1.The Difference of Interpretations of Cardiopulmonary Exercise Testing According to Interpretative Algorithms and Exercise Methods.

BACKGROUND: Recently, cardiopulmonary exercise testing (CPX) has become a popular diagnostic method for differentiating the main cause of exertional dyspnea or exercise limitation. We evaluated the difference in the CPX results according to interpretative algorithms and the methods of exercise in Korea. METHOD: Sixty-six patients with chronic lung disease and 48 adults with dyspneic symptoms, but with no abnormalities in a spirometry performed symptom limited CPX, were included in this study. The results were interpreted using both Wasserman's(WA) and Eschenbacher's algorithm (EA), and a comparison between both algorithms was made. Thirty-three healthy medical students performed the CPX with a cycle ergometer and treadmill. The results were interpreted with EA and the concurrenec in interpretations was evaluated according to the methods of exercise. RESULTS: 1. In patients with chronic lung disease, the overall concordance rate between the two algorithms was 63.6%. The concordance rates were 69.8% in patients with obstructive, 25.0% in those with restrictive, and 66.7% in those with mixed pulmonary insufficiency. In patients with dyspneic symptoms but normal findings in resting spirometry, the concordance rate was 60.4%. 2. In healthy medical students, in results interpreted with EA, the concordance rate between the cycle ergometer and treadmill exercise was 25.0%. CONCLUSION: Both interpretative algorithms and methods of exercise may affect the CPX results. In using CPX as a diagnostic test for the causes of dyspnea in the Korean population, the interpretative algorithms and method of exercise need to be standardized, and a predictive VO2max equation needs to be established.

2.The Actions of Sodium Valproate in Headache model Evoked by Substance-P in rats..

BACKGROUND AND PURPOSE: Valproic acid (2-propylpentanoic acid) which enhances GABA synthesis and blocks it's degradation has been useful treatment of migraine and may activate GABA receptors to modulate trigeminal nociceptive neurons innervating the meninges. But the mechanism and action of sodium valproate in headache is not clear. To investigate the mechanism of valproic acid action in headache model, we compared the change of dural plasma protein extravasation in both substance-P neurogenic inflammation rats with valproic acid pretreatment and without valproic acid pretreatment. METHOD: Sprague-Dawely rats were pretreated with valproate 30 minutes prior to substance-P administration in order to test the effects of sodium valproate on dural plasma protein extravasation by detecting the amount of extravasated Evans blue in the dura matter. To examine the abilities of either bicuculine (GABAA antagonist) and phaclofen (GABAB antagonist) to reverse the effect of valproate, they were administered 5 min before valproate administration. After then we also test the effect of muscimol (GABAA agonist) and bicuculine (GABAA antagonist) in substance-P induced neurogenic inflammation rats. RESULTS: Intraperitoneal injection of sodium valproate and muscimol reduced dural plasma protein extravasation after intravenous substance-P administration. The GABAA antagonist bicuculine completely reversed the effect of valproate and muscimol on plasma extravasation following substance-P administration, whereas the GABAB receptor antagonist, phaclofen, did not. CONCLUSION: We concluded that the attenuation of dural plasma protein extravasation by valproate and muscimol is mediated by via GABAA receptors within the meninges. Agonists and modulators at the GABAA receptor may become useful for the development of selective therapeutic agents for migraine headache.

4.Detection of the Proliferation of Muscle Fibers During Limb Lengthening by Monoclonal Antibody to Bromodeoxyuridine

Soft tissue related complications are quite frequent in limb lengthening. Muscle fibers may proliferate or regenerate after stretching injury over 10-20% of their original length. However, the cells which are engaged in this phenomenon are not confirmed yet. We chased the S-phase cells (phase for DNA replication) in the posterior leg muscle during limb lengthening by immunohistochemical technique. We lengthened the tibiae of fifteen New Zealand white rabbits. We divided them into three groups and each group is consisted of five rabbits. In group 1, we lengthened the left tibiae by 10% of their original length, in Group 2, 20%, and in group, 3, 25%, respectively, At the end of lengthening posterior muscles of lengthened left side and of controlled right side were fixed and processed for Immunohistochemical staining which could detect the incorporation of bromodeoxyuridine(BDU). Labelling index(LI:% of positively stained S-phase nuclei) of group 1 was zero. LI of groups for more than 20% lengthening (sum of group 2 and 3) was statistically significant. In conclusion, nuclei around or within the muscle tissue are in S-phase during limb lengthening which means proliferation of the muscle fivers or of the certain cells that abut muscle fibers.

8.Plasma Levels of Soluble Adhesion Molecules in Patients with Acute Cerebral Ischemic Stroke.

BACKGROUNDS: The inflammatory reaction after cerebral ischemia involving adhesion molecules aggravates neurologic deficit. This study aimed to study the change of plasma level of the adhesion molecules after acute cerebral ischemia. METHODS: Nineteen patients with acute cerebral infarction and ten control subjects without a history of cerebrovascular disease were included in this study. The patient groups were subgrouped into large artery atherosclerosis and small artery occlusion groups according to TOAST classification. Plasma levels of sP-selectin, sE-selectin, sICAM-1 and sVCAM-1 were measured within 24 hours and in 6 to 8 days after acute ischemic infarction. RESULTS: The plasma level of sP-selectin was elevated in acute stroke patients within 24 hours and in 6 to 8 days after stroke onset compared with control group(p<0.05). But plasma levels of sE-selectin, sICAM-1 and sVCAM-1 were not different from those of control group. The plasma level of sP-selectin was significantly elevated in large artery artherosclerosis group compared with control group. CONCLUSION: This study suggest that P-selectin actively involves in inflammatory process after acute ischemic stroke, especially associated with atherosclerosis.

9.The gastrofiberscopic findings in pediatric patients.

We observed 240 cases ranging in age from 3 months to 15 years seen by gastrofiberoscopy in the department of pediatrics of Seoul Red Cross Hospital from March 1990 to October 1992 and following results were obtained. 1) The male to female ratios in total cases and in 89 patients complaining of abdominal pain for more than one month were 0.94:1 and 1.02:1 respectively. The most prevalent age group was 10~12 years in both groups. 2) The indication in the order of frequency were chronic abdominal pain (37.1%), acute abdominal pain (32.5%), vomiting (7.9%), hematemesis and melena (5.4%), follow-up study (6.3%), further evaluation (3.8%), chest discomfort (2.5%), foreign body removal (2.1%), nausea (1.2%), dysphagia (0.8%) and abdominal mass (0.4%). 3) The common endoscopic findings in the 225 cases excluding follow-up ones were gastritis (34.7%), duodenitis (14.2%), ulcer (11.6%), esophagitis (7.6%) and foreign body (2.2%). 4) The common endoscopic findings in cases of chronic abdominal pain were gastritis (42.7%), duodenitis (16.9%) and ulcer (7.9%). 5) There was no complication except mild sore throat.

10.A Case of Inflammatory Pseudotumor of the Urinary Biadder.

Inflammatory pseudotumor of the urinary bladder is a proliferative spindle cell lesion that microscopically may suggest a sarcoma but that are benign without a recent history of an operation. The first such case was reported by Roth, in 1980, and thereafter about seven more cases were reported in medical literatures. We reported a case of inflammatory pseudotumor of the urinary bladder mimicking leiomyosarcoma. Patient was a 36-year-old woman with complaint of painless total and gross hematuria for 3 weeks. Partial cystectomy specimen showed a well-demarcated nodular mass of yellow white color, involving the submucosal and muscular layers. Microscopic examination revealed proliferating bundles of spindle cells interspersed with infiltration of many inflammatory cells including eosinophils. Spindle cells were positive for vimentin on immunohistochemistry and corresponding to myofibroblasts on the electron microscopic examination.
