1.Three Cases Of Symptomatic Hyponatremia After Mild Head Trauma.

Many patients whose chief complaint is headache caused by accident such as traffic accident, falling, or assaults visit to emergency center. Majority of these patients has mild or moderate symptoms, and there is no need to treat surgically. However, the fact that head injury can cause SIADH(syndrome of inappropriate secretion of antidiuretic hormone) is likely to be overseen. Acute hyponatremia might have been associated with death or permanent brain damage. Hyponatremia is best handled by early recognition and correction of a downward trend in the serum sodium. Unfortunately, this is often difficult in the head-injured patient, where hyponatremia may occur fairly rapidly and is confused by symptoms of head injury. when moderate to severe hyponatremia occurs, it is important to determine its etiology and expeditiously initiate corrective action. There has not been any reported case of the occurrence of symptomatic SIADH after mild head injury in Korea yet. The authors report the experience of three cases of symptomatic SIADH after mild head injury with review of the literatures.

2.A Histopathological Study on the Estrogen-induced Breast Lesion in Rats.

Forty eight female Sprague-Dawley rats received a subcutaneous implant containing 12.5 mg estradiol ant the age of 3 weeks. Three rats were killed in 1, 2, 3, 4, 6 weeks and in every month during 2~12 months after implantation, and the breasts were examined by light microscope. In all rats, enlargement of terminal end buds was obseved in 1~2 weeks, maximum development of hyperplastic alveolar nodules in 3 weeks, and marked dilatation and secretion of alveoli or ducts in 1~12 months after implantation. Ductal epithelial hyperplasia was observed in 27 rats and carcinomas developed in 23 rats in 2~12 months after implantation. It was thought that the changes induced by estradiol are more similar to the human breast lesions, compared with changes induced by chemical carcinogens such as dimethylbenzanthracene(DMBA), because breast carcinomas developed in close relationship with ductal epithelial hyperplasia in both estradiol-treated rats and humans, but not in DMBA-treated rats.

4.Cauda Equina Syndrome in the Lumbar Disc Herniation.

STUDY DESIGN: A retrospective analysis of the patients who had cauda equina syndrome caused by a herniated lumbar disc. OBJECTIVES: To assess the clinical debates concerning the diagnosis, treatment, and results of treatment. SUMMARY OF LITERATURE REVIEW: This syndrome has been considered as an absolute indication of surgical treatment in the herniated lumbar disc and poor prognosis after surgery were reported. Although there are debates on the timing of surgery, early recognition early surgical treatment are recommended for a better results of treatment. MATERIALS AND METHODS: 23 consecutive patients having a cauda equina syndrome caused by herniated lumbar disc were reviewed retrospectively. They were 14 males and 9 females having ages of 41.6 in average. The incidence of this syndrome was 3.8% of the patients who had surgical treatment of herniated lumbar disc. RESULTS: All patients had bladder dysfunction in addition to the low back pain and radicular pain in the leg, but five patient did not recognize their urinary retention which were shaded by severe leg pain. A surgical treatment were performed for all patients at 6.8 days in average after onset of the bladder dysfunction. At follow-up of 3 years and 3 months in average after surgery, 20 patients(86.9%) had satisfactory resolution of the low back pain and leg pain, and all patients had complete recovery of motor weakness except one patients who had residual foot drop. Bladder function recovered satisfactorily in 18 patients (78.3%), but 4 patients had some difficulty of urination and one patient needed a sphinterotomy for urination. CONCLUSIONS: The cauda equine syndrome in herniated lumbar disc was often not recognized early and a poor result of treatment was not rare, particularly in the patients who had acute onset and severe bladder dysfunction. So, early diagnosis and active surgical treatment are recommended for a better results of treatment.

5.Favorable effect of chemotherapy with BAM-M in mycosis fungoides.

Mycosis fungoides is an unusual malignancy of helper T-lymphocyte origin which presents initiaily with skin disease and progresses to the lymph nodes, peripheral blood. and viseral organs, and frequently results in death. A 51-year-old woman was admitted to the hospital in October, 1930 for the evaluation of a painful reddish brown tumor with ulceration of 6 months duration on the abdomen. Lymphadenopathy was not present, and routine laboratory examinations revealed negative or, within normal limits. She was diagnosed as mycosis fungoides with stage IIb according to the commirtee on staging and classification of cutaneous T-cell lymphoma, and treated with chemotherapy;regimen, BAM-M consisted of Bleomycin, Adriamycin, Methotrexate and topical nitrogen mustard. she achieved complete remission of the tumor with five cycles of BAM-M chemotherapy. We report favorable response after BAM-M chemotherapy in a pacent with advanced stage of mycosis fungoides(TNM IIb).

6.Sutureless Strabismus Surgery with Tissue Adhesive in Rabbit Models.

Inadvertent perforation of the globe, granuloma and hypersensitivity reaction were recognized complications of the strabismus surgery. We performed sutureless surgery with Beriplast in rabbit model to determine whether this technique can replace the classic suture technique and decrease it's complications. The authors measured maximum adhesion power and observed histopathologic findings at 1st, 2nd, 4th and 8th week following recession of the rectus muscle with Beriplast. Maximum adhesoin power were increased with time such as 210, 300, 480 and 650gm at each week. Pathologically inflammatory and foreign body reactions were decreased with time, and collagen fibers were proliferated at muscle scleral contact area.

8.A Clinical Observation of the Intravesical Thiotepa in Bladder Cancer.

Primary transitional cell carcinoma of the bladder was treated with prophylactic and therapeutic Intravesical thiotepa in 7 cases. In 5 cases of stage A tumor, postoperative follow up was averaged 10.8 months and 4 cases were tumor free (80%). In 2 cases of infiltrating tumor, the instillation was not effective. Thiotepa can be given safely in the 7 days post-operative period and no leukopenia and bladder irritation symptoms were occurred.

9.A Case of The Modified Jensen Procedure Saving Anterior Ciliary Arteries.

For the 6th nerve paralysis, we use the procedure described by Jensen combined with a 6-7mm recession of the madial rectus muscle. But, anterior segment ischemia(ASI) is an occasional complication of this surgery in which over three rectus muscles are partially or completely detached, ligated. The authors performed this modified Jensen procedure to the patient of 6th nerve paralysis, in which anterior ciliary arteries of vertical rectus muscles were separated from the muscles under the operating microscope and then ligated muscles only. So, this modified procedure preserve the integrity of the anterior ciliary arteries in those portions of the vertical muscles not involved in the muscle union to avoid the danger of anterior segment ischemia.

10.A Study on the Precancerous Lesion of Breast Carcinoma in 9, 10-Dimethyl-1, 2-Benzanthracene-treated Rats.

Following results were obtained from the light microscopic and stereomicroscopic observations of the breasts of rats treated with 9, 10-Dimethyl-1,2-Benzanthracene(DMBA). 1) Adenocarcinomas developed in 17 rats (24%) among 70 DMBA-treated rats. 2) Terminal and buds (TEB) were observed longer in DMBA-treated rats than in control group, but they finally disppeared 4 monthes after treatment. 3) Many hyperplastic alveolar nodules (HAN) developed in DMBA-treated rats. 4) There were no transitional lesions between TEB and adenocarcinoma or HAN and adenocarcinoma. 5) The number of lobules was decreased in DMBA-treated rats. On the other hand, terminal ducts were increased in number. These findings suggest that DMBA stimulate the regression of lobules and induce to form terminal ducts from which adenocarcinomas and HAN develop independently.
