4.Breast Cancer Risk and Dietary Factor: A Case-Control Study.

PURPOSE: This study was performed to evaluate the effect of dietary factors on the risk of breast cancer in Korean women. METHODS: New histologically proven breast cancer cases (n=108) were selected at Hanyang and Soonchunhyang University Hospitals in Seoul. Controls (n=121) were selected from patients in the Departments of Plastic Surgery, General Surgery and Opthalmology at the same hospitals by frequency matching of age and menopausal status. Informations on demographic, reproductive, and dietary factors were collected by an interviewer. Food intake was assessed by using food frequency questionnaires. The association of dietary factors with breast cancer risk was analyzed using total nutrient intake and food intake based on menopausal status. The statistical analysis was done by using unconditional logistic regression after adjusting for age and other confounding factors. RESULTS: The results were as follows: Breast cancer risk significantly decreased with consumption of the following nutrients: fiber in postmenopausal women, vit C in postmenopausal and total women, and vit E in premenopausal women. However, neither total energy nor fat intake was associated with development of breast cancer. Grape and pepper intake in premenopausal women and kimchi intake in total women were significantly associated with decreasing risk of breast cancer. Overall consumption of vegetables and fruits was associated with decreased breast cancer risk. CONCLUSION: Consumption of vegetables and fruits showed a protective effect in breast cancer risk, but neither the intake of fat nor the intake of protein was associated with breast cancer risk in this study. These findings suggest that the consumption of vegetables, and fruits is a protective factor. Further studies with larger numbers of subjects should be conducted to evaluate the association between breast cancer risk and dietary factors.

6.Incidence and Contributing Factors of Malpositioning of the Endotracheal Tube after Endotracheal Intubation with Chest X-ray.

BACKGROUND: Malpositioning of the endotracheal tube within airway after intubation may results in serious complications, such as accidental extubation and inadvertent endobronchial intubation. Therefore, early detection of malposioning of the endotracheal tube is very impotant for deciding patient's prognosis. METHODS: We assessed the distance from the tip of the endotracheal tube to the carina according to the patient's age, sex, and the time, the location, and the route of intubation. The chest x-ray was taken for 333 patients (men, 226: women, 107) who were admitted to the intensive care unit of Wonju Christian Hospital from march 1, 1995 to February 28, 1996. Chest x-ray was obtained after intubation to verify endotracheal tube position. Appropriate endotracheal tube position on chest x-ray was defined as between 2 and 6 cm above the carina. RESULTS: Of the 333 intubations, 106 (31.8%) endotracheal tubes were inappropriately placed according to the chest x-ray. The percentage of malpositioned endotracheal tubes (<2 cm) was higher in women than in men (11.2% vs. 5.3%, p<0.05), with higher in night (7 PM to 7 AM) than in day (7 AM to 7 PM) (42% vs. 24%, p<0.05). Distance from the carina to the tip of endotracheal tube is 4.34 +/- 1.77cm in women and 5.23 +/- 1.64cm in men. Thus, position of the endotracheal tube in women is deeper than men (p<0.001). CONCLUSIONS: We conclude that the chest x-ray for confirmation of endotracheal tube position after endotracheal intubation may remain the standard of practice. And endotracheal tube position should be carefully assessed immediately after tracheal intubation, particularly in women and at night.

7.Two Cases of Recurrent Extramammary Paget's Disease after Wide Local Excision.

We report two cases of recurrent extramarnmary Pagets disease after wide local excision. Both cases involved the scrotum and the penoserotal junction, respectively. On histopathological examination, we could observed many Paget cells confined to the epidermis and the hair follicle, but not invading the underlying dermis in both cases. There was no evidence of internal malignancy. Both cases were treated with wide local reexcision.

9.Is There a Role of RigiScan(R) in the Measurement of Rigidity after Intracorporeal Injection of Prostaglandin E1?.

PURPOSE: To evaluate whether radial rigidity measured using RigiScan(R) represents the intracorporeal pressure effectively. MATERIALS AND METHODS: From January 1998 to May 1999, total of 23 patients with erectile dysfunction were evaluated by RigiScan(R) and duplex ultrasonography after the intracorporeal injection of prostaglandin E1. Peak systolic velocity and end diastolic velocity were measured by duplex ultrasonography and then the resistance index was calculated as (peak systolic velocity-end diastolic velocity)/(peak systolic velocity). Radial rigidity of penile tip and base was measured by RigiScanR . The results were analyzed statistically by PC-SPSS version 7.5. RESULTS: There were statistically significant correlations between radial rigidity of penile tip and base and the resistance index by Spearman's correlation analysis, respectively (r=0.680, p<0.001)(r=0.703, p<0.001). When radial rigidity of penile tip and base exceeded 60% of maximum, radial rigidity of penile tip and base again correlated well with the resistance index, respectively (r=0.659, p=0.020)(r=0.759, p=0.011). Based on clinically determined degree of erection, radial rigidity of penile tip and base represented the intracorporeal pressure effectively. CONCLUSIONS: Radial rigidity measured by RigiScan(R) represents the intracorporeal pressure effectively.

10.A clinical and histologic study of 71 cases of basal cell carcinoma.

A clinical and histologic survey was done on 71 cases of basal cel carcinoma(BCC) filed at Department of Clinical Fathology, Kang Nam, Han Kang, and Kang Dong Sacred Heart Hospital during past 13 year s from 1979 to 1991. The results were summerized as follows: 1. Of all malignant skin tumors BCC accounted for 31.6%. The frequency of BCC in the total number of outpatients visiting the Department of Dermatology was 0.06%. 2. The ratio of men to women with HCC was 1:1.03. 83.8% of the pat.ients with BCC were older than the age of l0 years with the mean age of 56.9 years. 3. 83.8% of the BCC appared in the face, especially on the nose(25.4%), eyelid(20.3%). 4. On the basis of classification of Lever et al, the solid type(66.7% ) was the most com mon histologic pattern, followed by the adenoid(8.8%), pigmented(8.8%), and etc. By the classification of Farmer et al, the nodulocystic type(35.1% ) was the most common histologic pattern followed by infiltrative(31.6%), adenoid(8.8%), pigmented(8.8%), and etc. By the classification on Sexton et al, in the order of decreasing frequency, liistologic subtypes were the nodular(43.9%), mixed (22.8%), infiltrative(19.3%), supeficial(3.5%), morpheic(3.5%), and micronodular(3.5%).
