1.Prevalence of Capillaria hepatica among house rat in Seoul.

During May-July, 1978 a survey on the prevalence of C. hepatica among house rat(Rattus norvegicus Erxl.) and wild mice (Mus musailus molossinus and Microtus mandarinus) in two districts in Seoul was carried out. Approximately 38 percent of 1,000 house rats examined were infected. Rat population as an established group in Ahyundong, neighbouring the city, revealed 39.2 percent of infection among 660 house rats, and that as newly formed group in Shinrimdong, a developing residential quarter on the southern outskirts of Seoul showed 35.9 percent among 340 rats. Infection rate of females and males was 38.9 percent and 37.2 percent respectively. Differences of infection rates by locality and sex were not statistically significant. It was very interesting that none of 64 wild mice were infected with this parasite in both districts. Some probable factors which are related on C. hepatica infection among rats were discussed briefly.

2.Studies on the incidence of the intestinal protopozoa in the mountainous area of Chollapuk-do.

This study was undertaken in order to elucidate the prevalences of the intestinal protozoan infection according to localities and the mode of the infestation in the mountainous area of Chollapuk-do, Korea during the year 1970-1971. 8,508 of fecal samples were collected among the inhabitants of the mountainous area, which covered 18 Myens(=Township) from 6 Guns(=County) in the eastern part of Chollapuk-do. Most of the inhabitants of this area were engaged in farming and living under lower economic and poor sanitary conditions. The samples were examined twice repeatedly by iodine stained direct fecal smear method. For the study on the mode of infestation, with the exception of drinking water, the zinc sulfate floatation technic was applied for the examinations of hog-feces, manure heap, garden soil, sewage, dishtowel, dust in room and floor, house fly and house rat. The results were summarized as follows: Among total 8,508 fecal specimens examined, 3,152(37.6 percent) were positive for the intestinal protozoa: E. histolytica 9.8 percent, E. coli 25.6 percent, E. nana 7.2 percent, G lamblia 6.8 percent, I. butschlii 0.6 percent, and C. mensnili 0.7 percent. A single infection of the intestinal protozoa was the highest(68.3 percent), and double infection(22.3 percent) triple, quadruple and quintuple infections were in decreasing order. The highest distribution (39.8 percent) was observed in the group aging 41-50, and the lowest (27.3 percent) in the group above 61. The groups under 10 years showed a relatively high positive rate among all age groups. Sexual distribution of protozoan infection showed slightly higher rate (38.6 percent) in male than that (35.4 percent) of female. Jangsoo-gun showed the highest positive rate (42.9 percent) and Namwon-gun was the lowest (32.5 percent). The prevalence of E. histolytica showed the highest (11.6 percent) in Imsil-gun and the lowest (6.9 percent) in Jangsoo-gun. E. histolytica cysts were found 9.1 percent among total 99 samples of hog feces, 4.8 percent among 78 samples of manure heap, 0.6 percent among 164 samples of fingernail dirt, 5.3 percent among 57 samples of drinking water, 15.8 percent among 19 samples of sewage, 7.1 percent among 42 samples of dishtowel, 0.1 percent among 780 samples of house fly and 5.0 percent among 40 samples of house rat.

3.A Case of Cutaneous Leishmaniasis Treated with Cryosurgery.

Cutaneous leishmaniasis (oriental sore) is usually a self-limited infection of the skin caused by the protozoan Leishmania tropica. The disease is endemic to the Mediterranean, Asia, Africa, and the Middle East. It has been seen in this country among many Korean technical experts and labourers working in the endemic areas of the disease. Our patient had acquired cutaneous leishmaniasis in Saudi Arabia and it had remained active for six months. He had been treated with antimony and metronidazole but failed because of severe side effects. And then we treated the patient witb cryosurgery and the skin lesions were followed by resolution with cosmetically acceptable scar in 4 months. The brief review of literature on the treatment of cutaneous leishmaniasis was undertaken.

4.A Study for Assessing the Irritancy of Toilet Soaps and Soap Ingredients.

In the present study soap chamber test was carried out for assessing irritancy of toilet soaps and soap ingredients. The results obtained in the present study are summarized as follows; This chamber test can discriminate the differences of irritancies between various soaps and soap ingredients. 2. Soap irritancy is not directly related with its alkalinity. 3, Erythema score and scaling score tend to parallel each other with weak correlation(r=p 46), but erythema score and fissuring score show strong correlation (r=p77) Scaling score and fissuring score show moderate correlation(r=p.64), 4. In the eighteen kinds of toilet soaps, two soaps are most irritating, harsh soap and the total irritancy scores of all soaps are within 1. 9 5. 1. Baby soaps are not milder than the common toilet soaps. 5. Generally, total irritancy score of soap ingredients is concentration dependent. 5.Two Cases of Aplasia Cutis Congenita.

Aplasia cutis congenita is an anomaly characterized by absence of localized areas of the integument. The most common type of aplasia cutis congenita is a circular or oval, sharply outlined ulcer 1 to 3cm in diameter on the vertex of the scalp at the level of the posterior fontanel. Two cases of aplasia cutis congenita are presented. The first case was in a male newborn infant with a localized defect on his scalp. The second case was also in a male newborn infant who had extensive symmetrical involvement of his heels. No similar conditions and associating other congenital anomalies were found in the family members of reported cases.

7.A Case of Multiple Endocrine Neoplasia Type I Presented with Secondary Amenorrhea and Osteoporosis.

MEN type 1 is characterized primarily by the presence of functioning and nonfunctioning tumors or hyperplasia of the pituitary gland, parathyroid glands, and pancreatic islet cells. Pancreatic islet tumors in MEN type 1 produce different kinds of hormone which were pancreatic polypeptide, gastrin, glucagon, insulin and so on. To date, ten cases had been reported in Korea. We report another case with MEN type 1 having prolatin-secreating pituitary adenoma, parathyroid hyperplasia and insulinoma. A 36-year-old woman was admitted because of long-standing amenorrhea and recently diagnosed osteoporosis. Otherwise, she had been in good health except experiencing one episode of loss of consciousness after skipped meal. The blood chemistries were normal except hypercalcemia and hypophosphatemia. Hormonal studies revealed elevated levels of intact PTH and prolactin and decreased value of estradiol with low LH and FSH. The neck CT revealed 1 cm-sized nodule at posterior portion of right thyroid gland and 99mTc-sestamibi sintigraphy showed a increased uptake in left lower and right lower parathyroid glands. The sella MRI showed 0.7 cm-sized enhanced lesion in the left pituitary gland. The ratio of immunoreactive insulin to glucose was elevated and 3-4 pancreatic masses of variable size were identified by endoscopic ultrasonography and angiography. Subtotal parathyroidectomy and pyrolus-preserving pancreaticoduodenectomy with spleen-preserving distal pancreatectomy was done. Postoperative she had been doing well with normocalcemia and normoglycemia. Transsphenoidal adenonectomy was done 5 months later. Histologic examination of removed tissues revealed a single insulinoma, prathyroid hyperplasia and prolactin-secreating pituitary adenoma.
