1.Clinical Features of Cataract Extraction with Negative-Power Intraocular Lens Implantation in High Myopia.

PURPOSE: To evaluate the clinical features of high myopic eyes with cataracts implanted with negative power intraocular lenses (IOLs) at the time of cataract surgery. METHODS: A retrospective chart review was conducted of 18 eyes of 14 patients who underwent cataract surgery with negative power IOLs and 10 eyes in 9 patients with low power IOLs. We investigated axial length, IOL power, preoperative and postoperative best-corrected visual acuity (BCVA) and preoperative and postoperative spherical equivalent (SE) refractive errors. RESULTS: Mean BCVA showed significant improvement in both groups. We measured postoperative SE refraction and the difference between the mean intended and the mean achieved SE refractive errors in the negative power group (17 eyes) was +1.59 ± 1.34 D and +0.31 ± 0.50 D in the low power group. CONCLUSIONS: BCVA was significantly improved in the majority of eyes, although they had myopic macular degeneration or posterior staphyloma. However, the mean achieved postoperative SE refraction was more hyperopic than the predicted postoperative SE error. Additionally, hyperopic refractive error was greater in the negative power group than the low power group. Therefore, we recommend that postoperative hyperopic refractive error should be considered when performing cataract surgery in high myopic patients.

The duration of the effect of muscle relaxants is extended in patients with acquired immunodeficiency syndrome (AIDS); however, a specific mechanism for this effect has not yet been discovered. In addition, streptomycin is known to prolong the action of muscle relaxants by blocking activity at neuromuscular junctions. Here, we report the case of patient with AIDS taking streptomycin for pulmonary tuberculosis for which sugammadex was effective in reversing prolonged paralysis induced by rocuronium and vecuronium for video-assisted thoracoscopic surgery (VATS) lung wedge resection.

6.A Study of Morphological Characteristics of Cerebral Aneurysms.

The pathogenesis of cerebral aneurysm until now is a subject of continuing debate. Some authors stress the important role of hemodynamics in the pathogenesis of cerebral aneurysm by confirming it with experimental work, pathological study, autopsy study and review of angiograms. With the findings of cerebral angiograms and operations, the authors had studied the difference in size and shape between ruptured and unruptured aneurysms, the relationship between afferent arteries from afferent arteries. 103 patients were admitted to the Keimyung University Dong San Hospital during the period from September 1982 to August 1985. According to studies on individual sizes of aneurysms, 52% of unruptured aneurysms were smaller than 5mm, but 68% of ruptured aneurysms were over 5mm but below 10mm. Moreover, the mean size of unruptured aneurysms was 4.87mm and ruptured aneurysms was 9.38mm. 53% of unruptured aneurysms were spherical in shape and combined daughter aneurysms in unruptured were 13%. However, in case of ruptured aneurysms, 34% were spherical and combined daughter aneurysms were 35%. The origin of daughter aneurysms in the dome was 59%, waist 29% and neck 12% in order. The direction of aneurysms was variable, but most of them pointed in the direction of blood flow in afferent arteries. We infer with this study that the hemodynamics of blood flow plays an important role in the pathogenesis of cerebral aneurysms.

7.Hand-Assisted Laparoscopic Nephrectomy and Auto-Transplantation for a Hilar Renal Artery Aneurysm: A Case Report.

A 52-year-old man was admitted with an incidentally detected right renal artery aneurysm (RAA). Computed tomographic angiography with three-dimensional reconstruction revealed that the aneurysm was 2.2 cm in diameter and located at the renal hilum. We performed hand-assisted laparoscopic nephrectomy with ex vivo repair of the RAA and auto-transplantation with minimal elongation of Gibson incision. The operation and postoperative course were uneventful. At last follow-up, the patient was alive with a well-functioning auto-transplant. Hand-assisted laparoscopic nephrectomy and auto-transplantation is a useful treatment option for hilar RAA.

8.Congenital Partial Left Pericardial Defect.

Congenital pericardial defect is relatively rare and two different types, partial and complete, of different clinical significance have been recognized. Most reported defects are complete type and left-sided lesion. Most patients are asymptomatic or complain of vague chest pain. Partial pericardial defect can be potentially fatal due to cardiac herniation and strangulation or coronary insufficiency. Plain chest reontgenography shows abnormal prominence along the cardiac border. 2-D echocardiography demonstrates a drop-off of pericardial echo and protruding cardiac chamber through the defect. Because of the potential fatality, surgical repair is recommended for the partial pericardial defect. We report a case of congenital partial left pericardial defect, which was diagnosed by plain chest reontgenography and 2-D echocardiography, with related literatures.

9.The physical properties of tin ion-plated Co-Cr (elgiloy) orthodontic wires.

To estimate the possibility of clinical application of TiN ion-plated Elgiloy(Co-Cr wire), measurements of tensile strength and hardness were made on the four tempers on each of the manufactured Elgiloy, the (heat-treated) Elgiloy for 30 minutes at 250degrees C and the TiN ion-plated Elgiloy. For comparison, the tensile strength and hardness of Stainless Steel wires were also measured. The following are the results of the study: · In the 4 tempers, tensile strength was the greatest in the TiN ion-plated group, followed by the heat-treated Elgiloy group and the manufactured Elgiloy group, but no statistical difference was noticed between heat-treated and manufactured Elgiloy groups(P<0.05). · In each temper, tensile strength- of ion-plated Elgiloy increased- about l0kgf/mm` in comparison with the values of the manufactured Elgiloy. · In yellow, green and red tempers except the blue, hardness was the greatest in ion-plated group. In the blue temper, there was no statistical difference between heat: treated and manufactured Elgiloy groups(P<0.05). · In each temper, hardness of ion-plated Elgiloy increased about 50-90VHN in comparison with the values of the manufactured Elgiloy. · The tensile strength of Stainless Steel wire was similar to that of the red temper of manufactured Elgiloy and the green temper of ion-plated Elgiloy.

10.The Clinical Significance of Lund-Mackay CT Staging System in Assessing the Severity of Chronic Rhinosinusitis.

Backgrounds and Objectives : Although the sinus CT (computed tomography) scan has emerged as the preferred method for assessing the extent of chronic rhinosinusitis, the relationship between sinus CT findings and symptom severity in chronic rhinosinusitis has not been established yet. The Lund-Mackay CT staging systems were known as one of the most prevalent methods in assessing the severity of chronic rhinosinusitis. We aimed to determine the relationship between the severity of the Lund- Mackay CT scores and symptom severity, and to determine the relationship between the laterality and symptom severity in chronic rhinosinusitis. MATERIALS AND METHODS: 108 patients who fit the eligiblity and exclusion criteria were evaluated. We scored each CT scan on the Lund-Mackay system and let the patients complete the questionnaires for the rhinosinusitis symptoms. The correlation between the CT scores and the severity of patients' symptoms was assessed. Then, we divided the patients into 4 groups according to the grades of CT scores. And in each group, we subgrouped them according to the CT score differences between the right and left sides. Statistical analyses of symptom scores between subgroups in each group were performed. RESULTS: The CT scores graded by the Lund-Mackay system and symptom scores were correlated significantly (rho=0.365, p< or = 0.0001). According to the laterality of chronic rhinosinusitis, there were no statistically significant differences between CT scores and symptom scores in the subgroups of each group. CONCLUSION: The CT score by the Lund-Mackay system was correlated with the patients-based reports of symptom severity regardless of the laterality of chronic rhinosinusitis. We therefore recommend that the Lund-Mackay system to be adopted as the standard method of measuring the extent of chronic rhinosinusitis.
