1.Effects of a Maternal Education Program Based on the Temperament Theory on the Temperamental Goodness-of-Fit between Mother and Child.

PURPOSE: This study was aimed to evaluate the effects of a maternal education program based on the temperament theory on the temperamental goodness-of-fit between mother and child. METHOD: The research method was a nonequivalent, control group, non-synchronized design and the experimental period was from May 10, 2004 to July 24, 2004. The subjects were mothers who had children 3-4 years old who registered in eight kindergartens or infant schools in M city. Among the total 94 subjects, 47 were allocated as an experimental group and 47 were a control group. Using the SPSS/WIN 10.0 program, the data was analyzed by a Chi-square test, and t-test. RESULT: 1. After treatment with a maternal education program based on the temperament theory, the experimental group significantly decreased in scores for perceived children's behavior problems in comparison to the control group(t=-4.01, p<.001). 2. After treatment with a maternal education program based on the temperament theory, the experimental group did not significantly decrease in scores for parenting stress in comparison to the control group(t=-.85, p=.40). 3. After treatment with a maternal education program based on the temperament theory, the experimental group did not significantly decrease in scores for rejective parenting behaviors in comparison to the control group(t=-1.32, p=.19). CONCLUSION: A maternal education program based on the temperament theory is a useful intervention to decrease perceived children's behavior problems.

2.Relationship between Goodness-of-Fit for Mother-Preschool Child and Parenting Stress in Mother.

PURPOSE: This study was done to identify the relationship between goodness-of-fit for mother-preschool child dyads and parenting stress experienced by the mother. METHODS: Study participants were 500 mothers who had children aged 3 to 5 who attended one of ten kindergartens or infant schools in M City or B City. Descriptive statistics and Pearson's correlation coefficients were calculated using the SPSS program. RESULTS: Comparison of goodness-of-fit scores for mother-preschool child dyad according to the characteristics of the participants, showed a significant difference according to child's age, gender, and birth order, mother's education and occupation, father's age and education, family income, and the chief caregiver in the family. There was a positive correlation between goodness-of-fit scores for mother-child dyad and parenting stress scores for mothers. CONCLUSION: The findings of the study indicate a need to identify differences between children's behavioral problems and parenting styles according to the degree of discord in the mother-child temperaments. It is also necessary to develop and apply nursing programs to promote harmonizing of temperaments, programs in which the characteristics of the child and the mother are considered.

3.Role of Growth Factors and Cytokines on Bleomycin Induced Pulmonary Fibrosis.

BACKGROUND: It is now thought that the earliest manifestation of idiopathic pulmonary fibrosis is alveolitis, that is, an accumulation of inflammatory and immune effector cells within alveolar walls and spaces. Inflammatory cells including alveolar macrophages and resident normal pulmonary tissue cells participate through the release of many variable mediators such as inflammatory growth factors and cytokines, which contribute to tissue damage and finally cause chronic pulmonary inflammation and fibrosis. This study was performed to investigate the source and distribution pattern of transforming growth factor-beta1(TGF-beta1), platelet derived growth factor(PDGF), basic fibroblast growth factor(bFGF), interleukin 1(IL-1), interleukin 6(IL-6), tumor necrosis factor-alpha(TNF-alpha) and the role of these mediators on bleomycin(BLM)-induced pulmonary injury and fibrosis in rats. METHOD: Wistar rats were divided into three groups(control group, BML treated group, BML and vitamine E treated group). Animals were sacrifices periodically at 1, 2, 3, 4, 5, 7, 14, 21, 28 days after saline or BLM administration. The effects were compared to the results of bronchoalveolar lavage fluid analysis, light microscopic findings, immunohistochemical stains for six defferent mediators(TGF-beta1, PDGF, bFGF, IL-1, IL-6 and TNF-alpha) and mRNA in situ hybridization for TGF-beta1. RESULTS: IL-1 and IL-6 are maximally expressed at postbleomycin 1~7th day which are mainly produced by neutrophils and bronchiolar epithelium. It is thought that they induce recruitment of inflammatory cells at the injury site. The expression of IL-1 and IL-6 at the bronchiolar epithelium within 7th day is an indirect evidence of contribution of bronchiolar epithelial cells to promote and maintain the inflammatory and immune responses adjacent to the airways. TNF-alpha is mainly produced by neutrophils and bronchiolar epithelial cells during 1~5th day, alveolar macrophages during 7~28th day. At the earlier period, TNF-alpha causes recruitment of inflammatory cells at the injury site and later stimulates pulmonary fibrosis. The main secreting cells of TGF-beta1 are alveolar macrophages and bronchiolar epithelium and the target is pulmonary fibroblasts and extracellular matrix. TGF-beta1 and PDGF stimulate proliferation of pulmonary fibroblasts and TGF-beta1 and bFGF incite the fibroblasts to produce extracellular matrix. The vitamine E and BLM treated group shows few positive cells(p<0.05). CONCLUSION: After endothelial and epithelial injury, the neutrophils and bronchiolar epithelium secrete IL-1, IL-6, TNF-alpha which induce infiltration of many neutrophils. It is thought that variable enzymes and O2 radicals released by these neutrophils cause destruction of normal lung architecture and progression of pulmonary fibrosis. At the 7~28th day, TGF-beta1, PDGF, bFGF, TNF-alpha secreted by alveolar macrophages sting pulmonary fibroblasts into proliferating with increased production of extracellular matrix and finally, they make progression of pulmonary fibrosis. TNF-alpha compares quite important with TGF-beta1 to cause pulmonary fibrosis. Vitamine E seems to decrease the extent of BLM induced pulmonary fibrosis.

Primary ampullary carcinoma is one of rare gastrointestinal malignancies which can be diagnosed by ERCP and endoscopic biopsy. However, sometimes it is difficult to comfirm the diagnosis when the tumor is not exposed out of orifice of ampulla. Recently, we diagnosed early stage ampullary carcinoma using endoscopic sphincetrotomy for the exposure of tumor in 54 year old woman ia whom ERCP revealed rather impacted stone like filling defect in distal common bile duct. we describe a case of papillary adenocarcinoma of the ampulla of Vater diagnosed by edoscopic sphincterotomy.

5.Giant condyloma acuminatum of rectum

Condyloma acuminatum, a benign disease caused by a filtrable virus, occurs predominantly in the perianal andgenital areas. The lesions are noninvasive but are subject to recurrence. In rare instances, a more aggresive formof this disease, known as "giant condlyloma acuminatum" or "Buschke-Lownestein tumor", occures. In this form,infiltration of the lesion into surrounding structures takes place. This tumor has been reported to occurprincipally in the genitourinary tract. The authors experienced a cases of giant condyloma acuminatum originatingfrom rectum in 67 years old male patient which recurred 3 months after electrofulguration.

6.The Painful Os Subfibulare: Report of Four Cases

The os subfibulare is a relatively uncommon bony anomaly of the foot. It is usually found between the malleolus and talus and ossifies during adolescence or preadolescence. It can be demonstrated on routine anteroposterior ankle film of the foot. In 4 patients presented, the abnormality caused pain and frequent episodes of ankle strain. The pain was relieved in all patients treated by excision of the accessory bone.

7.Secular trends and related factors in leisure-time sedentary behavior among Koreans: an analysis of data from the 2011-2017 Korea Community Health Survey

OBJECTIVES:
Sedentary behavior has gradually increased and has become a public health problem. Therefore, this study investigated trends in weekday leisure-time sedentary behaviors, as well as characteristic socio-demographic and lifestyle correlates in Korean adults.
METHODS:
We analyzed data from 914,946 adults aged ≥19 years who participated in the Korea Community Health Survey (2011, 2013, 2015, and 2017). Leisure-time sedentary behavior was categorized as a binary variable (<4 and ≥4 hr/day). Multivariable regression analysis was used to model the prevalence of sedentary behavior and estimate odds ratios.
RESULTS:
The prevalence of leisure-time sedentary behavior decreased from 15.2% to 14.4% in men and from 16.6% to 16.0% in women between 2011 and 2017, respectively. However, a significant increase was observed in subjects with an education lower than high school in both genders (β coefficient=0.12 for men and 0.08 for women, p for trend <0.001). Women in the lowest household income level (β coefficient=0.08, p for trend=0.001) and with poor subjective health status (β coefficient=0.05, p for trend=0.013) showed an increasing trend. Other factors associated with sedentary behavior were age, education level, body mass index, household income, walking activity, perceived stress level, and subjective health status in both genders.
CONCLUSIONS
Identifying the secular trends and correlates of sedentary behavior by gender and associated factors will provide empirical evidence for developing public health campaigns and promotion programs to reduce sedentary behavior in Koreans.
8.A Case of Child with Obstructive Sleep Apnea Syndrome Recurred after Adenotonsillectomy.

The most common cause of obstructive sleep apnea syndrome (OSAS) in childhood is adenotonsillar hypertrophy. Adenotonsillectomy improves the symptoms quite well in most cases. However, some patients could experience the OSAS again after adenotonsillectomy, who might have several risk factors such as incomplete operation, misdiagnosis, combined anatomical malformation, sinusitis or chronic allergic rhinitis, obesity, initial severe OSAS, and early onset OSAS. We report a case of 11-year-old obese boy who presented with snoring for several years. He was obese with body mass index (BMI) of 26.3 kg/m2 and also found to have fatty liver by ultrasonogram. Initial polysomnography (PSG) showed that he met the criteria of severe OSAS with the apnea-hypopnea index (AHI) of 70.5. He underwent adenotonsillectomy and symptoms improved immediately. Four months later symptoms were relieved with AHI of 0, but 1 year after the adenotonsillectomy he started to complain snoring again and the subsequent PSG results showed that OSAS has relapsed with AHI of 43. Paranasal sinus X-ray and physical examination showed sinusitis and re-growth of adenoid. Obesity was proved not to be a contributing factor because his BMI decreased to normal range (23.1 kg/m2) after diet control and regular exercise. Also, liver transaminase was normalized and fatty liver was disappeared on follow-up abdominal ultrasonogram. After treatment of sinusitis, symptoms were relieved with decreased AHI (8.5). This case suggests that simple adenotonsillectomy might not be the end of OSAS treatment in childhood. Patients who had adenotonsillectomy should be followed by subsequent PSG if symptoms recur. It is also important to be aware of risk factors in the recurrent OSAS for the proper intervention according to the cause.
