1.Hydrophobicity Test and DNA Probe Hybridization Assay in the Detection of Enterotoxigenic Escherichia coli.

The hydrophobicity assay and DNA probe hybridization assay were compared for analysis of enterotoxigenic Escherichia coli(ETEC), heat-labile enterotoxin(LT) and heat-stable enterotoxin (ST). The ETEC isolated from diarrheal patients were serotyped and investigated for the presence of colonization factor antigens CFA/1, CFA/II, CFA/III and CFA/IV with the expression of mannose-resistant hemagglutination(MRHA) and the levels of surface hydrophobicity. The following results were obtained. 1. Out of these 48 strains, 34 strains were found to be positive for LT production by DNA probe hybridization assay. Out of 34 strains, 1 strain was ST producer, 25 strains were LT producers, and 8 strains were produced both ST+LT producers by DNA probe hybridization assay. 2. Out of 34 strains of positive DNA probe hybridization test, 31 strains was positive in the hydrophobicity test. Among strains of positive hydrophobicity test, 20, 1, and 7 strains produced only LT, only ST and both ST-LT, respectively. Screening efficiency for identifying ETEC by salting out test was 82.4% in sensitivity and 78.6% in specificity. For ETEC detection, the hydrophobicity assay was the least sensitive but was simple, rapid and a good substitute for the DNA probe hybridization assay. 4. CFAs were identified in 43.8% of ETEC strains; 2.1% of the CFAs strains with CFAs harbored CFA/I, 29.2% carried CFA/II, 16.7% carried CFA/III and CFA/IV. And 35.4% expressed none of these CFAs. CFA/I was found in ETEC of serotype 0128: K67, CFA/II was 0128: K67, 0142: K+ and 0159: K+, CFA/III was 086a: K15 and 0128: K67, CFA/IV was 0 86a: K15, 0128: K67, 0125: K70 and 0148: K+.

3.Histopathologic Observation on Hydrofluoric Acid Burn Using Guinea Pigs.

BACKGROUND: Hydrofluoric acid(HF) is one of the strong stirritating, corrosive and poisonous inorganic acid, The HF burns are occuring with ever-increa firifrequency due to the wide use of this acid in industrial fields, such as electronics and semicor tor industries. OBJECTIVE: The purpie of this study is to quantiate theound depths produced by varying concentrations of HF aiud to suggest the treatment guideline. METHODS: Uarious cinicentrations of HF(5%, 25%, 50%),were applied to two pairs of test sites on a guinea pig's back in a uniform fashion with cotton-tip applicators and then the histopathologic changes of guinea pig skin were observed. RESULTS: 1, In the 5% HF applied group, intracellular edema vaobserved in the epidermis after 12 hours. After 3days, epidermal cell necrosis was observed. 2. In the 25% HF applied group, intraepidermal vesice was observed after 6hours. After 12hours, inflammatory cell infiltrations were observed in the apper dermis and then after 3days, dermal necrosis was oberved. 3. In the 50% HF applied group, epidermal cell necros After 6hours, liquefaction degeneration in the epidermis an was observed focally after 3hours. iflhmmatory cell infiltrations in the upper dermis were observed. CONCLUSION: This study showed that a higher concentrator HF exposure enhances The penetration of HF. For treatmi not alcohloic solution of a quarternary ammonium for avintralesional injection of calcium gluconate within 24 hours of HF and a longer period after we suggest wet dressing with an concentration of HF burn and high concentration of HF burn.

4.Treatment of a Difficult Periarticular Fracture of the Knee with a Circular External Fixator.

The purpose of this study was to evaluate the results and complications in difficult periarticular fractures of the knee treated with a circular(Ilizarov) external fixator. The mean follow-up period was 34 months(range: 12-55 months). Of the 27 cases, 12 cases involved fractures of distal femur (two M ller type A3, four type C2, six type C3) and 15 cases involved fractures of the proximal tibia (one Schatzker type V, eight type VI, two segmental, four metaphyseal comminuted fractures). Twenty-two cases(81%) were open fractures and most of the cases were open intra-articular comminuted fractures. The average duration of external fixation was 7.7 months (9.8 months for the distal femur cases, 6.5 months for the proximal tibia cases and 11.5 months for the cases which required lengthening, and 6 months in the non-lengthening group). In 8 cases, in which bone lenthening was done, no leg length discrepancy occurred after lengthening. The mean length gained was 5.9 cm and the mean healing index was 1.8 months/cm (1.8months/cm in distal femur cases and 1.9 months/cm in proximal tibia cases). According to Neers criteria, the treatment results for cases involving distal femur fracture were as follows: six satisfactory, three unsatisfactory, three failures; cases with proximal tibia fractures had these results: nine excellent, three satisfactory, three unsatisfactory. Average range of motion was 67 degrees in distal femur fracture cases and 114 degrees in proximal tibia fracture cases. According to Paleys complication grading system, six problems, six obstacles, and eight com- plications occurred. In conclusion, the Ilizarov external fixator can be a useful treatment method in periarticular fractures of the knee with severe intra-articular or metaphyseal comminution, bone loss, severe soft tissue injury, or multiple associated injury. Its use can reduce the associated complications such as leg length discrepancy, infection, and nonunion.

More than half of the cryptococcal infections occur in acquired immune deficiency (AIDS) patients, and more than half of the non-AIDS patients with cryptococcosis are immunocompromised. Most immunocompromised patients have meningoencephalitis at the time of diagnosis. Without the appropriate therapy, this from of the infection is invariably fatal. Death can occur any time from 2 weeks to several years after the onset of symptoms. Pulmonary crytococcosis in immunocompromised patients is usually asymptomatic, but coughing, chest pain, fever, or hemoptysis may occur in immunocompetent patients. Pulmonary cryptococcosis symptoms in immunocompetent patients tend to improve without treatment. Here, we describe the various pulmonary manifestations of cryptococcal pneumoniae in three immunocmpetent patients.

6.Three Cases of Hereditary Angioedema in One Family.

Hereditary angioedema is a rare genetic disease transmitted with a autosomal dominant trait result of a quantitative or functional defect of Cl inhibitor. We report three cases of heriditary angioedema from the same famly respectively the 35-year-old sister, the younger bn)ther of 32, and the younger sister of 20 The older sister died due to her airway being obstructel by acute laryngeal edema, and her you igirbrother and sister experienced recurrent episodes oswelling on the hand and forearm. The ore we examined the two patients serum complement levils to confirm diagnosis. In both, the r;ults showed decreased levels of Cl inhibitor and C4 which are the evidence of hereditary a giedema. The younger sister has been administered danazol (600mg/day) to prevent angioedem or 8 weeks, and we have not yet found a recurrenc of he symptoms. 7.A Case of Acquired Dacryocystocele Treated by Lacrimal Silicone Intubation.

PURPOSE: To report a case of an acquired dacryocystocele successfully treated with bicanalicular silicone intubation and to review relating literature. CASE SUMMARY: A 17-year-old girl visited our clinic with tearing of both eyes since birth and a mass on the right medial canthal area for 2 years. A firm, non-tender mass with a well-demarcated border was palpated in the subcutaneous level just inferior to the right medial canthal ligament. Lacrimal irrigation via the lower punctums showed reflux through the opposite punctums without nasal passage in both of her eyes. Computed tomographic scan showed a widening of the right lacrimal sac fossa and bony nasolacrimal canal and a 16 x 18 mm sized cyst-like mass in the right lacrimal sac. The patient was diagnosed with right acquired dacryocystocele associated with bilateral congenital nasolacrimal duct obstructions. After opening of the obstructed common canaliculus using a fine lacrimal probe, silicone intubation was performed. The tearing symptom improved and the mass disappeared during the subsequent follow-up period of 1 year. CONCLUSIONS: When only accompanied by distal nasolacrimal duct obstruction, acquired dacryocystocele can be inferred to be associated with congenital nasolacrimal duct obstruction. Subsequently, bicanalicular silicone intubation can be considered as a treatment of choice.

8.Achondrogenesis Type 2: An autopsy case.

Achondrogenesis type 2 is a lethal form of congenital skeletal dysplasia characterized by severe short-limbed dwarfism, decreased vertebral ossification and normal ossification of the skull. We report an autopsy case of achondrogenesis type 2 in a female fetus terminated at 29 weeks of gestation. External morphology revealed a relatively large head, short upper and lower extremities, short neck, and distended abdomen. The x-ray finding showed normal calvarial ossification, hypoplastic ilium and unossified ischium, and metaphyseal flares of the femur and tibia. Histologically, chondrocytes were large and irregular with increased vascularity.

9.Hyperkeratosis of the nipple and areola surgical excision and skin graft.

Hyperkeratosis of the nipple and areola is a rare skin disordr characterized clinically by thickening and discoloration of the skin of the areola and/or nipple and pathoiog cally by papillomaosis or acanthosis-nigricans like changes. We report a case of hyperkeratosis of the nipple and areola occuring in a woman with no underlying endocrinopathy and no other additional hyperkeratotic conditions. Ttie iresent case seems to be the nevoid form by the Levy-Franckel classification. Treatment with topical medications including steroids and keratolytic agents produced no improvement. Therefore we recommeneded surgical excision because the paient was greatly disturbed by the appearance of the lesions. Surgical excision and full thickness skin graft were performed sucessfully.

10.Clear Cell Meningioma.

Clear cell meningioma is a recently recognized morphologically unique entity. It shows no sex predilection, affects primarily the lumbar region, and the cerebellopontine angle. Despite its benign appearance, it may be aggressive, particularly in intracranial cases. All lesions are moderately cellular, with the exception of stromal hyalinization. The tumor consists largely of a sheet- like or somewhat lobular pattern of polygonal cells, the cytoplasm of which is clear. No close association is noted between the recurrence or the clinical outcome and factors such as mitotic activity, the PCNA index, and the DNA ploidy status. But the MIB-1 proliferation index is appreciably higher in recurrent tumors. We experienced a case of clear-cell meningioma showing a characteristic histologic finding. A 39-year-old man was admitted due to the recent onset of right-sided, facial-nerve palsy, left hemiparesis and general weakness. A CT scan of the head showed a well defined mass in the petroclival area. After surgical resection, the patient was in good condition, but 1 year later symptoms recurred. A CT scan of the head showed a huge, recurrent petroclival tumor with adhesion to the surrounding brain parenchyme.
