1.Changes in Awake Cystometry and Expression of Bladder beta-adrenoceptors after Partial Bladder Outlet Obstruction in Male Rats.

PURPOSE: To explore possible changes in awake cystometry and expression of beta-adrenoceptors (ARs) as a cause for bladder dysfunction in a male rat model of partial bladder outlet obstruction (pBOO). MATERIALS AND METHODS: Awake cystometry was performed in rats with pBOO (16) and sham-operated rats (16), 8 weeks after the operation. The expression of mRNA and protein of beta-ARs was assessed by real-time PCR and western blot. RESULTS: The bladders with pBOO (1030mg) were increased compared to those in control rats (230mg). In the cystometric studies, the maximum intravesical pressure significantly increased in the pBOO group compared to control group rats (p=0.001). The time to reach maximal intravesical pressure during micturition in the pBOO group was significantly longer than the sham group (p=0.003). The frequency of non-void contraction in the pBOO group was significantly more than the sham group (p=0.006). The mRNA expressions of beta2- and 3-ARs were increased insignificantly in pBOO group compared to sham group. The data of pBOO group expressed as folds of corresponding expression in sham group were 1.28 and 1.46 respectively in beta2- and 3-ARs. Compared to the sham groups, the density of 60Kda protein band recognized by beta2-AR antibodies and the density of 45Kda protein band recognized by beta3-AR antibodies were higher in the bladder from pBOO group rats. CONCLUSION: PBOO of male rats increase the maximal intravesical pressure and contraction time during micturition and the frequency of non-void contraction as well as weight of bladder. The expression of beta2- and 3-ARs subtypes was increased insignificantly compared to sham operated group. This study demonstrates that the changes of cystometric or non-void contraction parameters in pBOO is one of the pathophysiologic processes potentially associated with the alterations of bladder beta-ARs.

2.The Effect of Mitomycin C Instillation after Silicone Intubation in Adult Partial Nasolacrimal Duct Obstruction.

PURPOSE: To compare the result of mitomycin C (MMC) instillation after silicone intubation in partial nasolacrimal duct obstruction in adults according to duration of instillation. METHODS: An instillation of 0.04% MMC eyedrops was performed on 61 eyes of 38 patients diagnosed with partial nasolacrimal duct obstruction. The clinical result of patient groups, classified according to duration of MMC eyedrops instillation, was analyzed prospectively. The patients groups was as follows: no MMC eyedrop instillation (group A), 1 week of MMC instillation (group B), 2 weeks of MMC instillation (group C), and 3 weeks of MMC instillation (group D). RESULTS: Among all patients, 50 eyes (81.9%) showed improved symptoms. Fifteen eyes (71.4%) in group A, 8 eyes (72.7%) in group B, 13 eyes (86.7%) in group C and 14 eyes (100.0%) in group D had a successful outcome. The success rate was observed in groups D, C, B, and A, in descending order, which was statistically significant (p = 0.025). CONCLUSIONS: Silicone intubation is an effective treatment option for partial nasolacrimal duct obstruction in adults and MMC eyedrop instillation may improve silicone intubation results.

3.Does the Incidence of Urgency Symptoms Increase Along with the Severity of Stress Urinary Incontinence?.

PURPOSE: This study aimed to determine whether symptoms of urinary urgency increase according to the severity of stress urinary incontinence (SUI). For this purpose, we recruited women with symptoms of mixed as well as pure SUI and compared the clinical characteristics of each subgroup. MATERIALS AND METHODS: A total of 241 female patients who were diagnosed with SUI and mixed urinary incontinence (MUI) were analyzed retrospectively. Patients with only SUI were categorized as group 1. Patients with MUI were categorized as group 2. Clinical and urodynamic differences between the 2 groups were compared. RESULTS: The proportion of Stamey grade was significantly different between the 2 groups: grade 1 SUI was higher in group 1, but grades 2 and 3 SUI were higher in group 2. The incidence of urgency was proportional to the degree of Stamey grade (23.5% in grade 1, 36.9% in grade II, and 60.0% in grade III). In the urodynamic study, the presence of detrusor overactivity was significantly higher in group 2 than in group 1 (25.9% vs. 49.4%). Other clinical parameters were also significantly different between the 2 clinical groups: Q-tip angle (group 1: 42.1degrees, group 2: 28.6degrees, p<0.05), maximal urethral closure pressure (group 1: 54.7 cmH2O, group 2: 44.1 cmH2O, p<0.05), maximal bladder capacity (group 1: 356.3 ml, group 2: 282.0 ml, p<0.05), and bladder volume at first desire (group 1: 144.6 ml, group 2: 123.2 ml, p<0.05). CONCLUSIONS: According to this analysis, the more serious the symptoms of SUI, the higher the incidence of urinary urgency.

4.Comparison of Dacryocystographic Results Before and After Silicone Intubation in Incomplete Nasolacrimal Duct Obstruction.

PURPOSE: To compare the dacryocystographic results before and after silicone tube intubation in partial nasolacrimal duct obstruction. METHODS: Dacryocystography was performed on 33 eyes of 17 patients diagnosed with partial nasolacrimal duct obstruction. The anteroposterior (AP) diameters and the mediolateral diameters of the nasolacrimal ducts intubated at the operation were measured by dacryocystography, before the operation and after silicone tube removal. RESULTS: The mean AP, mediolateral diameter and length of nasolacrimal duct in the group who demonstrated improvement after the operation was 2.32 mm, 1.39 mm, and 17.14 mm before the operation, and 2.40 mm, 1.77 mm, and 17.38 mm after the operation, respectively. The mean AP, mediolateral diameter and length of nasolacrimal duct in the group who demonstrated no symptomatic improvement was 2.06 mm, 1.28 mm, and 17.42 mm before the operation, and 2.75 mm, 1.99 mm, and 18.03 mm after the operation, respectively. The alteration of the nasolacrimal duct size in the group with successful postoperative results compared with unsuccessful postoperative results showed no significant difference. CONCLUSIONS: The nasolacrimal duct showed expansion in size based on dacryocystographic results after silicone tube intubation in partial nasolacrimal duct obstruction. However, the operation results and the alteration of the nasolacrimal duct size based on dacryocystographic results demonstrated no accordance.

9.New Insulin Injection Recommendations.

Based on the 4th Injection Technique Questionnaire results, new insulin injection technique recommendations were announced at the Forum for Injection Technique and Therapy: Expert Recommendations (FITTER) workshop held in Rome, Italy, on October 23 and 24, 2015, in which 183 physicians, nurses, educators, and allied healthcare professionals from 54 countries attended. Through these new recommendations, we hope to identify and localize the new insulin injection technique recommendations that could be applicable in local clinical settings. It is recommended to use a pen needle with a wider inner diameter when its gauge remains the same. Also, recommended injection sites such as the abdomen, upper arms, thighs, and buttocks are well described based on anatomical landmarks. The insulin absorption rate is the fastest at the abdomen, upper arms, thighs and buttocks in the case of human insulin; however, there is no difference in absorption rate in the case of insulin analog, regardless of site selection. Also, air-shooting is not necessary if drops are observed as soon as the needle is attached to the pen. Diabetes educators should be familiar with new insulin injection technique recommendations, not only to keep themselves updated with new knowledge, but also so they can educate patients to assure patient safety and achieve better outcomes.

10.Effects of Vitamin D Supplementation on Testosterone, Prostate, and Lower Urinary Tract Symptoms: A Prospective, Comparative Study

Purpose:
Several studies have associated the serum vitamin D level with total testosterone levels and the prostate volume. Herein, we investigated the effect of vitamin D supplementation on testosterone, prostate, and lower urinary tract symptoms (LUTS) in men.
Materials and Methods:
Men over 40 years of age diagnosed with a vitamin D deficiency (25[OH]D <20 ng/mL) who received vitamin D supplementation for one year were included in the study and administered 25,000 IU of cholecalciferol every 2 weeks. Prostate ultrasound, uroflowmetry, postvoid residual urine volume measurement, and serological tests (serum testosterone levels, etc.) were performed upon diagnosis and one year later. Participants also answered the International Prostate Symptom Score (IPSS) and Aging Males’ Symptoms Scale (AMS) questionnaires.
Results:
A significant increase was observed in the vitamin D level following one year of vitamin D supplementation, with a significant decrease in the postvoid residual urine volume, total IPSS score and without a significant change in the prostate volume. Improved psychological subscale score of AMS questionnaire was observed with a statistical significance.
Conclusions
Vitamin D supplementation suppressed the increase in the prostate volume and improved the LUTS. Although there is no direct effect on serum testosterone levels, vitamin D supplementation helped improve hypogonadal symptoms.
