1.Analysis of Expression of p63 in Cervical Neoplasia Comparing with Other Immunohistochemical Markers .

BACKGROUND: The reproducibility in grading a cervical intraepithelial neoplasia (CIN) are not perfect. The aim of this study was to assess the value of the immunohistochemical expression of p63 and the other biomarkers for grading a CIN (dysplasia and in situ carcinoma), and diagnosing invasive carcinomas. METHODS: Sixty six cervical specimens were immunostained with the monoclonal antibodies against p63, Ki-67, p27Kip1, and p53 to determine the localization. RESULTS: The p63 positive cells are well linked with squamous cell maturation and the degree of dysplasia. In mild dysplasia, the p63 positive cells were localized to the basal and parabasal cells, which gradually extended into the middle and upper layers in moderate and severe dysplasia. p63 expression was strong in immature squamous epithelium and invasive squamous cells, but was constantly absent in an adenocarcinoma. The Ki-67 positive cells were scattered from the parabasal cells to the superficial cells in accordance with the degree of dysplasia. p27Kip1 expression was noted in the intermediate cells in the normal cervix. In CIN, the p27Kip1 positive nuclei tended to extend to the basal cells, but it showed no diagnostic consistency in an invasive carcinoma. p53 expression was also variable. CONCLUSION: p63 is a useful diagnostic adjunct for grading CIN as well as for detecting microinvasion and squamous differentiation in invasive carcinoma. However, immunohistochemical expressions for the p27Kip1 and p53 have no correlation with the grade of CIN and squamous cell carcinoma.

2.Evaluation of the Phototoxic Potential of Some Quinolone Antibiotics.

BACKGROUND: The photsensitizing effect of quinolones has been recognized since their introdulation as an antibacterial agents. Recently several new second eneration antibacterial agents of this pharmacological class have become available for therapy, and are gaining increasing impotance. OBJECTIVE: To reveal the phototoxic potentials of some new quinolones by photohemolysis test, estimation of fluorescenc spectra, and Candida albicans test. METHODS: Nalidixic acid and four second-generation quinolones(ciprofloxacin, enoxacini, norfloxacin, and ofloxacitid were examined by fluorescence spertra which measured t.he phototoxc potentials by photochemial instability, photohemolsis test for the phototoxic properties against cell membranes and Candida tlbicans test for phototoxic properties against DNA. RESULTS: All drugs showed a fluorescence spectra within 360 nm to 450 nm, and in the photohemolysis test, all studied drug except ofloxacin got above 5% hemolytic value, and all drugs showed clear zone. in Candida albicans test after 48hours. CONCLUSION: These results suggested that all tested drugs were photochemically unstable. According to the mechanisris of cellular phototoxicity, ciprofloxacin, enoxacin, and norfloxacin was phtototoxic to nucleus and cell membrane, whereas ofloxacin was phototoxic to nucleus only.

4.A Clinical Observation on Ultrasonography of Renal Diseases.

Recent improvements in gray scale ultrasonic imaging and the development of real time ultrasound made it useful in directing the diagnostic examination and in determining which patients should undergo further, more invasive diagnostic procedures. Ultrasound scanning is cheaper and quicker than computed tomography and angiography, is not invasive and causes little discomfort to the patient. No intravenous contrast medium is used so that reactions to iodine and the difficulty in obtaining venous puncture, frequently a problem in children, are circumvented. There is no ionizing radiation or problem with breath holding. So we made a brief review of the clinical experience and value of ultrasound of renal diseases.

5.Pregnane X Receptor agonist Increases the Expression Levels of the Plasma Membrane Monoamine Transporter.

We evaluated the effect of the pregnane X receptor agonist, pregnenolone 16 alpha-carbonitrile (PCN) on the expression levels of plasma monoamine transporter (PMAT) in the intestine. Male C57/BL6 mice were divided into two 2 groups: mice in the PCN group (n=3) were administered PCN once a day for 4 days, while those in the control group (n=3) received the same volume of vehicle once a day for 4 days. After the mice were killed 24 h after administration of the last dose of PCN or vehicle, and the expression levels of PMAT in the intestine tissues were isolated and measured the expression level of PMAT using immunohistochemical and western blotting analyses. The expression level of PMAT expression levels in the small intestine increased after PCN treatment. These results suggest that the induction of PMAT may play a clinically significant role by increasing intestinal absorption of PMAT substrates such as metformin.

6.The Expression of c-erbB-2, EGFR, p53 and Ki-67 in Ovarian Borderline Tumors and Carcinomas of the Ovary.

BACKGROUND: An ovarian surface epithelial tumor is a heterogenous disease, and various biological and molecular factors are important for its development and progression. Several findings support EGFR or c-erbB-2 as adverse prognostic indicators for an ovarian carcinoma. METHODS: We reviewed the histological and clinical findings of 52 carcinomas (17 endometrioid, 16 serous, 13 mucinous and 6 clear cell tumors), and 26 borderline (10 serous and 16 mucinous) tumors. Expression of c-erbB-2, EGFR, p53, and Ki-67 was evaluated on paraffinembedded tissue from a primary ovarian tumor by immunohistochemical methods. RESULTS: Expression of c-erbB-2 was found in 7.6% of tumors and expression of EGFR was found in 9.6% of tumors by immunohistochemical analysis. No significance was found between cerbB- 2 and EGFR expression as indicators of a poor prognosis. The expression of p53 and Ki-67 (>50%) correlated with the grade and type of tumor in the ovarian cancers. p53 and Ki- 67 overexpression (>50%) was absent in the borderline ovarian tumors, whereas ovarian carcinomas showed expression of both p53 and Ki-67. CONCLUSION: Expression of c-erbB- 2, EGFR, p53, and Ki-67 as determined by immunohistochemical analysis did not correlate with prognostic significance. However, p53 and Ki-67 expression may be used as markers to predict aggressive behavior, and to differentiate between malignant and borderline epithelial ovarian tumors. Further large-scale studies are required to clarify the significance of c-erbB-2 and EGFR expression in ovarian tumors.

7.A Case of Occult Adenocarcinoma of Cystic Duct Associated with GB Empyema.

A case of adenocarcinoma of cystic duct associated with GB empyema is presented. A 72 year old male was admitted to Kwangju Christian hospital because of abdominal pain in right upper quadrant for 4 days. He underwent explolaparotomy under the impression of GB empyema or GB cancer. For seeking causative factor of GB empyema, serial sections of cystic duct and mapping were performed. We can find adenocarcinoma of cystic duct in the specimen removed at cholecystectomy. The criteria for disgnosis of cystie duct carcinoma which was outlined by Farrar. These criteria are; 1) The growth must be restricted to the cystic duct, 2) There is no neoplastic process in the gall bladder, hepatic and common bile duct. 3) Histopathological examination must be comfirm carcinoma. Here we report a case of adenocarcinoma of cystic duct associated with GB empyema with brief review of Literature.

8.A Case of Hemolytic Disease of the Newborm due to Anti - Dib Antibody.

The Diego blood group system consists of two pairs of antigens, Dia and Dib The incidence of Dia is low among pure Caucasian, Blacks, Polynesian and Eskimo, however, the rnongolians and American indians have both Dia and Dib. We report a case of two days old male who was admitted on first day of life for jaundice and subsquently exchange transfusion was performed on second day of life for bilirubin of 20 mg/dl. The blood groups of patient and his mother were both Rh D positive 0 type. Direct and indirect Coombstest were strong positive in the patient and indirect Coombstest was positive in his mother. We found anti-Dib antibody in his rnother's serum. The phenotype of Diego blood group system of the patient and his mother were Di (a+b+) and Di (a+b- ), respectively and hemolytic anemia in this case was due to anti-Dib antibody.

10.Changes in Urinary Biochemistry in Machinists Exposed to a Hot Environment.

PURPOSES: To demonstrate changes in urinary biochemistry that lead to a supersaturation of some salt, which makes a higher prevalence of urinary stone in workers chronically exposed to a hot environment and massive sweating. MATERIALS AND METHODS: The study was performed at a metal plant and exposure to heat stress was estimated by the Wet Bulb Globe Temperature climatic index. Stone risk profile from our laboratory included volume, specific gravity, creatinine, sodium, potassium, calcium, magnesium, chloride, uric acid, phosphorus and pH. On a randomly selected sample of 19 workers exposed and 19 workers not exposed to heat stress without any evidence of stone disease, and family history of nephrolithiasis or predisposing factors such as gout, we performed a stone risk profile by collecting the urine during the 8-hour work shift for 1 consecutive days. The independent student t-test was used to compare each parameter between the different groups. RESULTS: Among a randomly selected urine sample of 19 workers exposed and 19 workers not exposed to heat stress without any evidence of stone disease, significant differences were found in fluid intake (2816.0+/-1981.0 ml. versus 1200.0+/-4147.0), and concentration of creatinine (1481.0+/-691.0 ml./l. versus 1032.0+/-574.0), sodium (201.0+/-55.0 mg./l. versus 147.4+/-58.0), calcium (202.0+/-76.0 mg./l. versus 132.0+/-71.0), uric acid (475.0+/-138.0 mg./l. versus 359.0+/-196.0), phosphorus (638.0+/-283.0 mg./l. versus 328.0+/-196.0), pH(6.05+/-0.61 versus 6.72+/-0.92), specific gravity(1.029+/-0.005 versus 1.014+/-0.006) respectively. CONCLUSIONS: This study confirms the concept that heat stress represents a real lithogenic risk factors. Therefore, we recommend adequate fluid intake during hot occupations.
