1.Accumulation of Thallium-201 in Hemorrhagic Cerebral Infarction.

Thallium-201 brain SPECT is utilized in the diagnosis of brain tumor especially in cases where CT or MRI findings alone cannot differentiate malignant lesion from benign. Recently we came across two cases of positive T1-201 brain SPECT in clinically suspected brain tumor patients that turned out to be hemorrhagic cerebral infarction instead on biopsy. The findings in these cases demonstrate that thallium-201 accumulation may occur by the breakdown of the blood-brain barrier and phagocytic cell infiltration in the liquefaction stage of infarction.

2.Clinical Aspect of Central Serous Retinopathy.

The author surveyed on the ration of central serous retinopathy (C.S.R.) cases among total out patient, distribution of sex, distribution of age, sites of affected eye, and visual acuity in 271 cases of central serous retinopathy from March, 1973 to April, 1977. Fluorescein angiography was performed in 71 cases among them. Leakages of fluorescein were analysed by pictures. The results are obtained as follows: 1. The eyes of C.S.R. were observed in 0.75% among total out patients. 2. As to the distribution of sex, male were observed in 71.4%. 3. Among 271 cases, Unilateral affected cases (83.4%), and there were no difference between right and left eyes. 4. As to the distribution of age, 4-th decade were most frequently affected in 35.4%. 5. The cases were classified by leaking pattern as followings; Type I: Leaking was unremarkable. Type II: Leaking point appeared at early arterial phase and fades out gradually. Type III: Learking point appeared at early arterial phase and increased in density gradually, but the size of leaking area wes not enlarged. Type IVa: Leaking point appeared at early arterial phase and increase concentrically in the density and size. Type IVb: Leaking point appeared at early arterial phase and increased vertically (mushroom-shaped) in its size and density. 6. Type IVa in leaking pattern was most frequently observed. 7. A single leakage was found in 39.5% of all cases. 8. The leakages of fluorescein were found most frequently in the upper area particullary in the upper nasal area of the macula.

4.A Conceptual Analysis of Korean Elders' Yangsaeng in Nursing

PURPOSE: This study tries to analyze the concept of Yangsaeng in the Korean aged, with focus on nursing. METHODS: Rodgers'evolutionary approach was used to identify the common definitions, attributes, antecedents, and consequences of the Yangsaeng concept for the aged. Literature search was conducted at the NAL, NDSL, RISS, ProQuest, PubMed, AMED, and MEDLINE databases from 2004 to 2016, using the keywords “Yangsaeng,” “health promotion,” “health management,” “traditional oriental methods,” and “traditional oriental nursing.”Finally, 20 relevant articles were selected and thoroughly reviewed. RESULTS: The analysis shows that the model of Yangsaeng for the Korean aged is that of personal adaptive-capacity, totality, challenging, and coping resource and includes physical, social and psychological health and wellness. CONCLUSION: This work provides some implications on the development of nursing intervention related with Yangsaeng for the Korean aged, and suggests the implementation of such intervention in the practice of Yangsaeng for the Korean aged. Therefore, a better understanding of the Korean aged and Yangsaeng within the context of nursing can be achieved.

6.Analysis on the Key Words related to Healthcare Issues of the Prevention and Control of COVID-19 in Major Korean Newspapers, 2020

Background:
s: This study was performed to analyze the main key words of newspaper articles related to COVID-19 in 2020 for each category of quarantine measures according to the epidemic period of COVID-19.
Methods:
We analyzed articles related to COVID-19 in three major newspapers of Korea between February 17 and December 31, 2020. We targeted the front page articles on mondays and thursdays. The analysis of the relationship between the two variables was confirmed through the chi-square test.
Results:
As a result of analyzing the main key words for each category of quarantine measures, non-pharmaceutical intervention were the most common at 54.3%, followed by 3Ts(test, tracing, treatment and vaccine) at 31.9%. In the category of non-pharmaceutical intervention, social distancing was the most common at 33.9%. In the categories such as 3Ts(test, tracing, treatment) and vaccine, diagnostic tests were the most common at 41.8%.
Conclusions
It was identified that non-pharmaceutical intervention were the most common, and there was a difference in the reporting of main key words by category of quarantine measures for each epidemic period related to COVID-19 in 2020.
7.Genetic Polymorphism of Cytochrome P4501A1 Exon 7 and Glutathione S-Transferase M1 in the Head and Neck Squamous Cell Carcinoma Patients.

BACKGROUND AND OBJECTIVES: An individual difference in susceptibility to chemically induced carcinomas is in part ascribed to genetic differences of metabolic activity of environmental procarcinogens. The cytochrome P450 family (CYPs) and glutathione S-transferase (GST) have been reported to be associated with human cancers related with smoking. The purpose of this study was to determine the frequencies of the genotypes of CYP1A1 and GSTM1 genes in healthy control of Koreans and to identify the high-risk genotypes of these metabolic genes in head and neck cancer patients. MATERIALS AND METHOD: The genetic polymorphism of CYP1A1 exon 7 and GSTM1 genes were analysed in a group of 115 healthy Koreans and 107 head and neck squamous cell carcinoma patients using allelic-specific polymerase chain reaction. RESULTS: The genotypes of CYP1A1 exon 7 (Ile/Ile, Ile/Val and Val/Val) were 59.1%, 36.5% and 4.4%, respectively, in the healthy control group, and 57.0%, 31.8% and 11.2%, respectively, in the cancer patients . The distributions of GSTM1 [GSTM1 (-), GSTM1 (+)] in healthy control group were 46.1%, 53.9% respectively, and 53.3%, 46.7%, respectively, in the cancer patients. The relative risk (odds ratio) for combination of CYP1A1 Val/Val and GSTM1 (-) genotype was estimated to be 5.17, taking the risk of combined genotype Ile/Ile and GSTM1 (+) as a reference in cancer patients. CONCLUSION: These results suggest that the genetic polymorphisms of CYP1A1 exon 7 and GSTM1 were an important major factor in determining the individual susceptibility to head and neck squamous cell carcinoma in Koreans.

8.The impact of occupational experience on cognitive and physical functional status among older adults in a representative sample of Korean subjects.

OBJECTIVE: This study investigated the impact of occupation on cognitive and physical function within an occupational window of the past 15 years using a nationally representative sample in Korea. METHODS: A total of 4,408 subjects aged 60 and older were selected from the Korean Longitudinal Study of Aging (KLoSA). Functional health was evaluated using the Korean versions of the Mental State Examination (K-MMSE), Instrumental Activities of Daily Living (K-IADL), and Activities of Daily Living (K-ADL) measures. RESULTS: The prevalence of lower function was higher among women than among men, and employed persons had better cognitive and physical function compared with the retired and unemployed. Longer work duration during the past 15 years significantly and positively affected all measures of function in men, whereas it only improved physical function in women. Manual laborers exhibited improved functional capacity with longer work durations over the past 15 years, although they presented lower functional scores compared with non-manual laborers. There was a curvilinear relationship of work duration with cognitive and physical function among men and manual laborers. CONCLUSION: In our population, longer work duration, especially for men and for manual laborers, appears to be a significant contributor to the cognitive and physical function of older people.

9.Hyponatremia-Induced Seizure Following Polydipsia in a Schizophrenic Patient.

A 29-year-old man with schizophrenia was admitted to the emergency department in a stupor following the first generalized tonicclonic seizure. He had drunk about 1.5 L of Coke and 2 L of water each day for 1 week without eating any other food. A laboratory evaluation revealed severe hyponatremia (110 mEq/L), and we diagnosed a hyponatremia-induced seizure. As polydipsia and hyponatremia are not uncommon in schizophrenics, hyponatremia-induced seizure should be included in the causes of seizure in schizophrenics.

10.Effectiveness of Drain Insertion and Irrigation in the Treatment of Septic Arthritis of the Knee under Local Anesthesia

Purpose:
Septic arthritis of the knee is an orthopedic emergency that requires early diagnosis and surgical treatment. This study examined the effectiveness of drain insertion and irrigation in the treatment of septic arthritis of the knee under local anesthesia.
Materials and Methods:
A retrospective study was conducted on nine cases (eight patients) diagnosed with septic arthritis of the knee from September 2017 to February 2020 and treated with drain insertion and irrigation under local anesthesia. After penetrating through the superolateral portal to the superomedial portal and inserting the drain, daily irrigation of approximately 3 L of normal saline was done. The following were investigated: age, sex, underlying disease, cause, degree of osteoarthritis, time from diagnosis to surgery, duration of hospitalization, duration of normalization of C-reactive protein, and smear and culture.
Results:
The initial white blood cell count of joint fluid was 71,472±51,667/mm3 (32,400–203,904/mm3 ), and polymorphic leukocytes were 91.1%±2.6% (86%–95%). The average time from diagnosis to surgery was 8.3±1.3 hours (6–10 hours), and the irrigation period was 8.2±3.2 days (4–15 days). The average length of hospitalization was 20.8±8.7 days (9–37 days). There was no reoperation or recurrence. Smear and culture tests were not identified.
Conclusion
In the treatment of septic arthritis of the knee, the insertion of a drain tube and irrigation under local anesthesia is a relatively fast and simple method to reduce pain by repetitive draining of purulent joint fluid and can be used as an alternative treatment for patients with a risk of general or spinal anesthesia.
