1.Vitamin K-Deficient Hemorrhagic Disease in Infants with Acute Subdural Hematoma.

We had experienced 2 cases of acute subdural hematoma due to vitaimin K(vit. K) deficiency. They were 44-and 42-day-old. After the adminstration of vit.K1, prolonged prothrombin time(PT) and activated partial thromboplastin time(APTT) were corrected, so the surgical treatment ws performed. We supposed the predisposing factors were breast feeding and unknown hepatic pathology. No complication remained to one infant but the other multiple cerebral infarctions. The etiology, pathogenesis, diagnosis, prevention are discussed. We insist on neonatal administration of vit. K1 for the prevention of bleeding tendency. If intracranial hemorrhage develops, we must keep in mind that the early neurosurgical intervention may be needed.

3.Reactivity of the 19S ( IgM ) - FTA Test Among the Sera from VDRL Reactive Patients without History of Syphilitic Symptoms or Treatment.

On a total 233 sera from the VDRL reactive patients without a history of syphilitic symptoms or treatment for syphilis, TPHA and 19S(IgM)-FTA tests were performed, Also the reactivities of the 19 S (IgM)-FTA test in relation to the VDRL and the TPHA titers were observed. The results are summarized as follows: 1. Among 233 sera, 103 sera(44%) showed negative results in the 19S(IgM)- FTA test. 2. Reactivities of the 19 S(IgM)-FTA test were significantly increased in relation to the VDRL, titers arnong 233 sera reactive in the VDRL test: 47% in the VDRL titer 1: 1, 61% in 1: 2, 71% in 1: 4, 62% in 1: 8, 96% 1: 16 or more. 3. There was no relationship statistically (IgM)-FTA test and the TPHA titers. It can be derived from the above results that when a person with no history of treatment for syphilis and clear of any signs of syphilis shows a positive reaction in the VDRL test, he is less likey to be a syphilis patient when the VDRL titer is low.

4.Serum Levels of ICAM-1(In tercelluar Adhesion Molecule-1) in Invasive Cervical Cancer.

ICAM-l(Intercellular adhesion molecule-1) is an important early marker of immune activation and response. ICAM-1 is expressed on varous cell types and observed in a variety of diseases, including patients with asthma, melanoma, prostatic cancer, ovarian and colon cancer. Some authors demonstrated the expression of ICAM-1 protein in high-grade intraepithelial squamous neoplasia of cervix by immunohistochemistry and suggested that the expression was related to human papillomavirus infection. The aim of this study was to determine the serum levels of soluble intercellular adhesion molecule-1(sICAM-1) in patients with squamous cell carcinoma of the cervix, Serum levels of sICAM-1 were measured by enzyme-linked immunosorbent assay(ELISA), We evaluate invasive squamous cell carcinoma of the cervix (40), carcinoma in situ (16) and control (15) patients. Serum levels of sICAM-1 in healthy volunteers, in parients with carcinoma in situ and invasive cervical cancer were 150.1+/-41.3, 182.7+/-105.9, 189.8+/-60.0 ng/ml, respectively. Although the serum levels of sICAM-1 in patients with carcinoma in situ did not increase, serum levels of ICAM-1 in patients with invasive cervical cancer were significantly increased (control vs invasive cervical cancer, p < 0,05). From the above results, sICAM-1 is shed from the cancerous tissue in patients with squamous cell carcinoma of the cervix.

An association study with Korean alcoholic patients(n=50) and normal controls(n=53) was performed to find the relationship between catechol-O-methyltransferase(COMT) gene polymorphism and alcoholism using polymerase chain reaction-restriction fragment length polymorphism. When we compared the allele and genotype frequencies of Nla III COMT gene polymorphism in alcoholism and normal controls, there was no significant difference between two groups. Our results do not support an association between the Nla III polymorphism of COMT gene and alcoholism.

8.A female adnexal tumor of probable Wolffian origin showing positive O-6-methylguanine-DNA methyltransferase methylation.

Female adnexal tumor of probable Wolffian origin (FATWO) is a rare disease entity that arises from the mesonephric duct system. FATWO is different than other gynecological cancers in terms of embryology. Here, we describe the case of a 52-year-old woman with malignant FATWO. The patient underwent explorative laparotomy and surgical staging after a frozen section revealed malignancy. Detailed examination of the pathologic findings were consistent with FATWO. Counseling and further testing were provided to the patient to assess the risk of germline mutation and epigenetic change. An O-6-methylguanine-DNA methyltransferase gene methylation test was positive, and all other tests were normal. This is the first study to report a case of O-6-methylguanine-DNA methyltransferase methylation with FATWO in Korea.

10.A Case of Tsutsugamushi Disease Associated With Intermediate Uveitis and Retinal Detachment.

PURPOSE: The authors report a Tsutsugamushi patient who has intermediate uveitis and rhegmatogenous retinal detachment without a typical skin eschar, which may be rare. CASE SUMMARY: The authors present a case of a 63-year-old man who complained of fever, chills, and headache, with lymphadenopathy, hepatomegaly, reticulonodular pneumonia, and blurred vision in the right eye after descending a mountain in autumn. Serological findings showed elevated titers for strains of Rickettsia tsutsugamushi, even though no eschar could be found on his body. In addition, the ophthalmologic examination disclosed vitreous opacity as uveitis in his right eye; thus, the patient was treated for Tsutsugamushi disease. However, retinal detachment was found in his right eye even after treatment. Thus, a vitrectomy and intravitreal silicone oil injection was performed, and the patient's best corrected visual acuity improved from hand motion to 0.04 on the 7th day after the operation.
