2.Clinical Observation of Sulfobenzylpenicillin(Sulbencillin) in Urinary Tract Infection.

The effectiveness of Sulfobenzylpenicillin in the treatment of urinary tract infection was evaluated in the Department of Urology, Seoul National University Hospital during the period from May to November, 1973. Our clinical study was performed on the 28 patients who visited the outpatients clinic and admitted to the ward. These patients were studied by bacterioscopy with Gram's method of staining. and culture in blood agar plate and chocolate agar plate In this studies. the causative organisms were divided into 8 groups; E. coli (5), coagulase positive staphylococcus (5), pseudomonas (2), Corynebacterium spp. (1), a-hemolytic etreptococcus (2), N. Gonorrheae (10), Proteus (2) and KIebsiella (1). The dosage schedule was 1 gm. of Sulfobenzylpenicillin at intervals of 12 hours for consecutive 5 days in gonococcal urethritis and nongonococcal urethritis. But in upper urinary tract infections and post-operative infections, it lasted for 7 to 10 days or more. And following results were obtained. Effectiveness on Urological diseases 1) Gonococcal urethritis; Satisfactory clinical results were obtained with Sulfobenzylpenicillin for five days. Results were excellent in 5 cases, good in 4 cases and negative effect in 1 case. 2) Non-gonococcal urethritis; In this group, the therapeutic results were favorable in 90% of all cases. (excellent-4, good-5, fail-l) 3) Urethral stricture and fistula (2 cases); AII cases were responded to the Sulbenicillin. 4) Cystitis including Prostatism; Two of three cases were caused by E. coli and they had a good response to sulbenicillin. 5) Two cases of pyelonephritis and two cases of postoperative infections were studied with sulbenicillin and more than 60% of cases were effective with sulbenicillin. Side effects were not remarkable but in a few cases who received intramuscular injection, pain on the injection site was noticed.In summary, this new semi-synthetic penicillin is considered to be a useful chemotherapeutic weapon against a variety of infection in the urological field including those due to Gram negative bacilli such as pseudomonas and proteus sp.

3.Pseudoatrophoderma Colli.

Pseudoatrophoderma colli is a symptomless, persistent, persistent, pigmentary disorder involving the neck and upper thorax first described by Becker and Muir in. 1934 The lesion is usually localized to the neck and upper part of the thorax. It consists of macules or papules which show pigmentation or papules which show pigmentation or depigmentation and look atrophic. Histological examination shows hyperkeratosis, hypogranulosis, focal acanthosis and perivascular lymphocytic infiltration. We present herein a case of pseudoatrophoderma colli which showed symptomless, mottled, pigmentated macules involving the neck clinically and compatible findings histopathologically.

4.Multifocal Reticulum Cell Sarcoma of Bone: A Case Report

A Case of multifocal reticulum cell sarcoma in a 61 year old male is reported. The patient had multiple foci of sarcomatous lesion in the olecranon of the right ulna, medial candyle of the left tibia and in both ankle joints. His initial complaints on admission were pain and swelling in his both ankles and the right elbow of three months duration. Biopsy was carried out on his right ankle joint and diagnosis of reticulum cell sarcoma was made. Surgical excision of the right olecranon and below Knee amputation of the right lower extremity were performed. Following the surgical intervention, Cobalt 60 irradiation therapy was given for 20 days with total amount of 4,000 rads. The follow-up study three months after surgery revealed much improvement and no evidence of recurrence.

5.Noduloulcerative syphilis.

A 26-year-old female presented herself to our clinic due to noduloulcerative lesions of one month duration on her face and trunk with accompaning headache and mild fever. In family history, her husband was reported to have a history of chaner. On physical examinat.ion, rice sized eythematous papules and bean sized nodules with central ulceration were seen on the face and trunk. VDRL was 1:31 positive. A skin biopsy iif a noduloulcerative lesion showed ulceration, vascular dilatation with fibrinoid changes and perivascular inflarnmatory cellu infilt.rations, especially plasma cells. Immunoperoxidase stain showed T. pallidum at the epidermis and papillary dermis. The diagnsis was made asecondary syphilis with unusual clinical presentation as noduloucerative skin lesions. The skin lesions completely disappeared one month after treatment with injection of benzathine pelnicillin G 2.4 million unit intramuscularly weekly for three times.

7.A Case of Resection of Pulmonary Metastatic Choriocarcinoma in Drug-Resistant Patient.

Pulmonary metastasis occurs frequently in patients with gestational choriocarcinoma and most of these patients achieve remission with chemotherapy alone. Thus, the indications for surgical intervention are limited, but in appropriately selected patients, resection of a chemotherapy-resistant lung lesion can be curative. We have experienced that pulmonary metastatic choriocarcinoma in a drug-resistant patient was cured by pulmonary resection. So we report this case with a brief review of literatures.

8.CORRECTION OF ORIENTAL EPIBLEPHARON BY HALF A-PLASTY AND HOTZ PROCEDURE.

Epiblepharon is frequently encountered n oriental eyelid, especially in children. It is characterized by a horizontal fold of redundant skin and abundant orbicularis muscle near lid margin, which the lashes vertically, or posteriorly towards the eye. Surgical therapy of epiblepharon in correcting the trichiasis includes lid bracing sutures, burried sutures, skin resection, a Hotz procedure or its modification, and a Jones procedure. In our opinion, these procedures are not sufficient, because these procedures will not correct the epicanthal folds. Epicanthal folds have been a one of the characteristics of Asian over 50 percents of population in Japan and South Korea, and it either reduce the aesthetic result of the double-eyelid procedure or make the correction of the entropion and epiblepharon difficult. Although many surgical procedures are available to eliminate epicanthal folds, scarring on the medial canthus is in still and obstacle for surgeons to overcome. From January of 1998 to October of 1998, we used half z-plasty procedures in 5 cases to correct epicanthal folds in epiblepharon. There were few complications in our series. We believe that half z-plasty can get a good result in the correction of epibolepharon and oriental epicanthal folds.

9.Protein Antigens of Treponema pallidum Reacting with Serum IgM Antibodies of Syphilis Patients.

This study was undertaken to identify the protein antigens reacting with IgM antibodies in the sera of various stages of syphilis before and after treatment, employing sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE), immunoblotting and immunoperoxidase stain. The results were as follows : 1. Before treatment, the most strongly reacting antigens of T. pallidam precipitated by IgM antibodies in the sera of patients were polypeptides of molecular weights 47,000, 34,000 and 29,500. So it was observed that those were the major antigens of T. pallidum reacting with IgM antibodies. 2. After observing protein antigens of T. pallidum reacting with IgM antibodies in the sera of patients with syphilis before and after treatment, it was seen that in primary, secondary and early latent syphilis there was a loss of several antigens and s decrease in reactivity, but no changes occurred in late latent and reinfected syphilis. 3. From the observation of the reaction between serum antibodies of patients with trested syphilis and major antigens of T. pallidum, an evident decrease in reactivity was observed only with protein antigen of molecular weight 47,000 which reacts with IgM antibody. From the above results, it could be concluded that of molecular weight 47,000 could contribute to the assessment of the diagnosis of syphilis and the efficacy of treatment.

10.Protein Antigens of Trepanema pallidum Reacting with Serum IgG Antibodies of syphilis Patients.

This study was conducted to identify the protein antigens reacting with IgG antibodies in the sera of variaus stages of syphilis before treatment, the antigens common to both Treponema pallidum and Treponema phagedenis, and the antigens specific only to T. pzllidum, employing sodium dodecylsulfate polyacrylamide gel electrophoresis(SDS PAGE) and immunoblotting. Before treatment, the most strongly reacting antigens of T. pallidum precipitated by IgG antibodies in the sera of patients were polypeptides of molecular weights 47, 000, 36,500, 15,500 and 14,000. Eleven antigens common to both T. palbdum and T. phagedenis were observed, and antigens of molecular weights 86,500, 68,500,15,500 and 14,000 showed to be specific only to T. pallidum. From the results obtained, it could be concluded that of the major antigens of T. pallidum, the antigens of molecular weights 15,500 and 14,000 could serve to develop newer serologic tests for syphilis.
