1.Expression of Oncogene Product in the Colorectal Carcinoma.

The expression of p62"' and p2l can be seen in many solid tumor, but the pattern and incidence of expression were different according to organ, countries, and examiners, thus it is not definitely defined. Total 67 colorectal carcinoma in paraffin sections are analysed by immunohistochemically for evaluation of the p62and p21- expression according to the age, sex, chief complaints, location, differentiation, modified Dukes stage, using the specific monoclonal antibodies. The results were summarized as follows : The age of patients ranged from 32 years to 82 years. The mean age was 57.6 years. The expression of p 62y' and p21 was not correlated with age. Male was 29 cases(43.3%) and female was 38 cases(56.7%). The male to female ratio was 1:1.31. The and 95.0% in moderately differentiated adenocarcinoma, 90.0% and 90.0% in poorly differentiated <,;, adenocarcmoma, 100.0% and 100.0% in mucinous carcinoma. The positive rate of p62!and and p2l' was 94.1% and 88.2% in Dukes stage B,, 96.0% and 96.0% in Dukes stage B,, 100.0% and 100.0% in Dukes stage C,, 100.0% and 88.9% in Dukes stage C, and 100.0% and 100. o% in Dukes stage D. The expression of p62 y' in metastatic colorectal carcinoma showed diffuse and strongly positive reaction than primary colorectal carcinoma. The expression of p21 in primary colorectal carcinoma showed diffuse and strongly positive reaction than metastatic colorectal carcinoma.

2.A Case Report of Gastric Pseudolymphoma.

A 46-year-old female was admitted to Soon Chun Hyang University hospital with a complaint of intermittent epigastric pain. X-ray and endoscopic pictures showed diffase irregular shallow depression which is similar to the macroscopic pattern of the type IIc early gastric cancer. But endoseopic biopsy showed no malignant cell infiltration. Hndoaeopic biopsy plays an important role in excluding the possibility of carcinoma. But operation shoud be considered inspite of biopsy result because malignant lymphoma ean not be completely ruled out. Histopathologic diagnosis was pseudolymphoma of stomach.

3.The Successful Dissolution of 2 Cases of Fat Bezoars by Pancreatic Lipase.

Gastric bezoars are large masses or concretions, composed with plant fiber, hair and miscellaneous foreign bodies, found in the stomach and occur usually as sequele of gastric surgery. We experienced 2 cases of fat bezoars which developed after drinking of melted beef fst. They had postprandial epigastric fullness and pain, those were relieved on supine and left recumbent position. Upper gastrointestinal series showed a large movable mass in the stomach in hoth cases. On gastrofibercopic examination, a large egg sized white bezoar was cordirmed in both cases and a gastrie ulcer at antrum associated in one case. In attempt to removal of the bezoars, we tried to break them, but failed because of its character. The patients were treated with pancreatic lipase containg digestives, Pancreon-F and Azintal. From one day after this treatment, symptoms were completely relieved. Five days later, we confirmed disappearance of bezoars by gastrofiberscopy. and upper gastrointestinal barium

4.Skin Lesions Other than Pressure Ulcers of the Patients in a Rehabilitation Unit.

OBJECTIVE: The purpose of this study is to investigate the incidence and types of skin lesions occurring in the patients with a spinal cord injury(SCI), traumatic brain injury(TBI), cerebrovascular accident(CVA), and an amputation. METHOD: Five hundred and eighty-nine charts from 1991 to 1997 were retrospectively reviewed. During the hospitalization, the skin lesions were examined by the dermatologists upon consultations. The patients were divided by their diagnosis of the SCI, TBI, CVA, and amputation. The incidence of the skin lesions were analysed. RESULTS: The number of patients was as follows; SCI 228, TBI 181, CVA 143, and amputation 37. The skin lesions occurred in 72 cases(12.9%) of which 30 cases(13.2%) occured in SCI, 21 cases(11.6%) in TBI, 15 cases(9.8%) in CVA and 7 cases(18.9%) in amputation. These include dermatophytosis 23 cases(23.9%), seborrheic dermatitis 21 cases(21.8%), pilosebaceous disorder 17 cases(17.7%) such as folliculitis, acne, and acneiform eruption, eczema 11 cases(11.4%), drug eruption 9 cases(9.4%), candidiasis 6 cases(6.3%), and others 9 cases(9.4%) such as steatocytoma multiplex, epidermal cyst, intertrigo, alopecia areata, and etc. The incidence of ANS related skin lesion such as seborrheic dermatitis and pilosebaceous disorder in SCI was significantly higher than in TBI and stoke, which the defect is in the brain(p<0.05). However, the incidence in other lesions such as dermatophytosis, eczema, and candidiasis was not significantly different between these two groups(p>0.05). The number of cases of ANS related skin lesions was 21(52.5%) and 3(42.9%) in SCI patients whose level of injury was from C1 to T6(in 40) and at or below T7(in 7) respectively. In contrast, the number of cases of other skin lesions such as dermatophytosis, eczema, and candidiasis was 15(37.5%) of C1 to T6 level and 4(57.1%) of at or below T7 level in the same SCI patients groups. CONCLUSION: This study illustrates that the incidence of skin lesion in rehabilitation unit is 72 cases of 589 patients(12.9%). The incidence of ANS related skin lesion is higher in the patients with spinal cord injury than brain lesion. And the level of injury is higher in SCI, the more skin lesions occur.

5.Vancomycin-resistant Enterococcus avium Isolated from the Wound of a Patient with Diabetes Mellitus.

Enterococcus avium, very commonly found in birds, is rarely reported as a human pathogen. Although several studies have focused on cases of infection with E. avium, little is known about the clinical features of E. avium infection and the antimicrobial susceptibility pattern of E. avium. We isolated vanA-producing vancomycin-resistant E. avium from specimen obtained from the wound of a patient with diabetes mellitus. Vancomycin-resistant E. avium is rarely observed among the vancomycin-resistant enterococci colonizing the intestine, and there are very few reports of vancomycin-resistant E. avium isolated from clinical specimens. Here we confirmed that the clinical isolate was E. avium on the basis of phenotypic characterization and the results of 16S rRNA sequence analysis.

6.Evoked EMG Monitoring During Transpedicular Fixation of Lumbosacral Spine.

OBJECTIVE: The purpose of this study is to evaluate the searching stimulus intensity at each pedicle and to identify the most vulnerable roots in transpedicular screw fixation of lumbosacral spine. METHOD: Thirty-two patients with unstable lumbosacral vertebra were treated with intrapedicular screw fixation. Small holes were made by an air drill on the pedicle from L2 to S1 for screw fixation. Constant current stimulation pulses(0.2 msec duration) were delivered through a ball-tipped nasopharyngeal probe used to palpate the walls of each pedicle, and observation was made of electromyogram(EMG) evoked lower extremity muscles. The probes were placed in each pedicular wall manually, and evaluated for searching stimulus intensity, the current necessary to evoked EMG. RESULTS: The cases that the searching stimulus intensity was above 5 mA were 99 cases(97%), above 10 mA were 61 cases(59.8%) and below 5 mA were 3 cases(2.9%). The vastus medialis muscle is most sensitive in L2(100%) and L3(78.9%), tibialis anterior is in L4(81.8%), Peroneus longus is in L5(50.5%) and gastrocnemius is in S1(87.5%). CONCLUSION: We can regard the searching stimulus intensity as 5 mA and the root located at Infero-medial side of pedicle is most vulnerable in transpedicular screw fixation. Furthermore, stimulus-evoked EMG monitoring during transpedicular spine instrumentation is helpful to avoid neural tissue injury.

7.A study on the prevalence of dental anomalies in Korean dental-patients.

OBJECTIVE: The purpose of this study was to investigate the prevalence of dental anomalies in outpatient clinics. METHODS: The subjects of this study were 3,133 patients who visited the clinic between January 2009 and June 2011. The dental records and panoramic films of the patients and detection of supernumerary, missing, and impacted teeth, transposition, and peg lateralis were reviewed. The results were analyzed according to gender and types and locations of dental anomalies. RESULTS: Among the patients, 362 had dental anomalies, with a prevalence rate of 11.55%. Congenital missing teeth (5.71%) ranked first in the categories, and impacted teeth (3.09%) ranked second. The percentage of patients having supernumerary teeth, peg lateralis, and dislocated teeth were 1.79%, 1.66%, and 0.45%, respectively. CONCLUSIONS: Congenital missing teeth and impacted teeth are, respectively, the first and second most common dental anomalies in Korean dental patients.

8.Ultrasound-guided auriculotemporal nerve block for post-herpetic auriculotemporal neuropathy: A case report

Auriculotemporal neuropathy is a rare aetiology of orofacial pain. A 66-year old female patientpresented with severe, throbbing pain around the right ear after herpes zoster. The pain was consistentwith post-herpetic neuropathy and was not relieved by oral medications. To alleviate neuropathicpain, ultrasound-guided auriculotemporal nerve block was done. The pain was relieved completelyand the patient remained symptom-free until the last follow up at eight months after treatment. Withunderstanding of the anatomy and related symptoms of auriculotemporal nerve, auriculotemporalneuropathy can be controlled by ultrasound-guided auriculotemporal nerve block.
9.A study of mammographic appearance of the breast diseases

The history of the radiological examination of the breast has been long, and recently the methods forexamining the breast are variable: mammography, thermography, xeroradiography, ultrasonography, water-pathsonography, transillumination, mammary C.T., M.R. Imaging, infra-red scanning, nuclear medicine, andgalactography. Most of these methods were not proven or have not been satisfied on their clinical testing butconventional mammography has been the gold standard for diagnosis of the breast cancer and no other method hasreached its effectiveness. The authors performed mammography in 488 patiens with breast abnormalities who hadvisited to Yeung Nam University hospital form May, 1983 to Dec. 1984. And we analyzed the mammographic findingsand diagnostic accuracy in pathologically confirmed 53 cases by operations or biopsies. The results are asfollows: 1. Among 16 cases of breast malignancies, (1) the most prevalent age group is 6th decade(37.5%), followedby 5th decade(31.3%), (2) the most common pathologic type is ductal carcinoma(93.75%), (3) the masses are largerthan 2cm, in 8 cases, smaller than 2cm. in 5 cases, and no visible mass is observed in 3 cases. 2. Among 37 casesof benign breast diseases, (1) the most prevalent age group is 4th & 5th decades(each11 cases), and followed by3rd decade(8 cases), (2) the most common disease is benign tumor (17 casees), followed by fibrocystic dysplasia(13cases), and inflammating disease(7 cases). (3) radiologically visible masses are observed in 16 cases and themasses are smaller than 2cm. in 11 cases. 3. the diagnostic accuracy of mammography compared with that ofpathology, (1) in malignancy, the accuracy rate is 75%(12/16 cases), and 87%(32/37 cases) in benign diseases. (2)avearage accuracy rate is 81%(44/53 cases).

10.Partial HPRT Deficiency Due to a Missense Mutation in the HPRT Gene.

An 8-month-old male infant presented with persistent, gross, orange-colored crystals in his urine. His physical and neurological development was normal. Laboratory study showed hyperuricemia, hyperuricosuria and urate crystaluria. He was determined to have partial hypoxanthine-guanine phosphoribosyl transferase(HPRT) deficiency. The molecular genetic analysis revealed a missense mutation in the patient's HPRT gene. By sequencing the patient's cDNA, we identified an A-to-G transition at nucleotide 239, resulting in the replacement of Aspartate with Glycine at amino acid 80 in the HPRT. To our knowledge, this mutation has not previously been reported. Our patient is now being placed on allopurinol therapy, and has had no problem since. Partial HPRT deficiency has been known to cause recurrent acute renal failure without the phenotypic features of Lesch-Nyhan syndrome. Therefore, we think that early diagnosis and treatment are very crucial in preventing acute renal failure.
