1.Optimization on Formulation of Controlled-onset Extended-release Tablets of Nicorandil by Central Composite Design-response Surface Method

Objective To optimize the formulation of controlled-onset extended-release tablets of nicorandilby central composite design-response surface method. Methods The tablets containing nicorandil were prepared bydry-compression coating technique.The influence factors included the amount of HPMC, the amount of EC, and the ratio of lactose/MCC in coating film. The evaluation parameter was Q4 h and Q5 h . The possibly optimal formulation was predicted by response surface method. Results The OD was simulated using second-order polynomial equation and the r2was 0.970 1.Optimal prescription was as follows:HPMC 89.96 mg, EC 46.21 mg, Lactose/MCC 1.87.Bias between the observed and predicted values of the OD was within 0.43%. Conclusion The controlled-onset extended-release tablets of nicorandil can release drug quickly in vitro at the predetermined time.Central composite design-response surface method can be used to optimize the formulation and the model developed in this study was proved highly predictable.
2.Combining acupuncture and copper-tube moxibustion for 39 cases of recurrent peripheral facial paralysis

Objective:To observe the clinical effect of acupuncture combined with copper-tube moxibustion on peripheral facial paralysis. Methods:A total of 39 recurrent Bell’s palsy patients were treated with acupuncture plus self-made copper-tube moxibustion, once a day. Ten days made up a course of treatment. The patients were treated for 4 courses of treatment. There was a 3-day interval between two courses. The therapeutic efficacies were statistically analyzed after 4 courses of treatment. Results:Of the 39 cases, 18 cases obtained recovery, 15 cases got improvement and 6 cases failed. The total effective rate was 84.6%. Conclusion:Acupuncture combined with copper-tube moxibustion is effective for recurrent peripheral facial paralysis.

4.The effects of early rehabilitation on hemiplegic patients caused by stroke

Objective To study the effect of early rehabilitation on motor function of hemiplegic patients after cerebral infarction. Methods Forty two patients were divided into two groups randomly: a rehabilitation group and a control group. Both groups were given the same internal medical treatment to stabilize the patients conditions in the early stage. In addition, the patients in the rehabilitation group were given early formal rehabilitation training, while those in the control group were given no formal rehabilitation training. The results of therapy were assessed by using SIAS method (Stroke Impairment Assessment Set). 8.Hourglass Pattern:a Safe and Effective Method of Ultrasound-guided Fascia Iliaca Compartment Block

Objective To confirm the efficacy of the “Hourglass Pattern”of ultrasound-guided fascia iliaca compartment block (FICB). Methods Approved by the medical ethics committee,we selected 30 patients undergoing selective total hip arthroplasty (THA ) in our hospital between January and May 2014.Before neuraxial anesthesia,ultrasound-guided FICB was performed.After identification of the “Hourglass Pattern”(internal oblique muscle and the sartorius muscle),a catheter was inserted. The sheath between the fascia iliaca and the iliacus muscle was distended with 40 ml of 0.2% ropivacaine,and then a continuous infusion of ropivacaine at a rate of 10 ml /h was administered for 48 h.Loss of sensation in the distribution areas of the femoral nerve and lateral femoral cutaneous nerve within 30 min was recorded.Patients were interviewed at 4 h,8 h,12 h,24 h,36 h and 48 h after block for pain intensity.Patients’ satisfaction scores at 48 h after block and the occurrence of adverse events (infection,local anesthetic toxicity,hemorrhage and hematoma)was recorded. Results The successful rate of the block was 100% without any adverse events.The median of numerical rating pain scale (NRPS)was below 4 points at 4 h,8 h,12 h,24 h,36 h and 48 h after block,and the patients’satisfaction score was (8.0 ±0.9 )points at 48 h after block.No adverse events occurred. Conclusions Ultrasound-guided FICB by using the “Hourglass Pattern”technique can offer effective analgesia after THA.It’s a safe method and can be easily performed.
9.Combined posterior cruciate ligament with posterolateral corner injuries of the knee

[Objective]To explore the clinical characteristic of combined posterior cruciate ligament (PCL) with posterolateral corner (PLC) injuries,and evaluate the technique and outcome of arthroscopic single bundle reconstruction of PCL and reconstruction of PLC with posterior half of biceps femoris tendon.[Method]From Dec. 2001 to Dec. 2004,14 patients (14 knees) with severe posterior and posterolateral rotatory instability were treated surgically in our department,all of them presented III degree positive results of posterior drawer test,positive varus stress test and more than 10 degree of external rotation of the affected knees compared with the intact knees.PCL tear and abnormal opening of posterolateral compartment were seen in all of the 14 knees under arthroscopy.Of them,2 knee were combined with common peroneal nerve injury.All the damaged PCLs were reconstructed with single bundle of autogenous quadrupled hamstring tendons under arthroscopy,and PLCs were reconstructed with the posterior half of biceps femoris tendon tenodesis.[Result]No severe complications occurred in early stage after operation in the 14 patients.All of them were followed up from 12 to 33 months with an average of 21.14?7.26 months.Posterior stabilities were recovered significantly,varus stress test was negative at 30 degree of flexion and full extension,less than 10 degree of external rotation compared with the intact knee in all of the patients.Lysholm score was remarkable improved from 40~60 (mean 47.1?8.25) preoperatively to 70~95 ( mean 86.79?7.23) at follow up(P

Objective To study the effect of urapidil on the levels of 4 kinds of plasma neuropeptides : endothelin(ET),neuropetid Y(NPY),calcitonin gene-related peptide(CGRP) and neurotensin(NT) in patients with hypertension.
