2.A Case of Metastatic Basal Cell Carcinoma.

Basal cell carcinoma is the most common skin cancer, but it rarely metastasizes. Metastatic basal cell carcinoma (MBCC) is rare in terms of incidence as well as absolute numbers. We present a case of 67-year-old woman who had a dirty oozing, ulcerated, tumor mass on the vertex and suffered from dyspnea and mild lumbago. The diagnosis of MBCC was based on clinical, radiological, and histopathological findings. To our knowledge, the occurrence of MBCC has not been reported in the Korean literatures. So we reported herein this case of MBCC of the lung and the bone.

3.Morphologic Study on the Mesothelial Hyperplasia in the Rat.

Reactive mesothelial cells are frequently found in the smear, of body fludid dffusion when associated with liver cirrhosis, pulmonary tuberculosis, congestive heart failure or pulmonary infarction. But it is not easy to differentiate these reactive mesothelial cells from macrophages or various types of malignant cells. In order to investigate the morphology of hyperplastic mesothelial cells, in comparison with the various types of malignant cells in the effusion, this experiment was designed. The tissue reaction was evoked by intraperitoneal injection of 2.5 ml of 2% Talcum suspension. The results obtained were as follows: 1) On light microscopic observation of the tissure, until 5 days, cytoplasm and nucleus of mesothelial cells became larger and nucleoi became prominent. After then, cytoplasm and nucleus became smaller and at 28 days after injection of Talcum suspension, cellular features became similar to those of normal ones. 2) On cytologic observation of touch print specimens, hyperplastic mesothelial cells became round to oval, and their cytoplasm became conspicuous. Nuclei were vesicular, and nucleoi were prominent. N/C ratio was decreased than normal. 3) On electron microscopic observation, hyperplastic mesothelial cells did not have microvilli, but had a few cytoplasmic globular projections, loosely approximated cellular junctions, larger and more abundant intracytoplaomic vesicles when compared with normal one. Chromatin clumping, irregular thickening of nuclear membrane and prominent nucleoi were also noted.
4.Morphological Classification of Left Superior Vena Cava and Analysis of Associated Cardiac Anomalies.

The persistent left superior vena cava (LSVC) is not rare cardiovascular developmental anomaly occurring both in association with congenital heart disease and as an isolated anomaly of no hemodynamic importance. We have studied 73 cases of the LSCV out of 1,060 cases of congenital heart disease catheterized at Yonsei Cardiovascular Center. We conducted the study with a view point of position of the heart and abdominal organs and segmental analysis of the underlying congenital heart disease. We also analysed the associated extracardiac vascular anomalies. The following results were obtained: 1) The incidence of this anomaly among congenital heart disease was 6.9% and 41 cases(56.2%) had cyanosis. 2) We observed 20 cases(27.3%) with the malposition of the heart and 17 cases(23.3%) with malposition of the abdominal organs. The ventricular loops revealed D-loop in 60 cases, L-loop in 7 cases and in the remaining 6 cases, it was uncertain. 3) With a view point of type of LSVC by Lucas & Krabill, type A was in 50 cases(68.5%), type D in 14 cases(19.2%), type B in 5 cases(6.8%) and type C in 4 cases(5.5%). 4) Associated cardiovascular anomalies were as follows: ventricular septal defect; 42 cases(57.5%), atrial septal defect; 33 cases(45.2%), patent ductus arteriosus; 27 cases(36.9%), and tetralogy of Fallot; 18 cases(24.7%). In conclusion, LSVC usually has no hemodynamic importance, but this cardiac anomaly is frequently combined with complex intracardiac anomalies. Therefore, it is important to making accurate diagnosis and successful management for preventing the risk of it.

6.Prognostic Value of Somatosensory Evoked Potentials in Comatose Patients after Cardiopulmonary Resuscitation.

BACKGROUND: The improved technique for cardiopulmonary resuscitation(CPR) has resulted in the survival of many patient who experienced cardiac arrest. However, mortality in resuscitated patients is high, and the survival rate without brain damage is very low. Various neurological examination models, neuro-imaging techniques, electrophysiological procedures, and biochemical tests have been studied with respect to the detection of cerebral damage and outcome, but an early, reliable prediction of individual outcomes is still uncertain. METHODS: We studied twenty patient who had been in a coma for more than 24 hours after CPR, Somatosensory evoked potentials(SEP) were measured within the first three days after CPR. RESULTS: Of the twenty patients, seven patients(35%) had a good outcome, and thirteen patients(65%) had a bad outcome. Of the eleven patients with loss of the cortical evoked potential's N20 peak, all had a bad outcome. CONCLUSION: SEPs are of great benefit in prognostic evaluation after CPR.

9.The impact of beam angle configuration of intensity-modulated radiotherapy in the hepatocellular carcinoma.

PURPOSE: This treatment planning study was undertaken to evaluate the impact of beam angle configuration of intensity-modulated radiotherapy (IMRT) on the dose of the normal liver in hepatocellular carcinoma (HCC). MATERIALS AND METHODS: The computed tomography datasets of 25 patients treated with IMRT for HCC were selected. Two IMRT plans using five beams were made in each patient; beams with equidistance of 72degrees (Plan I), and beams with a 30degrees angle of separation entering the body near the tumor (Plan II). Both plans were generated using the same constraints in each patient. Conformity index (CI), homogeneity index (HI), gamma index, mean dose of the normal liver (Dmean_NL), Dmean_NL difference between the two plans, and percentage normal liver volumes receiving at least 10, 20, and 30 Gy (V10, V20, and V30) were evaluated and compared. RESULTS: Dmean_NL, V10, and V20 were significantly better for Plan II. The Dmean_NL was significantly lower for peripheral (p = 0.001) and central tumors (p = 0.034). Dmean_NL differences between the two plans increased in proportion to gross tumor volume to normal liver volume ratios (p = 0.002). CI, HI, and gamma indices were not significantly different for the two plans. CONCLUSION: The IMRT plan based on beams with narrow separations reduced the irradiated dose of the normal liver, which would allow radiation dose escalation for HCC.

10.Effect of Eicosapentaenioc acid and Butyrated hydroxyanisole on Hypercholestrolemic Diet Induced Atherogenesis in Rabbit.

It has been suggested that the fish oil can reduce atherogenesis in humans and animals, and that peroxidation of lipoproteins may be a major factor causing atherosclerotic lesions. We tested these posibilities in rabbits fed an atherogenic diet by comparing the effect of a eicosapentaenoic acid(EPA: a major component of fish oil)supplement and a butyrated hydroxyanisole(BHA: antioxidant)diet supplement. Tweenty-eight young male New Zealand White rabbits were used in this study. The animals were divided by control, cholesterol fed only, cholesterol + EPA, and cholesterol + BHA groups. The experimental course lasted 12 weeks and animals were sacrificed periodically(2, 5, 8, 12weeks)for quantitative studies of aortic atherosclerosis using light and electron microscopy. Plasma cholesterol levels were determined and lipopreteins were separated periodically. The cholesterol fed only group showed an increased serum cholseterol level and atherosclerotic lesions from 5 weeks of experiments. The EPA supplement resulted in similiar serum cholesterol levels with cholesterol fed only group, but greater lesion than cholesterol fed only group. The BHA supplement resulted in higher serum cholesterol levels except VLDL-cholesterol than EPA supplement group. However, the atherosclerotic lesion was not increased. Our studies support the theory that oxidative modification of lipoproteins is important for the atherogenesis and antioxidant may have a protective effect. However, it failed to show antiatherogenesis effect of fish oil.
