2.A Case of Prader-Willi Syndrome with Diabetes Mellitus.

Prader-Willi syndrome is characterized by infantile hypotonia, mental retardation, hyperhagia, hypogonadism and obesity. Approximately 60% of all PLW syndrome show an interstitial deletion of chromosome 15, 37% have apparently normal chromosome, and 3.6% have a variety of other abnormalities involving chromosome 15. Diabetes mellitus has been considered a component of PLW syndrome and the incidence is about 7%. We experienced a 17-year-old female who revealed mental retardation, hypogonadism, obesity, and non-insulin dependent type DM, compatible with Prader-Willi syndrome.

3.Calculated CT Volumes of Lacrimal Glands in Normal Korean Orbits.

PURPOSE: We used computed tomography (CT) scans to describe normal Korean lacrimal gland volume and lacrimal gland size and then examined their correlations with patient age. METHODS: CT scans were obtained in 213 orbits of 111 patients who underwent CT from January to August of 2013. Aquarius iNtuition (TeraRecon, Foster City, CA, USA) software was used to outline the lacrimal gland in consecutive axial slices and to calculate the volume. RESULTS: The mean volume of the lacrimal gland was 0.589 cm3 in right orbits (SD = 0.090), 0.583 cm3 in left orbits (SD = 0.289), 0.596 cm3 in males (SD = 0.083), and 0.575 cm3 in females (SD = 0.094). There was no significant difference in mean lacrimal gland volume according to laterality (p = 0.614) or sex (p = 0.102) (2-sample t-tests). We investigated mean lacrimal gland volume in 3 age groups. Mean lacrimal gland volume was 0.630 cm3 (SD = 0.080) for the 20 to 40 year old group, 0.553 cm3 (SD = 0.734) for the 41 to 60 year old group, and 0.544 cm3 (SD = 0.885) for the older than 60 years old group. There was an inverse relationship between gland volume and age (Pearson r = -0.384, p = 0.00). CONCLUSIONS: The mean volume of the lacrimal gland decreased with age and there was no significant difference between right and left orbits or between male and female patients.

Stabilization of the unstable spine created by the posterior decompression is as important as the decompression itself in the treatment of spinal stenosis. The purpose of this study is to evaluate the effectiveness of C-D pedicle screw fixation in stabilization after lumbar decompression, in reduction of spondylolisthesis and in restoration of the lumbar sagittal curvature. C-D pedicle screw fixation was performed in 102 spinal stenosis patients after posterior lumbar decompression and fusion during the period from March 1987 to December 1988. Their age ranged from 15 to 72 years with an average of 49.1 years. There were 34 males and 68 females. The follow up was from 6 to 21 months with an average of 12.5 months. The causes of spinal stenosis were degenerative in 50 patients, spondylolisthesis in 39, iatrogenic in 9 and degenerative lumbar scoliosis in 4. Objective clinical results showed significant improvement of claudication, SLR limitation, motor weakness, sensory and DTR changes in most patients. Following results were obtained from the study of C-D pedicle screw fixation after posterior decompression in lumbar spinal stenosis.1. C-D pedicle screws provide the secure fixation that allows early ambulation and shorter hospital stay. 2. C-D pedicle screws enable the reduction of spodylolisthesis at the time of posterior stabilization. 3. C-D pedicle screw fixation is successful in the restoration and maintenance of sagittal curvature of the lumbar spine. 4. C-D pedicle screw fixation enables the correction of scoliosis at the time of posterior decompression.

6.Analysis of the Flexor

As a new concept of resistive exercise, isokinetic exercise was developed during the early 1950's. In isokinetic exercise, the subject works at a fixed speed against variable and totally accommodating resistance. The isokinetic equipment has been used widely as a therapeutic modality in clinical setting, as a training and testing device for sports practicians, and as a method for scientific examination of postoperative status of muscles and joint function for orthopedists. In order to acquire the isokinetic normative data of the knee for clinical use, we examined the flexor and extensor muscle force of the knee joint in 160 subjects with Cybex II. The results were as follows; 1. Mean values of extensor strength were 76.04 ft.-1bs in right knee 75.45 ft.-lbs in left knee, 78.42 ft.-1bs in dominent hand side leg 73.84 ft.-1bs in non-dominent hand side leg, and 82.52 ft.-1bs in dominent leg 71.93 ft.-lbs in non-dominent leg. 2. Sex difference of extensor strength were 95.13 ft.-lbs in male 55.98 ft.-1bs in female, and that of flexor strength were 54.90 ft.-1bs in male 33.68 ft.-1bs in female. 3. Mean of position angle of maximal torque were between 58° and 65° for extensors, 35° and 45° for flexors. 4. Mean of total range of motion at working isokinetically with 60°/sec speed was 99°. 5. Mean values of extensor power were 54.43 ft.-1bs in male 30.63 ft.-lbs in female, and that of flexor power were 35.99 ft.-1bs in male 22.42 ft.-1bs in female. 6. Strength and power decreased as ages getting old, and there was significant diminish between 4th decade and 5th decade. 7. Power endurance had more difference when it was compared with occupation. Mean value of power endurance was between 12 times and 18 times.

7.Arthroscopic Ankle Arthrodesis.

Eleven cases of arthroscopic ankle arthrodesis were reviewed. Average age of patients was 59 years, 4 male and 7 female. Underlying diseases were osteoarthritis in 7 cases, rheumatoid arthritis in 3 cases and one post-traumatic arthritis. At the mean follow up of 15 months, fusion rate was 100%, mean time to complete union was 9.5 weeks. There were no complications such as nonunion, infection and skin problems. All patients stayed at the hospital just overnight after the surgery. Patients satisfaction was high and the functional clinical grade was excellent in 9 patients, good in 1 patient and fair in 1 patient. We concluded that arthroscopic technique was a simple and effective method for ankle arthrodesis in the selected patients and had significant advantages over the open technique.

8.A Case of Idiopathic Long QT Syndrome(LQTS).

The idiopathic long-QT syndrome is an infrequent inherited disorder, characterized by prolonged QT interval and by the occurrence of life-threatening tachyarrhythmia, particularly in association with emotional or physical stree. In its characteristic presentation, with obvious QT prolongation and stree induced syncope with torsades de points, the diagnosis is straightforward for physicians aware of the disease. But sometimes in cases of borderline QT prolongation and vague symptom, a correct diagnosis is delayed and frequently misdiagnosed as a seizure disorder. The mortality of untreated symptomatic patients with LQTS exceeds 20% in the year after their first syncopal episode and aapproaches 50% within 10 years. But this high mortality rate has been significantly reduced by the use of pharmacological or surgical antiadrenergic therapy or both. So early detection of the disease and antiadrenergic treatment sympathetic innervation to the heart. Second, there was intrinsic gene abnormality in the mechanisms responsible for cardiac repolarization. We report one case of LQTS in a 24 years old female patient who had been suffering from stree related syncope. Her initial EKG had prologed QT interval(588msec) and increased QT intrval dispersion(200msec). In the Holter monitor, long and short cycle sequence induced (pause-dependent) torsades de pointes was detected. She has been treated by beta-blocker and has maintained her condition without recurrence of the symptoms.

9.A Case of Respiratory Syncytial Virus Infection Presented with Apnea.

Respiratory syncytial virus(RSV) is currently implicated as the most common and otentially serious viral respiratory pathogen of young infants. Apnea in association with RSV infection occurred in about 18% of the infants. Premature birth and a young postnatal age were risk factors for development of apnea with RSV disease. We experienced a case of RSV infection in a 37 day-old premature girl. The presenting complaint was apnea, with respiratory signs and symptoms developing later. She required mechanical ventilation.

10.A Case of Generalized Hypertrichosis After Treatment with Topical Minoxidil.

There are few reports about generalized hypertrichosis after treatment with topical minoxidil , even though there are numerous reports about generalized hypertrichosis after treatment with oral minoxidil. We experienced a rare case of generalized hypertrichosis after treatment with 3% topical minoxidil in a 5-year-old boy. He visited our hospital for generalized hypertrichosis on face, trunk, arm after treatment with 3% topical minoxidil for 2 months due to alopecia areata. The laboratory examination including CBC, SMA and the level of corticotropin releasing hormone, luteinizing hormone, follicular stimulating hormone, cortisol, estradiol, testosterone were within normal limits. Two months after discontinuation of treatment with 3% topical minoxidil, the hair started to shed and six months after discontinuation of topical minoxidil, he showed almost complete resolution.
