1.Implementation of Pain Intervention among Clinical Nurses.

This study was carried out in clinical nurses to identify the factors related to implementation of pain intervention. Samples were selected from 215 nurses working at four different hospitals in Seoul, Taegue, Andong and Mungung between March 3 and April 4, 1997. The data were analyzed with descriptive statistics, t-test and chi-square using SAS statistical package. The results were as follows. 1. The answer of questions. Ninety six percent of the nurses agreed that pain perceived in each patients was influenced by several factors besides tissue demage. Fifty two percent of nurses demonstrated that they were more responsible for relieving patients's pain now than just after graduation. Ninety two percent of the nurses answered that they were not as sympathetic as for patients's pain now than just after graduation. Eighty five percent of the nurses thought that patients' pain score would be higher than nurses. At the moment of pain assessment, 34.4% of the nurses thought the most important factor was physiological change. The most applicated pain intervention method was analgesic medication. They thought that interesting pain intervention method were change of interest, psycotherapy, hypnotherapy, music therapy, relaxation technique, gi therapy, exercise therapy, supporting therapy, hot application and cold application. They thought that new pain intervention methods were gi therapy, hypnotherapy, imagenation therapy, music therapy and relaxation technique. Only seventeen percent of the nurses had practiced new pain intervention method to the patient. 2. The relation among characteristics, the level of assertiveness behavior, self-esteem and implementation of pain intervention. Source, graguation, responsibility for pain relief and the level of assertiveness behavior were related to implementation of pain intervention method and showed a statistically significant difference.

2.Prevalence and Epidemiologic Characteristics of Hypertension in a Rural Adult Population.

To determine the prevalence and risk factors of hypertension we performed cross-sectional study in a rural area. We measured BP and body mass index(BMI), and interviewed adult residents over 30-year-old age. 337 males and 357 females participated the survey. BP was checked twice and the mean was used to determine age and sex specific mean BP and prevalence of hypertension. Prevalence of definite hypertension(BP>or=160/95 or on medication) was 14. 0% in males and 17. 7% in females. Risk factor for hypertension was analysed by multiple logistic regression; age (10years: OR=l. 7) and BMI(1 BMI unit: OR=l. 2) were positively associated with hypertension but smoking(1 pack year: OR=0.9) was shown to have inverse relationship.

Placenta increta is a life-threatening complication of pregnancy characterized by invasion of placenta villi into the underlying myometrium. Usually, presentation is in the early postpartum period with hemorrhage during difficult placental removal. Although placenta increta may complicate first and early second-trimester pregnancy loss, this lesion is rarely found, whose diagnosis can be very difficult during these trimester. We had experienced a case of placenta increta which was found about 14 days after dilatation and curettage (D and C) due to missed abortion at private obstetrics' clinic and report this with brief reviewed the literatures.

8.A study on epidemiologic characteristics and factors associated with excess hospitalization of the patients injured by traffic accident.

The occurrence of injuries and death caused by traffic accidents have been increasing during last few decades in Korea, which resulted in enormous personal, social and economic loss in terms of death, disability, labour days and medical costs incurred for treatment. Generally, it is well known and even became an hot sociopolitical issue that the duration of hospital stay for traffic accident patients are much longer than that of other patients with the similar condition. The objective of the study is to find out epidemiologic characreristics and management pattern of traffic accident patients in a general hospital of medium-sized, industrialized town. This study was carried out on 177 cases injured by motor vehicle accidents, who were admitted and treated in a general hospital from Feb. 1, 1997 to Sept. 30, 1997. The results obtained are as followings: The most of the patients(44%) were 20-30 years old age group although the occurrence of injuries was more frequent among older ages, 50's for male and 60's for female being 2.1 times more frequent among male. The traffic accidents occurred most frequently(14.1%) in the morning(06:00-08:00) and afternoon(14:00-16:00), and on Saturdays(20.3%); the (14.1% traffic accidents were frequent during weekends for passengers whereas it was more frequent during weekdays for pedestrian. Old and child pedestrians were most vulnerable subjects to traffic accidents. The most common site injured was head(19.9%) and the injury types were fracture of lower extremity(9.3%), skull fracture(8.6%) and rib fracture(8.6%) for both sexes; rib fractures/thoracic spine fractures(12.0%) for female and tibia/ fibula fractures(11.6%) for male. Mean days of hospital stay presumptively judged by the doctor who examined, made diagnosis and admitted the patient was 43.8 days compare to 38.4 days of actual mean days of hospitalixation. On the other hand, the optimal mean days of hospital stay for the patients determined by two doctors independently was estimated to be 26.6 days. Thus the difference between actual days of hospitalization and optimal days of hospitalization was 11.7 days, which can be regarded as excess hospitalization days. It consists 30.5% of actual hospitalization days, an enormous waste of resources. The only factor associated with this excess hospitalization was payment responsibility for the hospital cost; when the hospitalization expenses are to be paid by the person inflicted the injury there were always excess days of hospitalization regardless of insurance status.

9.The Association of Serotonin in Labor.

OBJECTIVE: Although a common clinical problem, no uniform definition for fear of childbirth has been settled. The purpose of this study was to identify the association of serotonin in labor. METHODS: A prospective study of serotonin and review of the medical records from the department of obstetrics and gynecology, Catholic University of Korea Holy Family Hospital from January 2004 to May confirmed 30 patients with the diagnosis of singleton pregnancy without medical or obstetrical complications, being in 37-42 weeks. Blood sampling of serotonin was done during labor according to cervix dilatation. Sample collection series on patients latent phase (before labor begins S1), active phase (cervix dilatation 3-5 cm S2), second stage (cervix dilatation full S3), third stage (at delivery of placenta within 24 hours S4), cord artery blood (S5) could be administered. Patient were divided into two groups. From this group 17 patients (G1) who were initial serotonin normal concentration (1.5-7.5 ng/mL), and 13 patients (G2) who were initial serotonin high concentration. RESULTS: The concentration of serotonin of G1 were S1-5.8 +/- 0.8 ng/mL, S2-7.2 +/- 2.5 ng/mL, S3-8.5 +/- 3.7 ng/mL, S4-6.5 +/- 1.5 ng/mL, S5-4.9 +/- 1.2 ng/mL. The serotonin concentration of G2 were S1-10.1 +/- 2.7 ng/ mL, S2-11.2 +/- 5.2 ng/mL, S3-10.3 +/- 3.9 ng/mL, S4-11.8 +/- 8.6 ng/mL, S5-5.0 +/- 1.0 ng/mL. CONCLUSION: The association of serotonin in labor was studied that the change of serotonin concentration was determined according to initial serotonin concentration. This difference of initial serotonin concentration will be studied about genetic polymorphism and factor by molecular biology.

10.A Case of Orbital Organizing Hematoma Presenting as a Chalazion.

PURPOSE: Organizing hematomas (hematic pseudocysts) of the orbit are usually the consequence of direct blunt trauma and are important in the differential diagnosis of orbital cystic lesion. Herein, we report a case of orbital organizing hematoma masquerading as a chalazion. CASE SUMMARY: A 12-year-old female visited our clinic complaining of left lower eyelid swelling. CT scan and MR imaging showed a mass detected in the inferomedial space of her left orbit, which did not invade the adjacent tissue. Excisional biopsy of the orbital mass was performed. Histological examination showed the accumulation of blood-breakdown products within a thick fibrous capsule without epithelial or endothelial lining. The mass was diagnosed as an organizing hematoma. CONCLUSIONS: An orbital organizing hematoma can present, although rare, as a chalazion-like eyelid mass. Careful clinical and radiological examinations can help in making a differential diagnosis.
