The widespread use of diagnostic radiography, especially using magnetic resonance imaging, has helped to increase the diagnosis of paralabral cysts in patients with chronic shoulder pain. These paralabral cysts are frequent in the anterior, the superior, and the posterior compartment of the shoulder joint but are rare in the inferior compartment. Paralabral cysts in the shoulder appear particularly in men in their third and fourth decades but rarely in elderly patients. We report a case of an inferior paralabral cyst in an elderly patient whom we treated through arthroscopic decompression.

4.Development of a Demand Model for Physician Workforce Projection on Regional Inequity Problem in Korea Using System Dynamics

Background:
Appropriate physician workforce projection through reasonable discussions and decisions with a broad view on supply and demand of the workforce, thus, is very important for high-quality healthcare services. The study expects to provide preliminary research data on the workforce diagnosis standard model for Korean physician workforce policy decision through more flexible and objective physician workforce projection in reflection of diverse changes in healthcare policy and sociodemographic environments.
Methods:
A low flow rate through the causal map was developed, and an objective workforce demand projection from 2019 to 2040 was conducted. In addition, projections by scenarios under various situations were conducted with the low flow rate developed in the study. Lastly, the demand projection of the physician workforce by region of 17 cities and provinces was conducted.
Results:
First, demand of physicians in 2019 was 110,665, 113,450 in 2020, 129,496 in 2025, 146,837 in 2030, 163,719 in 2035, and 179,288 in 2040. Second, the scenario for the retirement of baby boomers led to a decrease in the growth rate due to time delay. Third, Seoul and Gyeonggi-do account for a high percentage of demand, a very high upward trend was identified in Gyeonggi-do, and as a result, the projection showed that the demand of the physician workforce in Gyeonggi-do would worsen over time.
Conclusion
This study is meaningful in that rational and collective physician workforce supply and demand and its imbalance in workforce distribution were verified through various projections by scenarios and regions of Korea with System Dynamics.
5.A Study on Somatosensory Evoked Potential in Patients with Cervical Spondylosis.

In order to study the relationship between clinical group of cervical spondylosis and electrical activity, we traced somatosensory evoked potential(SSEP) by stimulating the median nerve(MN) from the upper extremities, and the posterior tibial nerve(PTN) from the lower extremities simultaneously. The purpose of this study was to test the clinical diagnostic utility of the method, whether any consistent relationship exists between the severity and type of neurological symptoms and the results of the SSEP responses, and between neurological improved patients after operation and improved SSEP results. In this study patients were clinically divided into three groups : combined myelopathy and radiculopathy, myelopathy alone, radiculopathy alone. Over a 3-year period from 1987 to 1989, SSEP were performed on 112 patients of Korean adults and summary of the results obtained are as follows : 1) Among patients with cervical spondylosis, those with cervical myelopathy showed more abnormal SSEP than those cervical radiculopathy. 2) In the group with myelopathy, MN-cervical latensy, Ep-C2 interpeak latency. PTN-P1 latency, and N1 latency were prolonged more than MN-C2 latency, Ep-C2 interpeak latency, PTN-P1 latency, and N1 latency of the group with radiculopathy. 3) The longer the duration of the symptoms, the more abnormal were the SSEP in patients with cervical spondylosis. 4) In patient with cervical myelopathy, the latency of SSEP was prolonged not only in those with sensory deficit, but also with motor deficit. 5) Half of clinically improved patients after operation showed improved SSEP in follow-up examinations. 6) Improvement in SSEP after operation was much affected by posterior approach.

6.Dioscorea Quinqueloba-Induced Tubulointerstitial Nephritis.

Drug-induced tubulointerstitial nephritis is one cause of acute kidney injury. Although traditional remedies have been widely used in South Korea, Dioscorea quinqueloba-induced tubulointerstitial nephritis has not been reported in the general population. A 72-year-old male patient was transferred to our hospital with pulmonary edema, oliguria, decreased mentality, severe generalized edema after taking D. quinqueloba 25 days ago. His initial lab findings showed a blood urea nitrogen level of 43.4 mg/dL, a creatinine level of 5.3 mg/dL. Urinalysis revealed SG (1.015), blood (many), protein (++) and WBC (0-3/HPF). Kidney biopsy demonstrated severe mononuclear cell infiltration into the renal interstitium with mild tubular atrophy. Aggressive renal replacement therapy and supportive care resulted in gradual restoration of his renal function. This case implies that D. quinqueloba may be one cause that induces tubulointerstitial nephritis in some patients.

Urinalysis revealed SG (1.025), blood (many), protein (+), and WBC (0-3/HPF). Her 24-hour total protein level was 1,120 mg/day. No specific abnormalities were found on serum and urine protein electrophoresis. Kidney biopsy demonstrated severe mononuclear cell infiltration into the renal interstitium with mild tubular atrophy and pericapsular fibrosis. Immediate withdrawal of Corni fructus, appropriate education, and conservative management resulted in gradual restoration of her renal function. This case implies that Corni fructus may be a causative allergen that induces acute interstitial nephritis in some patients.

9.A Rare Cause of Automated Peritoneal Dialysis-Related Peritonitis: Aeromonas caviae.

Peritonitis is a major cause of morbidity in peritoneal dialysis patients; however, Aeromonas caviae rarely causes peritonitis in these patients. We report peritonitis due to A. caviae in a 72-year-old male with end-stage renal failure who had been undergoing automated peritoneal dialysis for 1 year. The white blood cell (WBC) count in the peritoneal fluid was 2,722/mm3 with 85% neutrophils. Gram staining of the peritoneal fluid yielded Gram-negative rods. Empirical antibiotic therapy with ceftriaxone was initiated intraperitoneally. However, drug sensitivity testing revealed that the organism was resistant. On the third hospital day, A. caviae was cultured from the peritoneal effluent, and the antibiotic regimen was switched to ciprofloxacin. The patient recovered rapidly and the WBC count of the peritoneal effluent decreased. He was discharged and the intraperitoneal ciprofloxacin therapy was continued for 14 days. The peritoneal catheter was not removed.

Knee joint gouty arthritis is commonly reported, but gouty tophi in bilateral popliteal cysts, such as this case, are rare. We report on a case treated by cystectomy of gouty tophi in bilateral popliteal cysts and review.
