2.A Case of Metastatic Angiosarcoma Diagnosed by Liquid-Based Preparation: Peculiar Cytoplasmic Changes.

Angiosarcoma with predominantly epithelioid features is a rare soft tissue neoplasm and the interpretation of its cytopathologic findings may be difficult. We report a case of metastatic angiosarcoma with predominantly epithelioid features diagnosed by liquid-based cytology. The cytopathologic findings in this case differed from those of the conventional preparation and we found a clean background, no hyperchromatic nuclei and several cytoplasmic changes, including intracytoplasmic vacuoles with peculiar shapes, juxtanuclear condensation and perinuclear clearing. Identification of these changes using liquid-based cytology supplemented with immunochemistry may be helpful in reaching a correct cytopathologic diagnosis.

3.Mucinous precursor lesions of mucinous carcinoma in breast: Incidence and histopathologic features.

OBJECTIVES: Columnar cell lesion (CCL), atypical ductal hyperplasia (ADH) and ductal carcinoma in situ (DCIS) may be premalignant lesion of mammary invasive carcinoma. A few recent investigators reported that the precursor lesions exhibited mucin production and they might be potential precursor lesion for mucinous carcinoma (mCA). This study aims to investigate the incidence and histopathologic characteristics of mucinous precursor lesions, including mucinous DCIS (mDCIS) and mucinous CCL (mCCL). METHODS: We retrospectively analyzed invasive carcinomas with mucin. Cases were grouped into three subgroups: pure mCA, mixed mCA, and invasive carcinoma of no special type with mucin production (IC of NST-m). Precursor lesions were evaluated with PAS and alcian blue staining. RESULTS: Total 27 cases of invasive carcinoma with mucin were analysed and classified as 18 pure mCA, 7 mixed mCA, and 2 IC of NST-m. mDCISs were found in 12 pure mCA, 4 mixed mCA and 2 IC of NST-m. mCCLs were found in 7 pure mCA and 2 mixed mCA. Majority of mucin was identified in both cytoplasm and ductal lumen, while some tumors exhibited only cytoplasmic mucin. We also observed three patterns of mDCIS classifiable by location of mucin and architecture of tumor cells. CONCLUSIONS: Cytoplasmic mucin suggested that mucinous feature of precursor lesions in the vicinity of mCA might not be a passive morphologic finding but be involved in development of mCA.

5.A Psychiatric Study on Hahn.

OBJECTIVE: This study aimed at identifying the etiological background, symptomatic expressions, defense styles and coping strategies related to hahn. METHOD: The subjects were given a questionnaire for demographic data, life experiences and psychological and physical symptoms, Bond's questionnaire of defense style, and Weisman's scale for coping strategies and scales for severity of hahn and hwabyung. The subjects were 110 normal healthy adults and 151 patients with neurotic disorders, diagnosed with ICD-9. RESULTS: More vivid hahn was found in the married women, the less educated, and those from a low socio-economic state. In the area of etiological life experiences, hahn is significantly related with an unhappy and frustrated marital life, unsatisfactory filial Piety, Poor family background, poverty, little education, a hard life since childhood, betrayal, failure in business, personality problems, injustice in society and resignation to fate. Hahn, unresolved and accumulated, was believed to cause various psychological and physical illnesses. In many cases hahn was corhad been dealt with by simple suppression and being dependent on religious faith or sharing and communicating with people. More people considered hahn as one of the etiologies of hwabyung, a chronic anger syndrome. Hahn of patients with hwabyung was more serious than that of those without hwabyung. Symptoms of hahn were - in addition to various symptoms of depression, anxiety and other neurotic conditions or somatization - a mass in the epigastrium, sighing, something pushing-up in the chest, paranoid ideas and hyprochondria sis nor hygochondriacal ideas. On the other hand, hypersensitivity and irritability were less significant. Defense styles and coping strategies related with hahn were somatization, splitting-projection, passive-aggressiveness, incorporation, primitive idealization, stimulus reduction, self-pity and shared-concerns and dependency. CONCLUSION: These results suggest that hahn is an unique emotional condition which is characterized by culture-related somatization symptoms, paranoid ideas and hypochondriasis, in addition to general neurotic symptoms such as depression and anxiety and related somatic symptoms.

6.A Case of Impetigo Herpetiformis.

Impetigo herpetiformis is a rare pustular disorder that primarily affects pregnant women. A 31 year-old woman at 37 weeks and 4 days' gestation of 3rd pregnancy presented with pruritic multiple erythematous pustules and crusts on the trunk and both antecubital fossa for 3 months. She had had similar lesions in her 1st and 2nd pregnancy, which subsided after delivery without any fetal or maternal complication. She had no personal or familial history of psoriasis. Histopathologic findings showed subcorneal pustules with numerous neutrophils and spongiform pustule of Kogoj. Bacterial culture from pustules was sterile. After Caesarean section at 38 weeks' gestation, a healthy female neonate was delivered and her skin lesion resolved spontaneously.

7.The olloimrnunizotion rote of onti-c concurrent with onti-E in R1R1 potients.

Anti-E and anti-c is one of the clinical significant irregular antibodies developing a delayed hemolytic transfusion reaction and hemolytic disease of the newborn. Since anti-c occurs frequently with anti-E in immunized people whosoe cells are E-and c-, it has been recommended to select blood of the patient's own R1 phenotype for transfusion, even when the presence of anti-c cannot be demonstrated in his/her serum. To determine the utility of this approach, we reviewed the blood bank laboratory records of patients identified anti-E and anti-c in his/her serum in Severance hospital over a 12 year period (1985-1996). During the 12-year period of study, 53 patients were identified with anti-E and/or anti-c; 30(56.6%) patients had anti-E alone, 6(11.3%) had anti-c, and 17(32.1%) had both. Thirty eight of forty two patients whose Rh-hr phenotypes were tested were R1R1. Of these 38 R1R1 patients, 16 patients (42.1%) presented with anti-c concomitant with anti-E. Ouru study showed that the incidence of antni-c in R1R1 patients with anti-E is similar to that of studies reported in Caucasian groups. We highly suggest the transfusion protocol of prophylactic use of c negative blood for R1R1 patients with anti-E. Furthermore, because anti-c may be present in concentrations too low to be detected, the enzyme technique is recommended in parallel with standard serologic methods for the identification of irregular antibodies.

8.A Case of Connubial Contact Dermatitis due to Rhus.

Allergic contact dermatitis that is attributable to contact with environmental sensitizers boine by a second individual, especially in marital or sexual relationships, has been described as connubial contact dermatitis. Severely pruritic multiple erythematous plaques, vesicles and bullae in a 35-year-old woman occurred on the face, posterior area of the neck, breasts, buttock, and both upper arms. No direct contact to allergens was obtained. However her husband had eaten some cooked sprouts from a lacquer tree for lunch at a restaurant the day before her eruption, then he caressed her that night. A patch test with extracts of lacquer tree in ethanol(2%), crushed extracts of lacquer tree (as is), and urushiol(0.002% pet.) on the patients back showed strong positive reactions.

Pseudomyxoma peritonei is a poorly understood condition characterized by mucinous ascites and multifocal peritoneal mucinous tumors. Pseudomyxoma peritonei is most commonly associated with mucinous tumors of the ovary and appendix. When two lesions are morphologically similar, it is difficult to determine whether one reflects a metastasis or whether the tumors are independent. Many studies concluded that most cases in which both the appendix and ovary were involved were primary appendiceal tumor with secondary involvement of the ovary, but we have experienced one case of pseudomyxoma peritonei originated from independent primary borderline mucinous tumors of the ovary and appendix. So we report this case with the brief review of literatures
