5.A Case of Leptomeningeal Carcinomatosis on Superficial Spreading Carcinoma of the Stomach.

Meningeal carcinomatosis is an important neurological complication of systemic cancer, and is increasingly being recognized as a cause of reurologic disability in life. Also there is evidenee that the incidence of this form of metastasis is increasing. We reported a case of 52-year-old female patient, who was diagnosed as early gastric cancer by biopsy under gastrofiberscopy. Other Lab. findings were all normal range except high alkaline phosphatase level. And then performed Billroth II operation. On 7th post-operative day, she was encountered severe headache and insomnia. Abnormal neurologic signs and subjective symptoms were developed and progressed, Repeated lumbar punture was done and CSF was examed. There findings were abnormal, but no specific finding of certain disease. She died of severe neurologic deficit and sudden respiratory arrest. Autopsy was done, and confirmed leptomeningeal carcinomatosis.

6.Patch Testing in Nummular Eczema: Comparison of Patch Test Resultsbetween Nummular Eczema and Atopic Dermatitis.

BACKGROUND: Nummular eczema, which is also known as discoid eczema, is defined by its clinical appearance as coin-shaped, circular, or oval lesions with a definite border. The etiology of nummular eczema is obscure, and many causative factors have been proposed, but there are only a few studies of the relevance of contact allergy in nummular eczema in Korea. OBJECTIVE: The purpose of this study was to investigate the role of allergic contact dermatitis in nummular eczema patients compared with atopic dermatitis. METHODS: A total of 86 patients were enrolled in this study. Patients combined with atopic dermatitis and nummular eczema were classed as atopic dermatitis. The group with atopic dermatitis was 32 patients. The group with nummular eczema was 54 patients. We performed patch tests on both groups, and evaluated their clinical features and the results of the patch testing. RESULTS: The patients comprised of 49 males and 37 females. The mean age of group with atopic dermatitis was 21.3 years, and the mean age of group with nummular eczema was 44.2 years. The distribution by age was most prevalent at 10~19 years for the group with atopic dermatitis group and 40~49 years for the group with nummular eczema. The predominant sites of the lesions were the arms (21.0%), trunk (21.0%), legs (16.3%), widespread on the body (15.1%), hands (13.9%), feet (7.0%), face and neck (5.8%). Sixty seven (77.9%) out of 86 patients showed a positive reaction to one or more allergens. The highest sensitization rates were found with: nickel sulphate (45.3%), cobalt chloride (29.1%), potassium dichromate (20.9%), thimerosal (17.4%), neomycin sulphate (15.1%), thiuram mix (14.0%), formaldehyde (14.0%), colophony (12.8%), 4-phenylenediamine mix (11.6%), fragrance mix (10.5%). Comparing the atopic dermatitis and nummular eczema groups, there was no significant difference in the positivity for patch test allergens and frequent antigens. Comparing with clinical manifestation between the group with positive reaction and the group with negative reaction to the patch test in nummular eczema and atopic dermatitis, in the group with positive reaction of patch test, the severity of disease increased. CONCLUSION: This study shows that contact sensitivity is relatively common both with nummular eczema and atopic dermatitis. But, when there is no difference in the positive rate of antigens in patch test, both groups show high positive rate of metal antigens. Also nummular eczema patients with consistent and recurrent symptoms, the possibility of allergic contact dermatitis should be taken into consideration and a patch test must be performed.

7.BCGitis of scrofuloderma-like lesions.

A 13-year-old male presented with a walnut-size tender swelling of the left supraclavicular area. The lesion developed 4 months after BCG vaccination in a left. upper arm location. The supraclavicular lesion gradually increased in size and became suppurative witii resultant sinus formation. Histopathological examination of the nodular lesion showei3 foci of epithelioid granulomas with caseation necrosis, and several Langhans' type giant cells. After a diagnosis of BCGitis was made, the patient was, treated with isoniazid an l rifampin for two months without much benefits. Surgical excision of the lesion were performed to expedite clinical regression. After six months of antituherculous chemctherapy, a clinical remission was evident. Scrofuloderma-like BCGitis as well as other cutaneous complications of BCG vaecination are not uncommon. Thus, the practicing dermatologist should be aware of these complications, for correct diagnosis and treatment.

9.Bleeding from Dieulafoy's Vascular Malformation of the Proximal Ileum: A case report .

Dieulafoy's vascular malformation is a rare cause of massive gastrointestinal bleeding. Most often it occurs in stomach within 6 cm from the gastroesophageal junction. Only a few cases have been reported to occur in the small intestine and colon. Occasionally, Dieulafoy's lesion of small intestine is difficult to recognize because of rarity, a paucity of symptoms and negative findings on barium studies. Therefore, this lesion needs to be considered in a patient with massive lower gastrointestinal bleeding. We report a case of Dieulafoy's vascular malformation in ileum 2 m proximal to ileocecal value in a 41-year-old woman who visited emergency clinic because of hematemesis, dizziness and vomiting. Small intestine revealed a wide-caliber artery within the submucosa showing intimal thickening, medial muscular hypertrophy and thrombosis.

10.Relationship between Radiation Induced Activation of DNA Repair Genes and Radiation Induced Apoptosis in Human Cell Line A431.

PURPOSE: The purpose of this study was to evaluate the relationship between radiation-induced activation of DNA repair genes and radiation induced apoptosis in A431 cell line. MATERALS AND METHODS: Five and 25 Gys of gamma radiation were given to A431 cells by a Cs-137 cell irradiator. Apoptosis was evaluated by flow cytometry using annexin V-fluorescein isothiocyanate and propidium iodide staining. The expression of DNA repair genes was evaluated by both Northern and Western blot analyses. RESULTS: The number of apoptotic cells increased with the increased radiation dose. It increased most significantly at 12 hours after irradiation. Expression of p53, p21, and hRAD50 reached the highest level at 12 hours after 5 Gy irradiation. In response to 25 Gy irradiation, hRAD50 and p21 were expressed maximally at 12 hours, but p53 and GADD45 genes showed the highest expression level after 12 hours. CONCLUSION: Induction of apoptosis and DNA repair by ionizing radiation were closely correlated. The peak time of inducing apoptosis and DNA repair was 12 hours in this study model. hRAD50, a recently discovered DNA repair gene, was also associated with radiation-induced apoptosis.
