2.The Relationship between Idiopathic Polyhydramnios and Oligohydramnios and Perinatal Outcomes.
Korean Journal of Perinatology 2006;17(3):287-293
OBJECTIVE:Ultrasound estimation of amniotic fluid volume (AFI) is a critical component of antenatal surveillance. Alterations in AFI have classically been considered an indication of fetal compromise. Thus, this study was undertaken to determine whether there is any association between idiopathic polyhydramnios or oligohydramnios and adverse perinatal outcome. METHODS:Women delivered between December 1999 and November 2005, and who underwent ultrasonography from 26 to 41 weeks gestation were analyzed. Twenty-seven consecutive women with singleton pregnancies complicated by idiopathic polyhydramnios (amniotic fluid index>25) and 31 women complicated by oligohydraminos (amniotic fluid index<5) were included in this study. We reviewed the perinatal outcomes including preterm delivery, birth weight, primary cesarean section rate and indication, Apgar score (1-min and 5-min), and admission to NICU (neonatal intensive care unit). These findings were compared with those of 50 matched control patients with normal amniotic fluid volume. RESULTS:We observed that idiopathic oligohydramnios was significantly associated with primary perinatal outcome (Cesarean delivery for fetal distress) as well as secondary outcome variables such as birthweight, SGA (small for gestational age), Apgar score<7 at 1-min, 5-min, and NICU admission rates. On the other hand, idiopathic polyhydramnios did not correlate with any other perinatal outcome, except for rates of LGA (large for gestational age). CONCLUSION:The present study suggests that AFI is a weak predictor of perinatal outcome than has been classically suggested. But we demonstrated that the AFI identification of polyhydramnios was helpful in identifying LGA, and idiopathic oligohydramnios was a significant predictor of poor perinatal outcome.

3.Evaluation of clinically applied visual evoked potential (VEP) in ophthalmological and neurological diseases.
Korean Journal of Ophthalmology 1987;1(1):26-30
VEPs weie recorded in 222 cases of different disease groups and in 42cases of the control group using a Nicolet CA 1000 system. The latency time of N1, P1, N2, and P2 from the prominent surface peak and the P1-N2 amplitude at full field pattern reversal VEP. The score of each disease group was compared with those of the control group. The results are as follows; 1. Functional amblyopia, optic neuritis, and optic nerve atrophy patients presented a significant derrease in amplitude in comparison to normal subiects. 2. In the P1 implicit time, optic neuritis, and optic nerve atrophy patients presented a marked delay. Functional amblyopia patients presented a moderate delay while other disease group patients presented normal to mild delays. 3. More than half of the optic neuritis and optic nerve atrophy patients presented a detraction of the P1 wave form.

Korean Journal of Dermatology 1992;30(4):551-557
Hypertrichosis is an overgrowth of hair not localized to the anclro en dependent areas of the skin. It may be congenital or acquired, localized or generalized localized hypertrichosis may occur in some types of pigrnentary nevi, chronic inflammation or irration, topical application of androgen or steroid, & localized myxedema. We report herein three i:ases of unilateral localized hypertrichosis with or without a brownish hyperpigmented patch developing on the face of a male adol scent. There are no known cau:,es of localized hyper trichosis. Histopath logically, it is characterized by increased melaniii pigment in the basal layer of the epidermis & enlarged hair follicles containing coarser liair. Electron microscopic findings show normal size & number of melanosomes in both mela nocytes & keratinocytes. Our cases differ frorn BecLers nevus in both clinical & histopatholcgic characteristics.

Cutis verticis gyrata(CVG) is an unusual conformation of the scap characterized by the presence of folds and furrows due to thickening of the corium and the subutlneous tissue. In sccordance with etiological factor and associated findings, CVG occurs as primarary disease or secondary to a variety of local or systemic diseases. Cerebriform intradermal nevus(CIN) is a cause of CVG that can usually be recognized clinically and for a certainty histologically. We report two cases of CVG due to CIN. Not only did they have CVG due to CIN on the scalp, but also they had associated congenital nevi on the face and whole boispy, respectively.

8.Changes in the Rate of Flow by Quantitative Analysis of Aqueous Humor after Argon Laser Trabeculoplasty in Rabbits.
Journal of the Korean Ophthalmological Society 1986;27(3):351-361
The measurement of the rate of flow of aqueous humor is very important for the investigation of the physiology in the aqueous humor and maintenance of the intraocular pressure. Argon laser trabeculoplasty apears to be effective in lowering intraocular pressure with much advantage over standard filtering operation but despite the, widespread use of argon laser traheculoplasty in medically unresponsive open angle glaucoma, the exact mechanism by which it reduce outflow resistance remains unknown and so the study of the mechanism and the most suitable site are required due to the associated complication and less effectiveness during long term evaluation. We studied the changes in the rate of constant, the rate of flow for 2 weeks, intraocular pressure and C-value for 4 weeks, histopathologic changes for 36 days after argon laser trabeculoplasty in 25 rabbits(21 white and 4 black). The normal control group of 20 rabbits was also measured for intraocular pressure and C-value and the rate of constant, the rate of flow. In trabeculoplasty, we used the Britt 900 argon laser photocoagulator(pulse wave) with 50~60 burns at a setting of 700~800 mW, 0.2 seconds duration, 50 micro beam diameter in 360 degrees portion at the site between scleral spur and trabecularmeshwork. The results were as follows; 1) In control group of 20 rabbits. 1. The mean normal intraocular pressure was 18.5 +/- 2.0mmHg. 2. The mean normal C-value was 0.385 +/- 0.072 cumm/min/mmHg. 3. The mean normal rate of constant was 0.0114 +/- 0.0021min(-1). 4. The mean normal rate of flow was 3.980 +/- 0.735 micro l/min. 5. The relationship between rate of flow and C-value was high. 2) In rabbits after argon laser trabeculoplasty. 1. The white and black rabbits showed a statistically significant decrease of intraocular pressure for 4 weeks and elevation of rate of flow for 2 weeks after argon laser trabeculoplasty (p<0.001). 2. The changes of post-laser intraocular pressure was related with changes of C-value after argon laser trabeculoplasty for 4 weeks. 3. The changes of the mean IOP and C-value for post-laser 4 weeks showed no significant differences between white group and black group. 4. In histopathologic changes for 36 days, inflammtory change was not seen for post-laser period, fibrosis and edema in trabecular meshwork was more prominent at the 8th day, edema and fibrosis was regressed as normal at the post-laser 27th day.

Korean Journal of Occupational and Environmental Medicine 2009;21(2):192-200
BACKGROUND: It is well-known that organic solvents can cause various neurologic toxicities, and in particular, it had been reported that Parkinson's syndrome can be caused by organic solvents. CASE REPORT: A 53-year-old man who had worked for 13 years as a shipyard spray and brush painter and manifested with moderate cognitive disorder and was diagnosed with chronic toxic encephalopathy. We can assume he had had considerable exposure to organic solvents considering the estimates of the amounts of organic solvents in his work place. He had no specific medical history. The Minnesota Multiphasic Personality Inventory (MMPI) showed mild cognitive deficit, depression, and anxiety. Single Photon Emission Computed Tomography (SPECT) indicated a slightly decreased flow at the edge of the left temporal brain area, while T2 brain Magnetic Resonance Imaging (MRI) showed no specific signs except for ischemic changes in small vessels in the periventricular and subcortical white matter. He was also diagnosed with both carpal tunnel syndrome and peripheral neuropathy through a neuroconductive study. Several years later, he developed progressive bradykinesia and rigidity and, later, resting tremors in the left hand. He was diagnosed with Parkinson's disease and treated with dopaminergic agents, but there was no effect. After that, he was given deep brain stimulation, both. As a result, his tremor is improved. but, the rigidity remained. Three years later, He has continuously received dopaminergic therapy. but, he complains about tremors in both hands and more greatly reduced cognitive function. CONCLUSION: In this case, we assumed by patient's work history and exposure estimates that he was exposed to excessive levels of organic solvents. The clinical symptoms of this patient were very similar to those from Parkinson's disease, but the psychological symptom appeared earlier than the other symptoms and there was no response to dopaminergic agents. We conclude that this case is likely Parkinson's syndrome caused by organic solvents.
