5.A Clinical Study of Colorectal Cancer.

This is a clinical study of 129 cases of colorectal cancer who were treated in the department of surgery, Inchon Christian Hospital from January 1989 to December 1996. The results were as follows; 1) The peak age incidence was 7th decade and the ratio between male and female was 1.87 : 1 2) The most frequent site of the tumor was the rectum(59.7%). 3) In the right and left colon cancer the most frequent symptom was abdominal pain and in the rectal cancer the most frequent symptom was bloody stool. 4) The duration of symptoms prior to admission was within 3months(58%). 5) The diagnostic methods were digital examination, sigmoidoscopy colonofiberscopy, barium enema and abdominal CT scanning. 6) The most frequently performed operation was Abdominoperineal resection(27%). 7) The staging of tumors was performed according to modified Dukes' classification and Dukes C was most frequent(55.1%). 8) The most common histopathologic type was adenocarcinoma(98%). 9) The most common macroscopic finding was ulcerative pattern. 10) The most common distant metastatic site was pelvic organ(12%). 11) The most common post-operative complication was wound infection(10.1%). 12) The 5year survival rate was analyzed by Kaplan-Meier's method : the overall 5year survival rate was 60.7%.

7.The Unstable Intertrochanteric Fractures of Femur Treated with Sliding Compression Hip Screw: The Comparison between Anatomical Reduction and Non-Anatomical Reduction Groups

Due to the instability of intertrochanteric fractures in elderly patients, various methods of reduction and fixation were introduced to obtain stable reduction and solid union, But there have been many controversies regarding advantages between anatomical reduction group and non-anatomical reduction group. The authors analyzed the 68 unstable intertrochanteric fractures out of total 94 cases of intertrochanteric fractures treated from Jan. 1988 to Jan. 1994 with mean follow-up 17 months and evaluated the radiologic and clinical results between those two groups. The results were obtained as follows: 1. The most common cause was low energy injury and according to the Tronzo classification, the type III was most common in 54 cases (79.4%). 2. The average union time was 14.0wks (84%), the union rate was low in the medialized reduction group(A-P) and anterior displaced reduction group(A-P), anterior displaced and anatomical reduction groups(Lateral). 3. The compression length more than 10mm of lag screw was deep in medialized reduction group(A-P), another displaced and anatomical reduction groups(Lateral). 4. The average length of lag screw sliding was 9.2mm and depth sliding of 6.8mm (74%) was occurred within immediate postoperative 1 month.

8.Capsular Contracture Rates in Augmentation Mammaplasty: Comparison of Round vs Anatomical Breast Implants

Purpose:
Capsular contracture is a major cause of poor postoperative satisfaction in augmentation mammaplasty. It is unclear whether round or anatomical textured breast implants lead to differences in the rates of capsular contracture. Accordingly, the objective of this study was to compare capsular contracture rates between round and anatomical textured breast implants.
Methods:
A total of 1,282 cases (2,564 breasts) of augmentation mammaplasty using textured type implants, performed at MD clinic between January 2012 and December 2015, were retrospectively reviewed. A total of 703 cases used round textured implants while 579 used anatomical textured implants. The median follow-up period for the round textured implant group was 9 months, and 7 months for the anatomical textured group.
Results
The overall capsular contracture rates in the first year was 2.0% in the round textured group and 1.5% in the anatomical textured group (p=0.609). Capsular contracture rates in the first year in those who underwent primary surgery were 1.6% in the round textured group and 1.3% in the anatomical textured group (p=0.187). The rates in those who underwent revision surgery were 5.2% in the round textured group and 2.6% in the anatomical textured group (p=0.178). Conclusion: Although the risk for capsular contracture in the anatomical textured groups appeared to be slightly lower than in the round textured groups, the difference was not significant. Further studies with longer follow-up periods are needed to assess potential differences.
9.A Case of Vesicourachal Diverticulum.

The urachus is a vestigial structure extending from the apex of the bladder to the umbilicus. Embryololigically, the urachus becomes obliterated during the third month of fetal life, but incomplete regression of the urachus results in anomalies. Urachal anomalies are very rare and present with varied appearances, one of which is vesicourachal diverticulum. Recently, we experienced a case of vesicourachal diverticulum complained of recurrent urinary tract infection in 13 month old male. Embryology, classification, clinical presentation, diagnostic modalities and treatment of urachal anomalies are reviewed.

10.A Case of Multiple Sclerosis Involving Hypothalamus.

Lesions of multiple sclerosis (MS) are rarely distributed in deep gray matter. We report a case of MS with hypothala-mic lesions. A 26-year-old woman with MS developed hyperphagia, hypersomnolence, and amenorrhea. Brain MRI revealed bilateral lesions in both hypothalamus and thalamus. Serum prolactin concentration was increased and thyroid stimulating hormone (TSH) level was decreased. Three months after steroid pulse therapy, her symptoms were com-pletely resolved and prolactin concentration became normal Follow-up brain MRI demonstrated complete disappear-ance of previous lesions.
