1.Wegener's Granulomatosis Involving Lung and Middle Ear: A case report.

A case of Wegener's granulomatosis is described, with special attention focused on the typical histologic findings and involvement of both middle ear and lung. The patient is a 37-year-old man presented with four-month history of cough and sputum. He had a past history of surgery of both ears because of otitis media followed by left facial palsy. Chest radiographs showed variable sized ill defined nodules in both lower lobes with internal airspace consolidation. Histologic preparations of the open lung biopsy specimens demonstrated a diffusely scattered palisading micro and macrogranulomas with central focus of neutrophils and necrotic collagen surrounded by histiocytes, histiocytic giant cells. Fibrinoid necrosis involved blood vessels and lung parenchyma. Chronic inflammation, diffuse granulation tissue formation and irregular fibrosis are also found in the lung parenchyma. The histologic findings of middle ear which was previously biopsied showed scattered palisading ill defined microgranulomas mixed with fibrotic tissue.

3.Porokeratosis Plantaris Palmaris et Disseminata ( PPPD ) Associated with Basal Cell Epithelioma.

Porokeratosis may be due to an abnormal clone of cells, predisposing affected individusls to development of malignant neoplssm over involved area. A 58-year old female is prsented with basal cell epithelioma associated with PPPD, exhibiting the classical histopathologic criteria of the disease. Total excision for basal cell epithelioma and oral administration of etretinate for the treatment of porokeratosis plantaris, palmaris et disseminata and for the prevention of cancer developrnent resulted in good response.

4.Serial Change of the Bone Density in Distraction Osteogenesis in Long Bone Lengthening in Lower Extremity - by the Pixel Value in PACS.

We investigate the serial change of the bone density of the lengthening sites in distraction osteogenesis in long bone lengthening of the lower extremity by measuring the pixel value of the PACS(Picture Archiving Communication System). The purpose of this study was to find the clinical implication of the pixel value in PACS in the distraction osteogenesis. The number of the distraction sites were 22 in tibia and 16 in femur. The average distraction length was 4.5cm ranged between 2.1cm and 7.0cm in femur, 4.1cm ranged hetween 1.9cm and 6.8cm in tibia. When the image were sent to the PACS workstations, they were directly interfaced to the workstation without any processing. The absolute and the relati ve pixel values of cortical bones of the original and the lengthening sites repr sented in workstation of PACS were obtained by average value measuring 3 times by 3 different persons. The average absoiute pixel value of the original cortical bone near distraction site was not significantly changed, maintaining 575+/-6 in femur, and 570+/-7 in tibia. The absolute pixel vaIues in AP and lateral view were not significantly changed until 6 week/cm, but rapidly increased after 7 week/cm hoth in the tibia and the femur. The relative pixel value of the lengthening sites were more than 95% in three of the four cortices at the time of the removal of the external fixators. in conclusion, the pixel value of the PACS can be a rapid, simple and easy method for detection of the change of the bone density in distraction osteogenesis.

5.Genetic Profile Analysis of a Patient with Metachronous Gastric Cancer with a Family History of Gastrointestinal Cancers.

A 67-year-old man underwent two endoscopic submucosal dissection procedures, one for gastric adenoma and one for early gastric cancer. The follow-up endoscopy showed metachronous recurrence at the anterior wall of the lower body, for which he underwent a subtotal gastrectomy. Four first- or second-degree relatives in his family have been diagnosed with gastric or colon cancers. The patient underwent counseling and genetic testing to identify single nucleotide polymorphisms and indel variants for 31 genes by next generation sequencing. Five missense mutations were identified, one each in ATM, BRIP1, and EPCAM and two in BRCA2. These genetic alterations may be candidates for genetic causes of this familial cluster of gastric cancer. This study identified genes that, for the first time, can be potentially associated with an increased risk of familial gastric cancer among the Korean population. These results may be helpful in evaluating other genetic factors related to the etiology of gastric cancer.

8.The Effect of All-trans and 13-cis-retinoic Acid in Medulloblastoma and Glioblastoma Cell Culture.

PURPOSE: It has been reported that retinoic acid, one of the most popular agents for chemoprevention could inhibit the proliferation of many cancer cells including neuroblastoma and glioblastoma. However, there is increasing demand reaccessing its in vitro inhibitory effect on the tumor proliferation because of the poor results from recent clinical trials of retinoic acid in the malignant brain tumor. Retinoicacid ptomoted the diffferentiation and apoptosis of tumor cell so that its effect might be obvious in the pediatric brain tumor. Therefore we are going to confirm the effectiveness of retinoic acid to inhibit the proliferation of the tumor cells; glioblastoma and medulloblastoma in childhood. METHODS: Medulloblastoma cells were derived from the primary culture of the patient's specimen, and glioblastoma cells were cell lines of 373-MG and 87-MG. We estimated growth inhibition rate of each tumor cells using MTT assay in the concentration from 10 M to 10((-5))M of all-trans and 13-cis retinoic acid. RESULTS: 13-cis retinoic acid in the concentration of 10 6M inhibited cell growth rate 10-22% on the 4th day of incubation, 10% on the 7th day, and 0-12% on the 14th day in the concentration from 10((-6))M to 10((-5))M. All-trans retinoic acid inhibited cell growth rate less than 5% in the concentration less than 10((-5))M though the whole incubation period, but 42% on the 4th day, 37% on the 7th day, and 0% on the 14th day in the concentration of 10((-5))M. 13-cis retinoic acid inhibited cell growth rate 30% on the 4th day, 20% on the 7th and 14th day in the concentration between 10((-6))M and 10((-5))M. Alltrans retinoic acid inhibited cell growth rate less than 5% on the 4th and 7th day. Medulloblastoma cells showed growth inhibition more than 25% by all-trans and 13-cis retinoic acid in the concentration less than 10((-6))M. 13-cis retinoic acid showed 25% growth inhibition in the concentration above 10((-6))M, but all-trans retinoic acid showed 40% growht inhibition in the same concentration. CONCLUSION: We could not find the effect of retinoic acid in the glioblastoma cells due to variable responses of the tumor cell growth inhibition in the concentration of maximum tolerable dose. However, there ia a significant inhibitory effect of medulloblastoma cell proliferation both in the 13-cis and all-trans retinoic acid.

In Korea, the metallic implant had to be supplied through import channel with many problems. KAIST succeeded in making AISI 316 LVM stainless steel within the criteria of American Society for Testing and Materials. With the animal experiment at preliminary report I, the biocompatibility in the muscle was examined. For biocompatibility test of new material, test in the bone is essential. In this animal experiment using rat tibia, the biocompatibility of the Kirschner wire of KAIST was compared with that of Zimmer. The result was as follows: 1. Tissue reaction of the Kirschner wire of KAIST was minimal to moderate. 2. Corrosion was negligible, and there was no definite difference between the KAIST and Zimmer Kirschner wires. 3. Authors observed the satisfactory biocompatibility of the KAIST Kirschner wite in bone and muscle, and concluded that the Kirschner wire of KAIST can be applied to human body without serious problems.

10.Analysis of Initial Choice Antibiotics Efficacy in Diabetic Foot Infection.

PURPOSE: Foot infections are common complications in patients with diabetes. The patients are usually immune-compromised; therefore the pathogens could be resistant to narrow spectrum antibiotics. Those drugs, however, are categorized as specially managed antibiotics, and access are difficult without confirming of the pathogens. Our aim was to analyze the common pathogens in diabetic foot infection and figure out the proper antibiotics. MATERIALS AND METHODS: We studied 68 patients treated with diabetic foot infection. The pathogens which caused the infection and their sensitivity to initial antibiotics were analyzed. We also investigated the change of the antibiotics after the confirming of the culture result and average time to get the result. RESULTS: Among the 68 patients, 56 (82%) received cephalosporin and beta-lactam antibiotics. Only 12 (18%) who were confirmed the drug resistant pathogens from previous culture, were treated with broad spectrum antibiotics such as vancomycin and tazoperan. Average culture study time was 6 days. Methicillin-resistant staphylococcus aureus (MRSA) was cultured in 19 patients (28%), Methicillin-resistant coagulase negative staphylococcus (MRCNS) in 11 patietns (17%), pseudomonas in 11 patients (17%). Total 44 (65%) including 3 of other antibiotics resistant pathogen needed broad spectrum antibiotics. Thirty two patients (47%) were resistant to initial antibiotics.irt follow up culture, 2 MRSA and 2 MRCNS were found. The antibiotics resistant pathogens were confirmed in 48 (71%) patients at last. CONCLUSION: Diabetic patients with foot infection need proper antibiotics from initial treatment. The proper broad spectrum antibiotics should assigned to the patients from the first time without the confirming of the culture results.
