrosis, foamy histiocytes within media, periadventital fibrosis and chronic inflammatory cells infiltration are present. Althouhg above mentioned finding are not pathognomonic, we thought them quite constant nd reproducible characteristics of radiation injury.
4.A Clinical Use of CT Scan in Rectal Cancer.

Authors evaluated the accuracy of preoperative pelvic CT scan staging and its effects on management in 12 biopsy proved rectal cancer patients. Authors also studied postoperative CT in 5 patients to detect disease recurrence and metastasis. Preoperative CT staging was identical to surgical and/or pathological staging in 9 patients (75%), but it was underestimated in two cases and overestimated in one instance than in surgical stagings. In 7 cases, CT scan did not alter original choice of procedures. However, preoperative CT staging gave definitive informations to change management plans in 5 cases otherwise the treatment would be difficult and inadequate. Postoperative CT showed local recurrence in one and liver metastases in 2 cases. One of them was not detected at exploratory laparotomy.

5.Efficacy of Anterior Chamber Paracentesis in Intravitreal Triamcinolone Injection.

PURPOSE: To investigate the efficacy of anterior chamber paracentesis in intravitreal triamcinolone acetonide injection (IVTA). METHODS: A prospective, randomized clinical trial was conducted on 30 eyes of 30 patients undergoing IVTA. Eyes were randomly divided into two groups, those which had undergone anterior chamber paracentesis (Group 1, 15 eyes) and those which had not (Group 2, 15 eyes). We analyzed postoperative changes in intraocular pressure (IOP) in each group. RESULTS: In group 1, the mean preoperative IOP was 15.33+/-1.72 mmHg; postoperative IOP at 2 and 15 minutes was 7.80+/-1.47 and 11.73+/-1.67 mmHg, respectively. In group 2, there was significant elevation of IOP (46.73+/-8.26 mmHg) 2 minutes after the injection, although this was reduced to the normal range (16.13+/-2.61 mmHg) within 15 minutes. CONCLUSIONS: Routine anterior chamber paracentesis seems to be inappropriate considering the brief elevation in IOP that immediately follows IVTA.

6.A Case of Pulmonary Carcinosarcoma which Metastasize to Pelvic Cavity After Left Pneumonectomy.

Pulmonary carcinosarcoma(Sarcomatoid carcinoma of the lung) is a rare pulmonary malignancy, which is defined as having an admixtture of both carcinomatous and sarcomatous components. Pulmonary carcinosarcoma occurs most frequentlly in males between 50 and 80 years of age. It predominantly affects the upper lobe and/or the principal bronchi, and is associated with a history of smoking. Here, we report a case of pulmonary carcinosarcoma with a left lobe atelectasis due to an endobronchial mass in a 56-year-old male. After a left pneumonectomy, the pathologic stage was IIb (T3N0M0). Four months later, an abdominal mass was observed and exploratory laparotomy revealed metastases of the pulmonary carcinosarcoma to the pelvic cavity.

7.Visual Loss in One Eye after Spinal Surgery.

PURPOSE: To report a patient who developed an unusual combination of central retinal artery occlusion with ophthalmoplegia following spinal surgery in the prone position. METHODS: A 60-year-old man underwent a cervical spinal surgery in the prone position. Soon after recovery he could not open his right eye and had ocular pain due to the general anesthesia. Upon examination, we determined that he had a central retinal artery occlusion with total ophthalmoplegia. RESULTS: Despite medical treatment, optic atrophy was still present at the following examination. Ptosis and the afferent pupillary defect disappeared and ocular motility was recovered, but visual loss persisted until the last follow-up. CONCLUSIONS: A prolonged prone position during spinal surgery can cause external compression of the eye, causing serious and irreversible injury to the orbital structures. Therefore, if the patient shows postoperative signs of orbital swelling after spinal surgery the condition should be immediately evaluated and treated.

8.Advanced Surface Ablation-Photorefractive Keratectomy (ASA-PRK): Safety and Clinical Outcome for the Correction of Mild to Moderate Myopia with a Thin Cornea.

PURPOSE: To evaluate the safety, clinical outcome, and change in higher-order wavefront aberrations (HOAs) of Advanced Surface Ablation-Photorefractive Keratectomy (ASA-PRK) for the correction of mild to moderate myopia in cases with a thin cornea. METHODS: ASA-PRK was performed, using a rotary epithelial scrubber, post-laser chilled BSS(R) irrigation, and bandage contact lens, in 127 eyes of 70 patients who had a preoperative spherical equivalent within -7.0 D and who were followed-up for more than six months. The patients were divided into two groups, according to their central corneal thickness (CCT), by ultrasonic pachymetry (Group: CCT< or =520 micrometer, 32 eyes, mean 506.6+/-8.6 micrometer; Group: CCT>520 micrometer, 95 eyes, mean 552.3+/-24.8 micrometer). The clinical outcomes of Group I and II were compared. RESULTS: At six months, 100% of Group I (32/32 eyes) and 93.7% of Group II (89/95 eyes) had an UCVA of 1.0 or better (p>0.05). At six months, a spherical equivalent within 0.5 D of emmetropia was observed in 100% (32/32) of the eyes in Group I and in 94.7% (90/95) of the eyes in Group II (p>0.05). At the 12-month follow-up, all eyes showed good stability of the refractive errors, and none showed corneal haze of grade 1 or more, loss of two or more lines of BSCVA, or serious iatrogenic keratectasia. The magnitude of total HOAs, spherical ablation, and coma were significantly higher six and 12 months after surgery in both groups (p<0.05). The postoperative changes in the HOAs at six and 12 months were similar in both groups (p>0.05). CONCLUSIONS: ASA-PRK performed on patients with mild to moderate myopia and a thin cornea (> or =490 micrometer) showed a high level of efficacy, predictability, stability, and safety. There was no statistically significant difference in surgically induced HOAs between Groups I and II. The authors suggest that ASA-PRK be used for the correction of mild to moderate myopia, especially in cases with a thin cornea. Further study will be needed to determine the safety of this procedure for thinner corneas in surface keratorefractive surgery.

9.Treatment of Distal Femoral Fractures with a Retrograde Supracondylar Intramedullary Nail assisted with Arthroscopy.

Since Green et al has introduced a new technique of retrograde intramedullary nail, the use of retrograde interlocking intramedullary nails has been recommended as one of the treatment options for the distal femoral fracture. However there are some disadvantages that an arthrotomy is required for insertion and the knee joint could often be violated. Authors present a simple, arthroscopically assisted method using the retrograde intramedullary nailing for distal femoral fractures with minimal invasiveness to the knee. From March 1995 to March 1997, the retrograde intramedullary nail was used to treat 9 distal femoral fractures. Five of 9 patients were fractured at the distal shaft of the femur and others were fractured at the supracondylar region of the femur. Only one of the fractures was open injury(Gustilo-Anderson grade II). Significant concomitant knee joint injuries were revealed through the arthroscopy in 3 patients. Eight of 9 fractures healed by 5 months, but one fracture was not healed and required bone grafting. Average knee range of motion was 130. Complications included 1 nonunion and 1 hardware failure. There were no patellofemoral problems and no posttraumatic arthritis of the knee joint. This arthroscope-assisted method have some potential benefits that include decreased risk of damage to the knee joint, early evaluation and treatment of the associated knee joint injuries, and accurate placement of the nail.
