1.The Significance of Interleukin-6 in Henoch-Schonlein Purpura Nephritis in Children.

Carcinsarcoma is an uncommon pulmonary malignancy characterized by carcinmatous parenchyma and sarcomatous stroma. The cytologic, immunohistochemical and ultrastructural features of a case of pulmonary carcinosarcoma suspected by fine needle aspiration cytology is presented. Only bizarre spindle cells arranged in loose groups, in microtissue fragments and in a dissociate fashion were present in the aspiration smears. They were markedly positive for vimentin. The epithelial component was not found, which was probably due to marked paucity of carcinomatous component that was proved by histologic examination of the resected tumor. The diagnosis of pulmonary carcinosarcoma should be conidered whenever poorly differentiated epithelial cell groups with a malignant mesenchymal component set in a myxoid background are seen in a pulmonary cytology specimen.

5.A Case of Systemic Lupus Erythematosus coexistent with Diffuse Subcutaneous Tissue Calcification.

Systemic lupus erythematosus (SLE) is a disease of unknown etiology in which tissues and cells are damaged by pathogenic autoantibodies and immune complexes. Skin manifestations of SLE include malar rash, discoid rash, photosensitivity, oral ulcer, panniculitis, urticaria, bullae, erythema multiforme and lichen planus-like lesions. It has long been recognized that dystrophic soft tissue calcification may occur in association with certain connective tissue disorders such as scleroderma or dermatomyositis. Soft tissue calcification in a patient with SLE has been rarely reported. We have experienced a patient with SLE who presented with diffuse subcutaneous tissue calcification on face, chest, abdomen and all extremities.

7.The bone mineral densities in Korean postmenopausal women: a comparison between natural and surgically induced menopause.

OBJECTIVE: To investigate the bone mineral density(BMD) in Korean postmenopausal women. METHODS: We performed the retrospective study for 1,508 postmenopausal women who had menopause after 40 years of age and didn't take any hormonal agent before measuring bone mineral density. Spinal and femur neck BMD were measured using dual energy X-ray absorptiometry(DXA, Lunar Co.). RESULTS: The mean age of the study subjects was 53.7+/-5.7 years and the average BMD of the lumbar spine(L2-L4; L24) and femur neck(FN) were 0.996+/-0.173 g/cm(2) and 0.821+/-0.133 g/cm(2), respectively. Of all subjects, 44.1%(665/1,508) had osteopenia and 14.7%(222/1,508) were already osteoporotic. The prevalence of osteoporosis increased with age; 5.2% in 41-50 years of age, 15.6% in fifties, 32.5% in sixties, and 45.5% in over 70 years of age. There was no significant difference in the age-adjusted BMD between naturally menopausal women(n=828) and surgically menopausal women(n=78). The mean age at menopause of surgically menopausal women was significantly younger than that of naturally menopausal women(47.6+/-3.6 vs 49.2+/-3.5). Duration of menopause, body weight, and height were revealed to be the significant risk factors for osteoporosis. The annual bone loss rates in lumbar spine and femur neck were 1.2% and 1.1% respectively in naturally menopausal women, and 1.5% and 1.2% in surgically menopausal women. CONCLUSIONS: This study established the BMD curve and regression equation in Korean postmenopausal women according to the causes of menopause. There was no significant difference in the mean BMD and the prevalence of osteoporosis between naturally and surgically menopausal women. Duration of menopause, body weight, and height were found to be the risk factors which influence the development of the osteoporosis. So the postmenopausal women who are at increased risks of developing osteoporosis need more active interventions for preventing the osteoporotic fracture.

8.High-spatial-resolution, instantaneous passive cavitation imaging with temporal resolution in histotripsy: a simulation study

Purpose:
In histotripsy, a shock wave is transmitted, and the resulting inertial bubble cavitation that disrupts tissue is used for treatment. Therefore, it is necessary to detect when cavitation occurs and track the position of cavitation occurrence using a new passive cavitation (PC) imaging method.
Methods:
An integrated PC image, which is constructed by collecting the focused signals at all times, does not provide information on when cavitation occurs and has poor spatial resolution. To solve this problem, we constructed instantaneous PC images by applying delay and sum beamforming at instantaneous time instants. By calculating instantaneous PC images at all data acquisition times, the proposed method can detect cavitation when it occurs by using the property that when signals from the cavitation are focused, their amplitude becomes large, and it can obtain a high-resolution PC image by masking out side lobes in the vicinity of cavitation.
Results:
Ultrasound image simulation confirmed that the proposed method has higher resolution than conventional integrated PC imaging and showed that it can determine the position and time of cavitation occurrence as well as the signal strength.
Conclusion
Since the proposed novel PC imaging method can detect each cavitation separately when the incidence of cavitations is low, it can be used to monitor the treatment process of shock wave therapy and histotripsy, in which cavitation is an important mechanism of treatment.
10.C. T. arthrography on Bankart lesion

Accompanying to the development of the knowledgment of mechanism and pathology of recurrent dislocation of the shoulder, accurate diagnostic tools to detect that pathology have been developed. During the period from May 1990 to May 1992, we treated 28 cases of recurrent anterior dislocation of the shoulder. Twenty-eight cases were examined with C.T. arthrography before the operation. Among the 28 cases, we identified the Bankart lesion in 20 cases by C.T. arthrography, and in 21 cases intraoperatively. We concluded that the C.T. arthrography is an accurate method for confirming the clinical diagnosis of recurrent dislocation of the shoulder and to be helpful in detecting the Bankart lesion and its severity or pathologic findings.
