1.Clinical and pathological analysis of hepatitis B virus-associated glomerulonephritis in 20 cases

Objective To understand the clinical and pathological features of hepatitis B virus-associated glomerulonephritis(HBV-GN).Methods Twenty children with HBV-GN were analysed including 17 cases of nephritic syndrome (4 cases of simple nephrosis, 13 cases of nephritic nephrosis),2 cases of isolated proteinuria, 1 case of nephritic syndrome according to the clinical classification.Results On the basis of pathology of kidney biopsy, there were 15 cases of membranous glomerulonephritis(MN),3 cases of manbranoproliferative glomerulonephritis(MPGN),2 cases of mesangial proliferative glomerulonephritis(MsPGN).Eight of 20 cases were treated with recombinated human alpha-interfeon. The average follow-up time was 5.7 years. As the result, complete remission clinically was on 5 cases,partial remission on 2 cases and ineffectiveness on one case. Besides, serum HBsAg, HBeAg were converted to negative on 3 cases , HBeAg disappeared and anti-HBe appeared on one case.Conclusion MN is common in childhood with HBV-GN.It also indicates the prognosis of the associated nephropathies is quite favorable.
2.Clinical and Pathological Analysis of Complement 1q Nephropathy in 10 Cases of Children

Objective To explore the relationship between clinical and pathological changes of complement 1q(C1q) nephropathy. Methods Clinical manifestation, pathologic features including glomerulus change, renal tubule - interstitial change and im-munopathology were compared between 10 cases of C1q nephropathy in children, who were diagnosed by renal biopsy. Results Presentation included idiopathic nephritic syndrome(6 cases), simple hematuria(2 cases), nephritic syndrome(1 case), rapidly progressive glomerulonephritis( 1 case); Renal biopsy revealed focal segmental glomerulosclerosis( FSGS) in 5, minimal-change disease( MCD) and mesangial proliferative glomerulonephritis (MsPGN) respectively in two and crescentic glomerulonephritis in one. In addition, there were renal - tubule interstitial changes with 3 cases of grade I and grade II each other, 2 of grade III , 1 of grade IV . The prominent immunofluorescent features was the presence of bright mesangial deposition of C1q. The average follow - up time was 25.7 months. Six cases presenting nephrotic syndrome were resistant to steroid, but 5 were released after immunosuppressive therapy, the other had progressive renal insufficiency. Conclusions C1q nephropathy falls with the clinical - pathologic spectrum of FSGS generally. It is also presented as steroid - resistant nephritic syndrome. Moreover, the prognosis of C1q nephropathy is related to renal tubulointerstitial pathologic lesions not to C1q deposition.
3.Relationship between parathyroid hormone changes in patients with chronic renal failure and cerebrovascular events

Objective To investigate the relationship of iPTH levels, calcium and phosphorus metabolism with cardiovascular event status in the chronic renal failure (CRF) patients. Methods This article retrospectivly studied the clinical and laboratory data of 87 CRF patients. 66 patients of A group didnt occur cardiovascular events,and 21 cases of B group occurred cardiovascular and cerebrovascular events. This article analyzed iPTH levels,serum calcium, phosphorus, calcium and phosphorus product and therelationship between cardiovascular and cerebrovascular events between the two groups. Results Decreased iPTH,increased serum C reactive protein and increased lipoprotein(a) had relationship with cardiovascular and cerebrovascular events in patients with chronic renal failure,but calcium,phosphorus,calcium and phosphorus product has nothing to do with the cardiovascular and cerebrovascular events. Cardiovascular and cerebrovascular events increased significantly in dialysis patients than in non - dialysis patients. Conclusiou Chronic renal failure patients on dialysis levels of serum iPTH, CRP level, LP(a) level and dialysis could lead to cardiovascular and cerebrovascular events.
4.Intramedullary pin versus plate for treatment of displaced mid-shaft clavicle fractures: a meta-analysis

Objective To compare the clinical efficacy of intramedullary pin versus plate fixation for displaced mid-shaft clavicle fractures.Methods The databases,PubMed,CENTAL and CBM,were searched according to the standards of the Cochrane Collaboration for all the randomized controlled trials (RCTs) and clinical controlled trials (CCTs) comparing intramedullary pin with plate fixation for treatment of displaced mid-shaft clavicle fractures.In addition,hand search was also conducted in relevant journals.Time limit for search was from the beginning till December 2013.Trial quality was assessed using the modified Jadad scale and effective data were pooled for Meta analysis with software RevMan5.2.7,including wound infection,hospital stay,wound size,intraoperative blood loss,operation duration,implant irritation,implant problems,delayed union,malunion,nonunion,total complications and Constant scoring.Results Three RCTs and 7 CCTs with 697 patients were included in our study.The modified Jadad scoring showed fine quality of the 10 trials.Intramedullary fixation was significantly superior to plate fixation in wound size,intraoperative blood loss,operation duration,hospital stay,wound infection,and symptomatic hardware (P < 0.05),but led to a significantly higher rate of implant problems (P < 0.05).However,no significant differences were identified in total complication rate,delayed union,nonunion,malunion or Constant score (P > 0.05).Conclusions The superiority of intramedullary fixation for displaced mid-shaft clavicle fracture lies in its advantages of minimally invasion,such as limited incision,less intraoperative bleeding,shorter operation time and hospital stay,less wound infection and implant irritation.Ahhough intramedullary fixation may lead to a higher rate of implant problems,it shows no significant differences from plate fixation in bone healing and functional recovery.
5.Model of Salvia miltiorrhiza extraction by alcohol based on ANFIS

AIM:The model of Salvia miltiorrhiza extracted by alcohol based on adaptive network-based fuzzy inference system(ANFIS)was studied in this paper.METHODS:The ANFIS model was set up by a set of experimental data which were gained on a pilot extraction condition.Then the tanshinone ⅡA content of the alcohol extract from Salvia miltiorrhiza in processing conditions could be predicted by this ANFIS model.The Radix Salvia miltiorrhiza was extracted by alcohol thermal reflux;then initially created a fuzzy inference system model based on network and trained by mixing learning arithmetic.RESULTS:The relative errors between the predicted values and the experimental values were in a allowable range,there were only 4 points greater than 10% of totally 90 calibration points.CONCLUSION:The analytical and experimental results show that the method can effectively forecast the content of the effective ingredient according to preset extraction conditions.

8.Effect of pseudolaric acid B on proliferation of human LiBr cell line in vitro

Object To study the inhibitory effects and mechanism of pseudolaric acid B (PAB) on the growth of LiBr cell line. Methods PAB in different concentrations was added into the medium in which LiBr cells were cultured. The changes of cell morphology were observed by phase contrast microscope. Cytotoxicity of PAB was detected by MTT. The number of apoptotic cell was calculated by double fluorescent staining. The level of p21 WAF1 protein was measured with immunocytochemistry. Results The growth of LiBr cell line was remarkably inhibited by PAB. The IC 50 values of PAB for LiBr cells was 2.5?10 -5 mol/L. The inhibitory rate and the apoptotic cell rate were related with the medicine concentration. The level of p21 WAF1 protein in LiBr cells treated with PAB was obviously increased. Conclusion PAB can effectively inhibit the proliferation of LiBr cell line.
9.Selection of flaps for the reconstruction of facial skin defects

Objective To investigate the selection of flaps for the reconstruction of facial skin defects.Methods A total of 40 patients with facial skin defects were treated from February 2011 to February 2015 in our hospital,and they were given different surgery methods ac-cording to the different facial skin defect sizes.Minor defects were primarily treated by direct suturing;medium-sized defects were treated by local skin flap transposition or island skin flap;and larger defects were treated by expanded skin flap.Results All patients were followed up for 1 to 2 years.All the flaps survived with good color and texture match.All the scars healed well which were almost invisible.There was no obvious deformity in the donor and recipient sites.In addition,there was no lesion recurred.Conclusion Select suitable flaps for the recon-struction of facial skin defects according to the face defect area could get reliable blood supply of the flap,inapparent incision scar and a high level of satisfaction.
10.Application of hypocaloric parenteral nutrition in postoperative elderly patients with gastric cancer

Objective To investigate the application of hypocaloric parenteral nutrition in postoperarive elderly patients with gastric cancer. Methods 59 elderly patients aged 60- 79 years with gastric cancer after operation were randomly assigned to receive 84 kJ · kg -1.d -1 hypocaloric parenteral nutrition (29 cases) and 126 kJ · kg-1 · d -1 standard-calorie parenteral nutrition (30cases),totally 6 d from postoperative 2 d. Complications, serum proteins, immune function,inflammation,blood glucose, liver function and recovery time of gastrointestinal function were observed during nutritional support. Results The postoperative complication rate were 37.9%(11/29) in hypocalorie group and 43.3 % (13/30) in standard calorie group ( P>0.05),mainly pulmonary infection.There were no ditferences in levels of serum albumin,total protein and prealbumin between two groups at postoperative 3 d and 6 d (P>0.05).Peripheral blood lymphocytes and T lymphocyte subsets CD3+,CD4+,CD4+/CD8- ratio were decreased after sugery,but CD8 1 values increased in two groups (P>0.05).C reactive protein was increased significantly at postoperative 3 d compared with preoperation,and decreased at 6d than 3 d (P>0.05).Blood glucose was decreased gradually at 3 and 6 d (P>0.05).Postoperative liver function paraneters were elevated,no difference was found at 3 d (P>0.05),and continued to rise at 6 d in standard calorie group than in hypocalorie group (P<0.05). There was no difference in times of aerofluxus and cacation after operation between hypocalorie and standard calorie groups (P>0.05). Conclusions Hypocaloric parenteral nutrition is a safe and effective method for nutrition support in postoperative elderly patients with gastric cancer.
