1.A Study on the Relationship between CEA Immunohistochemical Findings and Expression Rate of PCNA and Clinicopathologic Factors of Colorectal Cancers.

Immunohistochemical study was performed for CEA staining patterns and PCNA indices. And the relationship between immunohistochemical findings and well-known clinical prognostic factors on the purpose of the clinical usefulness was evaluated. In forty seven cases of surgically removed colorectal carcinomas, the results were as follows; CEA staining patterns were apical (17 cases) and cytoplasmic (30 cases) type. Carcinomas with cyto plasmic pattern for CEA revealed more advanced Dukes' stage and more undifferentiated type and higher incidence of lymph node metastasis and were correlated with increased serum CEA levels. But PCNA indices showed no correlation with the Dukes' stage, histologic grade and CEA staining patterns. The cytoplasmic pattern of CEA immunohistochemistry may be a useful marker suggesting more aggressive biologic behavior of the colorectal carcinomas.

3.A Case of Steroid-induced Atrophy of External Genitalia.

Skin atrophy is one of the most common side effects of topical steroid use and results from decreased skin proalphaI collagen mRNA production due to specific alterations in collagen gene transcription or from reduced collagen mRNA stability. This report describes a case of a 8 year-old-girl who visited our hospital with a complaint of ambiguous genitalia. She was observed to have atrophic labia majora and atrophic striae resembling scrotal rugae. However, her serum levels of FSH, LH, 17-KS, 17-OHCS, 5-DHT, DHT and DHEA-S were all in normal range and her karyotype was 46,XX. Abdominal and pelvic ultrasonogram showed a normal uterus and ovaries. Careful history taking revealed frequent use of steroid ointment due to vulvar pruritus for 2 years and she was finally diagnosed to have atrophy of external genitalia induced by long-term use of topical steroid.

4.Awareness and Knowledge about Hepatitis B among North Korean Immigrants.

PURPOSE: The purpose of this study was to identify awareness and knowledge about hepatitis B virus (HBV) infection among North Korean immigrants who had resided in South Korea. METHODS: This was a cross-sectional, community based study. Data were collected from a convenience sample of 198 North Korean immigrants aged 18 years old through 65 years old among those who had attended community centers, local churches, and gathering places in four cities. The questionnaire included 15 items about participants' knowledge on HBV infection and its consequences, mode of transmission, and preventive measures. RESULTS: The mean score of HBV knowledge was 6.3±3.8 (range 0~15). The items that over 60% of the respondents checked correct answer regarding knowledge about HBV were 'There is blood screening for hepatitis B infection', 'Hepatitis B can be transmitted through sharing needles with infected person'. Multivariate analysis showed that received education for the prevention of HBV infection and disease morbidity were significantly associated with the knowledge of HBV infection. Overall, approximately 11% of total variability in the knowledge of HBV infection could be explained by the 2 variables in this model (R²=.109, p<.001). CONCLUSION: The results showed that North Korean immigrants need more education on HBV infection and prevention to improve their knowledge about HBV.

5.Use of the Pill Questionnaire to detect cognitive deficits and assess their impact on daily life in patients with Parkinson’s disease

The Pill Questionnaire (PillQ) has been proposed as a simple way to evaluate cognitive deficits and their
impact on the daily lives of those with Parkinson’s disease (PD) by asking patients or caregivers about
whether patients can independently manage their pills. We used the PillQ to investigate the association
of ability to manage medication with cognition and activities of daily living (ADLs) in patients with
PD. Patients were divided into two groups based on PillQ scores. The no-impact group was able to take
their antiparkinsonian medication independently, and the impact group exhibited problems describing
their treatment or taking their drugs independently. A total of 208 participants (93 men) were included.
111 patients (53.4%) were included in the no-impact group, and 97 (46.6%) were included in the
impact group. The impact group showed significantly lower cognitive functioning, difficulties with
the performance of ADLs, and severe motor dysfunction. PillQ scores were significantly correlated
with Mini-Mental State Examination and the Montreal Cognitive Assessment, and Clinical Dementia
Rating scores. Management of medication by PD patients is associated with cognitive function, and
the PillQ is an easy and useful test for detecting cognitive impairment and its impact on daily life.
6.Alfa-Synuclein polymorphism and Parkinson’s disease in a tau homogeneous population

Background & Objective: The MAPT H1 haplotype and SNCA single nucleotide polymorphism (SNP)
rs356219 have been reported to have a synergistic effect on the risk of Parkinson’s disease (PD).
Because the H1/H1 genotype has been reported to predominate in Korean population, we investigated
the polymorphism of rs356219 in 878 PD patients and 559 controls. Methods: The SNCA SNP rs356219
was analyzed in 878 PD patients and in 559 healthy Korean subjects. Results: The G allele of SNCA
SNP rs356219 was found to contribute to PD susceptibility with odds ratios (ORs) similar to those
reported previously. However, the ORs were not as large as that of the SNCA rs356219 plus MAPT
H1/H1 combination reported in the literature, which cast doubt on the existence of a synergistic effect
between the two genotypes in our population.
Conclusions: This study supports that the G allele of the SNCA SNP rs356219 contributes to PD
susceptibility as reported previously, but it does not support the presence of a synergistic interaction
between SNCA and MAPT.
7.A Case of Subcutaneous Sweet's Syndrome in MYelodysplastic Syndrome Showing Nuelear Segmentation Anomalies of Neutrophils.

We encountered a case of Sweet's syndrome associated with myelodysplastic syndrome in a 60-year-old woman. The clinical presentation strongly suggested Sweet's syndrome. A skin biopsy specimen taken from her skin lesion on the leg showed an infiltration of numerous neutrophils in the subcutis instead of the dermis which is the main area of neutrophilic infiltration in Sweet's syndrome. Among infiltrating cells, many neutrophils(8%) showed nuclear segmentation anomalies, that is, hyposegmentation(pseudo-Pelger-Huet anomaly) or hypersegmentation. Peripheral blood neutrophils had similar anomalies. This subcutaneous variant of Sweet's syndrome, Sweet's panniculitis, is rare and nuclear segmentation anomalies of neutrophils are probably a good clue to underlying myelodysplastic syndrome.

8.Study of Acid-Base in Arterial and Central Venous Blood during.

BACKGROUND: Although pulmonary artery catheters are useful to monitor hydration, these devices may be associated with severe morbidity and are not routinely used in kidney transplantation. A central venous pressure (CVP) catheter is preferred rather than a pulmonary artery catheter. Noninvasive continuous blood pressure monitors may substitute for intraarterial catheters, thereby preserving the radial artery in kidney transplantation patients should it be needed later to create an arteriovenous fistula. If there is a relationship between central venous and radial arterial blood for acid-base (pH, BE, HCO3(-)), we can use the blood sample from a CVP catheter instead of arterial blood from aradial artery catheter for testing acid-base and it can help patients. METHODS: A central venous catheter and radial artery catheter was inserted in 67 patients while undergoing kidney transplantation. To assess arteriovenous differences in acid-base status at operation start, before reperfusion of the transplanted kidney, after reperfusion, we measured the pH, BE and HCO3(-) simultaneously from the arterial and central venous circulation. RESULTS: Aacid-base using arterial and central venous samples at operation start, before reperfusion and after reperfusionb was evaluated. We found the relationship as follows: pH between arterial (pHa) and central venous blood (pHcv) in each: linear regression equation; pHcv = 0.668 + (0.906 X pHa), pHcv = 0.225 + (0.965 X pHa), pHcv = 0.646 + (0.908 X pHa), determination coefficient; 0.908, 0.926, 0.888, P values < 0.001 in each period. Base excess (BE) between BEa and BEcv in each period: linear regression equation; BEcv = 0.483 + (0.952 X BEa), BEcv = 0.032 + (0.939 X BEa), BEcv = 0.008 + (0.954 X BEa), determination coefficient; 0.844, 0.954, 0.962 P values < 0.001 in each period. HCO3(-) concentration between HCO3(-)a and HCO3(-)cv in each period: linear regression equation; HCO3(-)cv = 2.434 + (0.937 X HCO3(-)a), HCO3(-)cv = 2.093 + (0.942 X HCO3(-)a), HCO3(-)cv = 1.755 + (0.954 X HCO3 a), determination coefficient; 0.950, 0.925, 0.932 P values < 0.001 in each period. CONCLUSIONS: The acid base status of arterial blood is similar to that of central venous blood. Central venous blood gas values (pH, BE, HCO3(-)) may be an acceptable alternative to arterial blood gas values in kidney transplantation patients.

9.Verbal Auditory Agnosia Developed after Unilateral Temporal Lobe Infarction

Stroke results in sudden loss of function related to its damaged portion. When this occurs in temporal lobe, the function of hearing and listening may be affected, although receptive language processing is affected while hearing perception is relatively spared. This is called as “central deafness.” It has been known that hearing ability is seldom impaired in the case of temporal lobe stroke except in the case of bilateral lesions. However, we experienced a 72-year-old, right-handed woman who presented with both sudden hearing difficulty due to unilateral temporal lobe infarctions after suddenly not being able to respond to any verbal questions except environmental sounds. Pure-tone audiometry showed both flat sensorineural hearing loss of mild degree in both ears but she could not understand any verbal sound. Brain MRI demonstrated extensive ischemic infarcts in the left temporal lobes involving primary auditory cortex. She was diagnosed as verbal auditory agnosia and has received speech-language therapy.
10.Fracture strength of zirconia monolithic crowns and metal-ceramic crowns after cyclic loading and thermocycling.

PURPOSE: The purpose of this study was to compare the fracture strength of the zirconia monolithic all-ceramic crowns according to the thickness (0.5mm, 0.8mm, 1.1mm) and metal-ceramic crowns (1.0mm, 1.5mm). MATERIAL AND METHOD: Twelve crowns for each of 3 zirconia crown groups were fabricated using CAD/CAM system (Kavo, Germany) and twelve crowns for each of 2 metal-ceramic crown groups were made by the conventional method. All crowns were luted to the metal dies using resin cement. Half of the specimens were exposed to thermocycling (5-55degrees C, 1 Hz) and cyclic loading (300,000 cycles, 50N). Subsequently, all crowns were mounted on the testing jig in a universal testing machine. The load was directed at the center of crown with perpendicular to the long axis of each specimen until catastrophic failure occurred. Analysis of variance and Tukey multiple comparison test (P<.05) were used for statistical analysis of all groups, and paired t-test (P<.05) was followed for statistical comparison between each groups'fracture load before and after cyclic loading. RESULTS: 1. The fracture strength of the zirconia monolithic crowns and the metal-ceramic crown increased as thickness increased (P<.05). 2. The cyclic loading and thermocycling significantly decreased the fracture strength of the zirconia monolithic crowns (P<.05). 3. The standard deviation of fracture strength of the zirconia monolithic crowns was very low. CONCLUSION: The fracture strength of the zirconia monolithic crowns for the posterior area tends to be higher with thickness increased and 0.8mm or over in thickness is recommended to have similar or over the fracture strength of metal-ceramic crowns.
