A clinical and histologic survey was done on 71 cases of basal cel carcinoma(BCC) filed at Department of Clinical Fathology, Kang Nam, Han Kang, and Kang Dong Sacred Heart Hospital during past 13 year s from 1979 to 1991. The results were summerized as follows: 1. Of all malignant skin tumors BCC accounted for 31.6%. The frequency of BCC in the total number of outpatients visiting the Department of Dermatology was 0.06%. 2. The ratio of men to women with HCC was 1:1.03. 83.8% of the pat.ients with BCC were older than the age of l0 years with the mean age of 56.9 years. 3. 83.8% of the BCC appared in the face, especially on the nose(25.4%), eyelid(20.3%). 4. On the basis of classification of Lever et al, the solid type(66.7% ) was the most com mon histologic pattern, followed by the adenoid(8.8%), pigmented(8.8%), and etc. By the classification of Farmer et al, the nodulocystic type(35.1% ) was the most common histologic pattern followed by infiltrative(31.6%), adenoid(8.8%), pigmented(8.8%), and etc. By the classification on Sexton et al, in the order of decreasing frequency, liistologic subtypes were the nodular(43.9%), mixed (22.8%), infiltrative(19.3%), supeficial(3.5%), morpheic(3.5%), and micronodular(3.5%).

We report two cases of multiple neurilemmomas occured in a 17-year-old female and in a 76-year-old male. Both cases were histopathologically confirmed as neurilemmoma and immunohistochemical study showed S-100 protein in the tumor cells of case 1.

Fifteen cases of genital warts were examined for the presence of human papillomavirus (HPV) using polymerise chain reaction (PCR). HPV6/11 DNA were found in all cases of genital warts. The PCR based methods described here provide a sensitive, accurate means of detecting genital HPVs.

The insulinoma is a rare tumor, usually small, solitary and benign with no prediction for any part of the pancreas. It is amendable to surgical cure but approximately 10percent are malignant and the peak incidence is encountered between ages 40~60. Surgery has been established as the treatment of choice. The reason for electing to operate rather than treat symptomatically is prevention of complications, such as increasing obesity, or prolonged and irreversible episodes of hypoglycemic coma with resultant permanent central nervous damage. During the anesthesia for a patient with insulinoam, the important problems are to recognize and treat hypoglycemia. This is a case report of a patient with insulinoma who underwent surgical treatment. Two years age, under the diagnosis of insulinoma, she was underwent distal pancreatectomy with splenectomy. But 3 months ago, hypoglycemic attacks recurred during fasting periods surgical intervention was performed under the diagnosis of recurrent insulinoma. She was anesthetized with thalamonal-nitrous oxide-oxygen, pancuronium, and 10 percent dextrose solution was administered throughout the operation. We monitered the level of blood sugar intermittently by using a rapie sugar analyser(Glucometer, Ames, Japan). No hypoglycemic episode during anesthesia was observed. We report the case of our anesthetic experience withan insulinoma and review the anesthetic choice and the management of the patient.

Although intramedullary nailing is mostly used in the management of fractures in the long bone shaft, plate fixation is still alternative option in periprosthetic or metaphyseal fractures. For the rigid fixation during plating, sometimes we need wiring. Many surgeons experience slippage of wire resulting in loss of rigid fixation because plate and wire is so slippery and the diameter of bone is changing by level especially in the metaphyseal area. Wiring plate(CHO plate) was designed to prevent slippage. This plate has transverse holes for wires between screw holes on traditionally used dynamic compression plate. Sixteen fractures of long bone shaft were fixed with wiring plate and 11 cases which were followed up for more than one year were evaluated. There were periprosthetic fractures(4 cases), spiral or butterfly fractures(4 cases) and fractures of poor bone quality(two cases of malignancy and one case of osteoporosis). Nine cases were successfully reduced and the healed without loss of fixation. In two cases, delayed union and nonunion were observed due to wire breakage with plate loosening. Preliminarily, wiring plate fixation can be used for the periprosthetic fracture and one of alternative option in butterfly or spiral fracture and fracture in osteoporotic bone or pathologic fracture.

To determine size and shape discrepancy between the bone of Korean women and the prostheses in total knee arthroplasty, the height(antero-posterior length) and width(medio-lateral length) of the distal femur and the proximal tibia were measured at the bone resection level for 104 knees of 62 female patients who underwent primary total knee arthroplasty and compared with the same parameters of the total knee prostheses designed by five different companies. On the condition that the height of the prostheses matched with each cases of the bone were optimal, the difference in the width was evaluated, and the criteria of the discrepancy were overhang if the width of the prosthesis was larger than bone, optimal if width of the prosthesis was same or smaller and difference was within 5 mm, and down-size if smaller and difference was over Smm. In femoral prostheses, 34.6% of all were optimal, 13.1%, overhang and 52.3%, down-size. In tibial prosthesis, 54.1% of all were optimal, 13.9%, overhang and 32%, down-size. In conclusion, although further investigation for clinical application is needed, there were discrepancies in size and shape between the femur and tibia of Korean women and total knee prostheses and improvement in design should be considered.

OBJECTIVES: The objective of this study was to assess the characteristics of smartphone use in elementary school children and the emotional and behavioral problems associated with smartphone overuse and addiction. METHODS: Children referred to a community based pediatric mental health center were screened for smartphone addiction and related problematic behavior using the Smartphone Addiction Proneness Scale and the Korean version of the Child Behavior Checklist (CBCL). RESULTS: 1) Statistically significant differences in gender, usage time, ownership, and internalization symptoms (somatization, anxiety/depression, withdrawal) were observed between the smartphone excessive user group and the control group. 2) Variants showing the most significant correlation with smartphone addiction were usage time, internalization problems somatization, anxiety/depression, withdrawal, aggressive behaviors, and total behavior problems. CONCLUSION: The results of this study indicated an association of addiction tendency in elementary school children with internalization symptoms. Close observation in the familial and academic setting may be important for children at high risk for smartphone addiction.
