Accompanying to the development of the knowledgment of mechanism and pathology of recurrent dislocation of the shoulder, accurate diagnostic tools to detect that pathology have been developed. During the period from May 1990 to May 1992, we treated 28 cases of recurrent anterior dislocation of the shoulder. Twenty-eight cases were examined with C.T. arthrography before the operation. Among the 28 cases, we identified the Bankart lesion in 20 cases by C.T. arthrography, and in 21 cases intraoperatively. We concluded that the C.T. arthrography is an accurate method for confirming the clinical diagnosis of recurrent dislocation of the shoulder and to be helpful in detecting the Bankart lesion and its severity or pathologic findings.

4.Anxiolytic Effect of Ginseng Total Saponin.

Ginseng root, as a folk medicine, has been used in for eastern countries for thousands of years. Ginseng extract has been shown to have a variety of effects on the activity of the central nervous system, promoting simulation as well as inhibition of the cortical activity. A survey of the relevant literatures has indicated that the putative anxiolytic activity of red ginseng has not been scientifically investigated. Therefore, the present study was designed to assess anxiolytic effect of ginseng total saponinis(GTS). The putative anxiolytic effects of several fractions of GTS were investigated in mice using an elevated plus maze paradigm. Single dose administration of TS Fr.- I showed anxiolytic action in mice. Anxiolytic effect induced by TS Fr.-I was similar to that induced by diazepam. TS Fr.-II, TS Fr.-III and TS Fr.-IV did not show the anxiolytic action compared with that of TS Fr.-I. It was suggested that regulation of GABAergic neurotransmission may be important in the action of GTS. The Interaction of GTS fractions with benzodiazepine receptor was performed using rat cortical membranes. GTS inhibited the binding of [3H] Rp 15-1788 on the benzodiazepine receptor. Among from TS fractions, the binding activity of GTS in the TS Fr.-IV was highest, which did not show the anxiolytic activity. From these results, we conclude that GTS has anxiolytic action, and the is not related to benzodiazepine receptor binding activity.

5.A comparative study on reliability of the root parallelism of the posterior teeth projected on the orthopantomogram with the 45 degrees oblique cephalogram.

At present, many orthodontists regard the root parallelism of the posterior teeth important not only in diagnosis and treatment planning but also for predicting posttreatment stability. To evaluate it, they usually refer to the orthopantomogram. At this study, 97 samples were collected from students of Yonsei University, who have well-proportioned face, Angles's class I canine & molar relationships and no crown axis deviation of the posterior teeth including canines. Reliability of the root parallelism observed from orthopantomogram was evaluated by comparison with 45degrees oblique cephalogram. The results were as follows : 1. In comparing the differences between anglular measurements in 45degrees oblique cephalogram & orthopantomogram with 5degrees, those to mandibular plane were significantly less than to occlusal plane in number of items which showed less differences than 5degrees. 2. Compared the root parallelisms in the orthopantomogram with those in 45degrees oblique cephalogram with 1% significance level, parallelism between upper canine & 1st premolar, lower canine & Ist premolar, lower 1st premolar & 2nd premolar, and lower right 2nd molar & 3rd molar showed statistically significant differences. 3. When the significance between the differences of the root parallelism between above two kinds of film and 5degrees was verified by two sided paired t-test, more or less large difference was shown between lower right 2nd molar & 3rd molar, a little larger than 5degrees between lower canine & 1st premolar, smaller at the rest of them. 4. In 45degrees oblique cephalogram, lower canine & 1st premolar showed convergent root arrangements each other, while in orthopantomogram they were divergent each other. All the others except them showed convergency on the upper, divergency on the lower in both films.

6.Role of Tc-99m Ciprofloxacin Imaging in the Diagnosis of Bacterial Prostatitis.

PURPOSE: With traditional laboratory tests it is often difficult to differentiate chronic bacterial prostatitis (CBP) from chronic pelvic pain syndrome (CPPS), resulting in inappropriate antibiotic therapy, without definite evidence of infection. Tc-99m Ciprofloxacin imaging was developed to discriminate an infection from inflammation. The value of this imaging for differential diagnosis of CBP was investigated. MATERIALS AND METHODS: The study included 104 patients diagnosed as CBP or CPPS by traditional laboratory tests and 4 normal subjects and 2 patients with acute prostatitis or cystourethritis as positive and negative controls, respectively. Patients that had received antibiotic therapy within the previous 6 weeks were excluded. Single photon emission computerized tomography (SPECT) images were obtained 3 hours after an injection of Tc-99m Ciprofloxacin. The results of the imaging were compared with those of the 4-glass test for identification of microorganism. RESULTS: On the images, negative uptake was shown in all normal subjects, while strong hot uptake was shown in the entire prostate of the patient with acute prostatitis and the entire urethra of the patient with acute cystourethritis. Based on traditional laboratory tests, 104 patients were classified as CBP (n=32) and CPPS (n=72). Nine of the 32 CBP patients (28%) showed negative uptake in the prostate, and 47 (65%) of the 72 CPPS patients showed hot uptake in the prostate. CONCLUSIONS: Tc-99m Ciprofloxacin imaging may have potential as a diagnostic technique for the discrimination of chronic bacterial prostatitis that cannot be identified by traditional laboratory tests.

7.Myocardial Protective Effect of Tezosentan, an Endothelin Receptor Antagonist, for Ischemia-Reperfusion Injury in Experimental Heart Failure Models.

The myocardial protective effects of endothelin antagonist in ischemic cardiomyopathy (ICMP), doxorubicin-induced cardiomyopathy (DOX) and pressure-overload hypertrophy by transverse aortic constriction (TAC) models have been predicted to be different. The objective of this experiment, therefore, is to evaluate the myocardial protective effect of tezosentan, an endothelin receptor antagonist, in various experimental heart failure models. Sprague-Dawley rats (6-8 weeks old, 200-300 g) were randomized to three experimental groups (n=30 each): ICMP; DOX; and TAC group. Each of these groups was randomly assigned further to the following subgroups (n=10 each): sham-operated ischemia-reperfusion subgroup (SHAM); tezosentan treated ischemia-reperfusion subgroup (Tezo); and tezosentan non-treated ischemia-reperfusion subgroup (N-Tezo). Total circulatory arrest was induced for 1 hr, followed by 2 hr of reperfusion. The left ventricular developed pressure, peak positive and negative first derivatives, and coronary blood flow were significantly different (P<0.05) among the SHAM, Tezo, and N-Tezo subgroups of the ICMP group at 30 min of reperfusion, but there were no statistically significant differences among the subgroups of the DOX and TAC groups. In conclusion, tezosentan, an endothelin receptor antagonist, showed myocardial protection effects only on the ischemic cardiomyopathy rat model, but not in the non-ischemic heart failure rat models.

8.Distribution of 5-Fluorouracil-14C in Body Tissues after Systemic and Regional Administration in Gastric Cancer.

This study was to determine which of two routes of administration of 5-fluorouracil (5-FU) is more effective, by measuring the radioactvity in the body tissues of gastric cancer patients after the administration of 5-FU-l4C via the systemic intravenous and the regional intra-arterial routes. After the drug was administered intravenously in one group of patients, and intra-arterially in another; samples of portal venous blood, the liver, the lymph nodes, and the normal and the cancerous tissues of the stomach were obtained. The radioactivities of the samples were measured, and it was found that those of the regional lymph nodes, the liver, and the normal and the cancerous tissues of the stomach were much higher in the latter group. The regional intra-arterial routes is the more effective way to administer 5-FU in patients with stomach cancer.

9.Bone response of two different surface titanium subperiosteal implants: anodized surface, ibad ha coating surface.

STATEMENT OF PROBLEM: In case of poor bone quality or immediately loaded implant, various strategies have been developed focusing on the surface of materials to improve direct implant fixation to the bone. The microscopic properties of implant surfaces play a major role in the osseous healing of dental implant. PURPOSE OF STUDY: This study was undertaken to evaluate bone response of ion beam-assisted deposition(IBAD) of hydroxyapatite(HA) on the anodized surface of subperiosteal titanium implants. MATERIAL AND METHODS: Two half doughnut shape subperiosteal titanium implants were made. The control group was treated with Anodized surface treatment and the test group was treated with IBAD of HA on control surface. Then two implants inserted together into the subperiosteum of the skull of 30 rats and histological response around implant was observed under LM(light microscope) and TEM(transmission electron microscope) on 4th, 6th and 8th week. RESULTS: Many subperiosteal implants were fixed with fibrous connective tissue not with bony tissue because of weak primary stability. The control group observed poor bone response and there was no significant change at any observation time. However the test group showed advanced bone formation and showed direct bone to implant contact under LM on 8th week. The test group observed much rER in the cell of osteoblast but the control group showed little rER under TEM. CONCLUSIONS: The test group showed better bone formation than the control group at the condition of weak primary stability. With these results IBAD surface treatment method on Anodized surface, may be good effect at the condition of weak primary stability.

10.Present Status of Human Paragonimiasis and Intestinal Parasitic Infection in Bokildo (Islet), Korea.

BACKGROUND: Bokildo (Islet) has been well known as an endemic area of paragonimiasis in Korea, and still crayfishes caught from this area are used as source of intermediate host of experimental paragonimiasis. For the reason, this study was carried out to elucidate the present status of human paragonimiasis in this region and intestinal parasitoses were studied at the same time. METHODS: From June 9 to June 12, 1999 authors visited Buyong-ri and Buwhang-ri which are located in the endemic area. All possible inhabitants including nursery children, primary and middle school children were examined by intra-dermal skin test with veronal buffered saline (VBS) antigen of Paragonimus westermani and Clonorchis sinensis. Sputa were collected from 22 adults of positive reactors. Stool examination was done for the intestinal parasites, and anal swabs with adhesive tape for the pinworm were carried out simultaneously. RESULTS: Overall positive rate in skin test was 9.5% in inhabitants, and higher rate was observed in older age groups. On microscopic examination of sputa for pargonimiasis, 2 egg-positive cases from 55-year old man and woman were detected. Prevalence rate of intestinal parasites was 7.8% (26/332) and the most predominant infected parasite was Trichuris trichiura (4.2%). The pinworm infection rate was 29.7% (nursery school children 30.0%, primary school children 29.6%, respectively). CONCLUSION: Through this study human paragonimiasis was confirmed and the life cycle of Paragonimus westermani was known to be maintained continuously in this region. Meanwhile control of intestinal parasites should be performed, especially in school children.
