2.A Clinical Observation of Urinary Stone Culture.

The relationship between urea-splitting organism in the urine and urinary stone is well known. We have checked bacteriologic study of 28 urinary stones after surgical removal of stone and brushing, and following results were obtained. 1. Positive stone culture were 6 cases (21.4%). 2. Organisms were Pseudomonas, Klebsiella, Proteus, Serratia, Staphylococcus and Enterococcus. 3. Antibiotics sensitivity of 6 cases of infected stones were Klebsiella, proteus: 2) Amikacin, Pseudomonas: 1) Ampicilline, Serratia: 1) Cefamezine, Staphylococcus: 2) Kanamycin, Gentamycin, Enterococcus: 3) Penicilline. 1) Sensitive 2) Moderately sensitive 3) Relatively resistant. 4. The most prevalent age group of infected stone was between 40-49. 5. Ureter was the most favorable site of infected stone and multiple stone were 2 cases. 6. Positive urine culture were detected in all of 6 cases of infected stone. 7. In 4 cases of infected stone, organisms of stone culture and urine culture were identical. They were Pseudomonas, Enterococcus, Proteus, Staphylococcus. 8. In 2 cases of infected stone, organisms of stone culture and urine culture were not identical. They were Serratia, Klebsiella in stone culture and proteus, E. coli in urine culture respectively.

4.Inhibition Effect of Angiostatin and Endostatin on Human Angiogenesis.

PURPOSE: Angiogenesis is a critical determinant of tumor growth and the development of metastasis. Angiostatin and endostatin have been used in a variety of in vitro and in vivo animal models as effective inhibitors of angiogenesis. However, human angiostatin and endostatin have not been tested against an intact human tissue target in vitro to determine its ability to achieve an antiangiogenic response. We performed our study to determine if human angiostatin and endostatin would inhibit the development of an angiogenic response (initiation) and to determine the subsequent growth (angiogenic index) of human vessels in a dose-dependent manner with a human placental vein angiogenesis model (HPVAM). METHODS: We used full thickness human placental vein discs cultured in three-dimensional fibrin-thrombin clots with an overlay of liquid media. Human angiostatin and endostatin were evaluated in concentrations ranging from 10-9 M to 10-4 M. A positive control containing 20% fetal bovine serum and a negative control using heparin and hydrocortisone 21-phosphate were also tested. RESULTS: Human angiostatin did not inhibit the initiation of an angiogenic response and the subsequent development of the angiogenic response (angiogenic index) at any concentration. Human endostatin significantly inhibited the initiation rate of an angiogenic response at a concentration of 10-4 M (p<0.001) and the subsequent development of an angiogenic response (angiogenic index) from a concentrations of 10-5 M to 10-4 M (p<0.001, p<0.001, respectively). CONCLUSION: We conclude that a very high concentration of human endostatin can inhibit the angiogenic response in human vascular tissue and that human angiostatin will not inhibit angiogenesis of normal human blood vessels in vitroThese results suggest that human endostatin has a more powerful antiangiogenic effect than human angiostatin, but we need further investigations of human angiostatin against an intact human tissue target.

5.Unilateral Parieto-Occipital Lobe Infarction Presenting with Optic Ataxia and Saccadic Abnormalties: A Case Report .

Optic ataxia is characterized by an impaired visual control of the direction of arm reaching to a visual target, accompanied by defective hand orientation and grip formation. In humans, optic ataxia is associated with lesions of the superior parietal lobule, which also affect visually guided saccades and other forms of eye-hand coordination. A 67-year-old woman presented with sudden, unilateral, direct optic ataxia in the right homonymous half field and saccadic abnormalities which consisted of prolonged latency, undershoot dysmetria, and decreased velocity. Brain MRI showed a left parieto-occipital watershed infarction. It is suggested that lesions of the superior parietal lobule and the adjacent parietal eye field are responsible for optic ataxia and saccadic abnormalities, respectively.

7.Point Prevalence of Anxiety and Depression among Cancer Inpatients in a General Hospital: A Preliminary Study for the Development of Distress Management Strategies in Cancer Patients.

OBJECTIVES: This study aimed to examine the point prevalence of anxiety, depression and the associated risk factors in hospitalized patients with cancer. METHODS: A total of 114 patients with cancer who admitted to the Cancer Center in St. Mary's Hospital were enrolled for a day. The patients were asked to complete Hospital Anxiety and Depression Scale(HADS). Sociodemographic and cancer related clinical variables were also evaluated. RESULTS: The point prevalence of anxiety and depression was 10.5% and 34.2%, respectively, as assessed with HADS. Logistic regression models revealed that distant metastasis was significantly associated with anxiety in patients with cancer and previous chemotherapy and Low socioeconomic status were significantly associated with depression. CONCLUSIONS: Our results indicate that more than one third of patients with cancer suffer from either anxiety or depression. In addition, previous chemotherapy as well as low socioeconomic status and distant metastasis were associated risk factors of anxiety and depression, respectively. These findings suggest the need for screening and systematized psychosocial intervention for psychological distress in hospitalized patients with cancer.

8.Detection of Treponema pallidum in Tissues by the Avidin - Biotin - Peroxidase Complex Techniques.

Using the avidin-biotin-peroxidase complex(ABC) technique, 36 skin specimens from 30 patients with primary and secondary syphilis and a gastric mucosal specimen from a patient with secondary syphilis which were confirmed by clinical history, physical examination, the VDRL, FTA-ABS, TPHA, and 19s(IgM)-FTA test, were examined. The results were compared with that of the indirect mmunoperoxidase technique which was done by authors previously with same specimens. The following results were obtained. 1. Of the 37 specimens, 35(95%) were positive in ABC technique and 33 of the 37 specirnens(89%) were positive in the indirect immunoperoxidase technique. 2. The ratio of agreement of the results between the ABC and the indirect immunoperoxidase technique was 89%. 3. Most of the treponemes were located in the upper dermis, epidermis, and blood vessel walls in the arder named, and rarely in the lower dermis of the syphilitic skin lesions. There was no remarkable difference in histologic distribution of treponemes between the clinical stages and types of syphilitic skin lesions. :From the results, the ABC technique is considered to be an excellent method for detecting the treponemes in the suspected syphilitic lesions.

9.Detection of Treponema pallidum in Tissues by Indirect Immunoperoxidase Technique and FTA - ABS Complement Technique.

Using the indirect immunoperoxidase and FTA ABS complement techniques, 36 skin specimen: from 30 patients with primary and secondary syphilis and a gastric mucosal specimen from a patient with secondary syphilis which were confirmed by elinical history, physical examination, VDRL, FTA ABS, TPHA, and 19s(IgM)-FTA test, were examined. The follwing results were obtained. l. Of the 37 specimens, 33(89%) were positive in the indirect immunoperoxidase techique and 26 of the 37 specimens(70g) were positive in the FTA ABS complement technique. Of the 17 specimens, 12(71%) were positive in the darkfield examination. 2. The ratio of agreement of the results between the indirect immunoperoxidase and FTA ABS complement techniques was 81%, The ratio of agreement of the results between the indirect immunoperoxidase technique and darkfield examination was 82%. 3. Most, of the treponemes were located in the upper dermis, epidermis, and blood vessel walls in the order named, and rarely in the lower dermis of the syphilitic skin lesion. There was no remarkable difference in histologic clistribution of treponemes between the clinical stages and types of syphilitic skin lesions. From the results, the indirect immunoperoxidase technique is considered a superior method than the FTA ABS complement technique and darkfield examination for detecting the treponemes in the suspected syphilitic lesions.

10.Lipoma of the Heart: An Autopsy case report.

Lipomas of the heart are benign neoplasms and have rarely been described. Due to the fact that they normally cause no symptoms, diagnosis is often purely accidental. Because of the rarity of these tumors, it seems worthwhile to present an example studied at autopsy. It was associated with the sudden death of a 15-year-old boy. The tumor arose from the wall of the left ventricle and occupied the pericardial cavity, measuring 13x7x6 cm in size. The tumor was whitish-yellow, translucent, and soft. Microscopically, the tumor was composed of mature adipose tissue which extended between muscle fibers. This current case, the giant cardiac lipoma is believed to produce disturbances of the conduction system and distrubances of cardiac filling.
