1.Analysis of manifestation of fundus fluorescein angiography in multiple sclerosis

Objective To observe the features of the manifestations of fundus fluorescein angiography (FFA) in multiple sclerosis (MS) and their value in clinical diagnosis. Methods The clinical data of 42 patients (84 eyes) with MS diagnosed by magnetic resonance imaging (MRI) and examination of cerebrospinal fluid (CSF) were retrospectively analyzed. The clinical data included visual acuity, ocular fundus examined by direct ophthalmoscope after mydriasis, FFA, visual field, CSF,visual evoked potential (VEP) and MRI examination. Results In 42 patients (84 eyes),the positive detectable rate of examination of direct ophthalmoscope, CSF, visual field, VEP, and MRI was 36.9%, 21.4%, (71.4%, and) 83.3% respectively. Abnormal results of FFA were found in 44 eyes (52.38%), including papillitis in 4 eyes (4.76%)at the early stage with extended physiological scotoma and central scotoma; neuroretinitis in 7 eyes (8.33%)at the medium stage with central or para- central scotoma; optic atrophy in 33 eyes(39.29%) at the late stage with centripetal constriction and even tubular visual field. Conclusion The main angiographic features of MS are papillitis, neuroretinitis and optic atrophy. The manifestations of FFA combined with the results of examination of CSF,visual field, VEP and MRI is helpful for comprehensive and exact diagnosis of MS.
2.Relationship between Psychological Factors and Mode or Duration of Labor

Objective To observe the relationship between psychological factor(anxiety and depression) and postpartum hemorrhage?mode and duration of labor. Methods Ninety two healthy nulliparas who had no gist of cesarean section were involved in our study. Their psychosis were evaluated with Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS),according to which they were divided into two groups:high-score group and low-score group.All women were not interfered during the courses of delivery. Mode, duration of labor and postpartum hemorrhage were observed. Results Among the 92 nulliparas observed, the spontaneous delivery rate was 86.96% (80 cases) and the postpartum hemorrhage rate was 10% (8 cases). The scores of SAS and SDS of the low educational level group, the spontaneous group or the non-hemorrhage group were significantly lower than those of the high educational level group, the dystocia group or the hemorrhage group respectively. 5.Application of preoperative eye position training in FL-LASIK

Objective To investigate the clinical effects of preoperative eye position training on femtosecond laser-assisted laser in situ keratomileusis(FL-LASIK)? Methods One hundred and sixty-eight myopia patients(328 eyes)scheduled for selective FL-LASIK were randomly divided into the experiment group of 85 cases(166 eyes)and the control group of 83 cases(162 eyes)?The patients in the control group were given the routine preoperative education and the patients in the experiment group received the preoperative intervention of eye position besides routine preoperative education? The two groups were compared in terms of intraoperative changes of eye position,corneal topography and the postoperative visual acuity? Results The frequency of eye position adjustment and machine stopping in the experiment group was significantly smaller than that of the control group(P < 0?001)? But no significant differences were found between the two groups in corneal topography and the postoperative visual acuity(P > 0?05)? Conclusion Preoperative training of eye position may maintain ideal eye position and reduce the frequency of downtime due to eye position adjustment during FL-LASIK,which may ensure the successful completion of FL-LASIK?
6.MRI in diagnosis of Herlyn-Werner-Wunderlich syndrome

Objective To assess the value of MRI in diagnosis of Herlyn-Werner-Wunderlich syndrome (HWWS).Methods MR images of 10 patients with HWWS confirmed surgically and pathologically were analyzed retrospectively.The classification of HWWS on MRI was discussed.Results The 10 patients were included 4 cases of type Ⅰ,3 cases of type Ⅱ and 3 cases of type Ⅲ.All of them were complicated with ipsilateral renal agenesis.Eight cases were complicated with didelphic uterus and double cervix,2 were complicated with complete septate uterus.Type Ⅰ showed the complete oblique vaginal septum,hematocele in the oblique septum cavity and contralateral uterine cavity.Type Ⅱ showed the oblique septum cavity was connected with main vaginal through the hole of septum.Type Ⅲ showed the oblique septum cavity was connected with main vaginal through the cervical fistula.The prognosis were good in 8 patients.One patient occured obvious dysmenorrhea postoperation and MRI reminded right ovarian endometriosis cyst.One patient occured gusty hypogastralgia,and MRI reminded pelvic inflammation.Conclusion MRI can definitely diagnose and classify HWWS,and can provide useful information in the treatment of HWWS.
7.Chemical constituents of Hedyotis corymbosa

Objective To investigate the chemical constituents in the whole plant of Hedyotis corymbosa.Methods The compounds were isolated by column chromatography,pre-TLC,and recrystallization.Their structures were elucidated by spectroscopic methods.Results Thirteen compounds were isolated and identified as(+)-lyoniresinol-3?-O-?-D-glucopyranoside(Ⅰ),quercetin(Ⅱ),esculetin(Ⅲ),scopoletin(Ⅳ),hedyotiscone A(Ⅴ),p-hydroxybenzoic acid(Ⅵ),protocatechuic acid(Ⅶ),vanillic acid(Ⅷ),syringic acid(Ⅸ),(+)-vomifoliol(Ⅹ),(-)-dihydrovomifoliol(Ⅺ),S-(+)-dehydrovomifoliol(ⅩⅡ),and alizarin 1-methyl ether(ⅩⅢ),respectively.Conclusion Compounds Ⅰ—ⅩⅢ are isolated from this plant for the first time.
8.Therapeutic Effects of Bailing Capsules Combined with Dermatophagoides Farinae Drop in the Treatment of Allergic Rhinitis

Objective:To investigate the clinical effects of dermatophagoides farinae drop combined with Bailing capsules in the treatment of allergic rhinitis. Methods:Totally 96 patients with allergic rhinitis in our hospital were selected and randomly divided into two groups with 48 cases in each by a randomization table. The patients in the treatment group received dermatophagoides farinae drop and Bailing capsules,and those in the control group only received dermatophagoides farinae drop. The scores of symptoms and physical signs and adverse reactions were compared between the two groups. Results:After the treatment,the scores of symptoms and physical signs in the two groups were significantly decreased(P < 0. 05),and the decrease in the treatment group was more notable(P < 0. 05). The effective rate of the treatment group(93. 75% )was higher than that of the control group(62. 50% ,P <0. 05),while the incidence of adverse reactions of the two groups was without significant difference(P > 0. 05). Conclusion:Bailing capsules combined with dermatophagoides farinae drop can significantly relieve the clinical symptoms and signs,and improve the quality of life of the patients with allergic rhinitis,which is worthy of promoted application in clinics.
9.Bacteria distribution and drug sensitivity analysis of nosocomial infection cases in children′s hospital

Objective To understand and analysis of the pathogens distribution and drug resistance of nosocomial infection in children′s hospital,so as to provide reliable scientific basis for the prevention and control of hospital infection.Methods 396 cases of upper respiratory tract specimens were collected from pediatric patients with nosocomial infection.These specimens were detected by sputum specimens conventional methods of microorganism cultivation,and K-B method was used to determine the bacteria sensi-tivities to clinical common drug.Results There were 225 cases of specimens were pathogen positive among all the 396 specimens, and 234 strains of bacteria were isolated in all.The positive isolated rate was 56.8%(225/396).Among the 234 isolated strains, Gram negative bacteria accounted for 72.6%(170/234),and Klebsiella occupied the first place[49.4%(84/170)].Gram positive bacteria accounted for 23.5%(55/234),and Staphylococcus had the highest isolated rate in Gram positive bacteria[58.2% (32/55)].In all the 9 kinds of clinical common antimicrobial agents,imipenem had high drug sensitivity to the 234 isolated strains,and the aminoglycosides came next.Conclusion It is necessary for the pediatric patients with nosocomial infection to collect upper re-spiratory tract specimens for bacteriologic studies and drug sensitivity tests.
10.Factors and validity analysis of Mini-Mental State Examination in Chinese elderly people

Objective:To examine factors that may have impact on the Mini-Mental State Examination ( MMSE) screening validity, which could lead to further establishing the general model of the MMSE score in Chinese health elderly and to improve the screening accuracy of the existing MMSE reference. Methods:Based on the data of the Chinese Longitudinal Healthy Longevity Survey ( CLHLS ) , the MMSE scores of 19 117 normal elderly and 137 dementia patients who met the inclusion criteria were used for the analysis. The area under the curve ( AUC) and validity indexes were used to compare the screening accuracy of various criteria. Multiple linear regression was used to identify factors that had impact on the MMSE score for both the normal and dementia elderly. Descriptive analysis was performed for differences in the MMSE scores by age trends and gender between the normal and dementia elderly. Results:The AUC of MMSE was≥0 . 75 ( P<0 . 05 ) . The MMSE score of the normal elderly declined nonlinearly as the age grew older(male:R2 =0. 924, P<0. 05; female: R2 =0. 951, P<0. 05), and increased nonlinearly as the education level rose(male: R2 =0. 948, P <0. 05; female: R2 =0. 859, P<0. 05). The females had significantly lower MMSE scores than the males, with a faster decline trend with age than the males (95%CI of female partial regression coefficient was not overlapped with 95%CI of male partial regression coefficient) . The dementia elderly showed a much lower MMSE score ( male:difference of Z score:-1 . 573 , P<0 . 05;female:difference of Z score:-1 . 222 , P<0 . 05 ) and ten-ded to with a faster decline speed than that of the normal elderly (95%CI of dementia partial regression coefficient included 95%CI of normal partial regression coefficient) . Conclusion:The screening validity of MMSE in CLHLS is not affected by educational level. The analysis of factors that may impact on the MMSE screening validity are gender, age, vision and residence which with validity identification. These four fac-tors can be used as assist tool of MMSE in the screening of dementia to improve the screening accuracy.
