1.Association of HLA Class II and Non-HLA Gene Polymorphisms with Disease Susceptibility in Korean Children with Type 1 Diabetes Mellitus.

Purpose : The main genetic contribution to type 1 diabetes susceptibility is the human leukocyte antigen (HLA) class II gene. Several non-HLA chromosomal regions are also known to be involved. We studied the association of HLA class II and non-HLA candidate genes, which are cytotoxic T lymphocyte antigen-4 (CTLA4), tumor necrosis factor (TNF), lymphotoxin-alpha(LT-alpha, and vitamin D receptor (VDR) gene, polymorphisms with disease susceptibility in Korean children with type 1 diabetes.Methods : Fifty Korean children with type 1 diabetes (29 girls and 21 boys) and 166 healthy Koreans were investigated in this study. HLA class II alleles were determined by PCR-SSP (sequence-specific primer) and PCR-SSOP (sequence specific oliogonucleotide probe) method. CTLA4 exon 1 polymor phism was analyzed by PCR-SSCP (single strand conformation polymorphism), and TNF promotor and LT-alphagene polymorphism by PCR-RFLP (restriction fragment length polymorphism), respectively. VDR gene polymorphisms were analyzed by PCR-RFLP using restriction enzyme FokI, ApaI, TaqI, and BsmI. Results : The frequencies of HLA-DRB1*04, DRB1*09, and DQB1*04 were significantly increased and those of HLA-DRB1*14, DRB1*15, DQB1*05 and DQB1*06 were significantly decreased in the patients with type 1 diabetes compared with the control subjects. No significant differences in the distribution of CTLA4 exon 1, TNF promotor, LT-alpha and VDR gene polymorphisms were observed between the patients with type 1 diabetes and the control subjects. Conclusion : These data suggest that HLA-DRB1*04, DRB1*09, and DQB1*04 are susceptible genes for type 1 diabetes, whereas HLA-DRB1*14, DRB1*15, DQB1*05, and DQB1*06 are protective genes in Korean children. CTLA4 exon 1, TNF promotor, LT-alpha and VDR gene polymorphisms are not associated with susceptibility to type 1 diabetes in Korean children.

5.The Evaluation of Anterior Segment Circulation after Strabismus Surgery by using Indocyanine Green(ICG) Iris Angiography.

Anterior segment ischemia is a rare, but serious complication of strabismus surgery, Iris microvascular circulation in eyes with blue irides has been investigated by fluorescein iris angiorgaphy, which failed to demonstrate the iris vascular pattern in eyes with heavily pigmented irides like Oriental brown iris. Anterior segment circulation in eyes of Korean after strabismus surgery was investigated by iris angiography using indocyanine green(ICG), which highly penetrates through the pigmented tissue or hemorrhage ICG iris angiography using confocal scanning laser ophthalmolscope was performed before and after strabismus surgery in 20 eyes of 20 Koreans. Tenotomies of medial and lateral recti produced no appreciable circulatory disturbance in the iris in any patients, but there was mild delayed filling in 2 eyes of 6 eyes undergoing tenotomy of one or both vertical recti. When tenotomies of a horizontal and a vertical rectus were combined, the filling defect occurred in the region of radial vessels was observed in eyes with tenotomies of medial rectus and lateral halves of superior and inferior recti. Our study suggests that ICG iris angiography is a useful means for the evaluation of anterior segment circulation I darkly pigmented iris like Oriental brown iris. And there was the remarkable change in iris microvascular circulation only after tenotomy of multiple muscles including vertical rectus muscles.

6.A Case of Orbital Malignant Lymphoma associated with Acquired Immunodeficiency Syndrome (AIDS).

PURPOSE: To report a rare case of orbital malignant lymphoma that occurred in a patient with acquired immunodeficiency syndrome. CASE SUMMARY: A 39-year-old male with acquired immunodeficiency syndrome complained of proptosis and loss of visual acuity. The best corrected visual acuity of his right eye was no light perception, and the BCVA of his left eye was 1.0. Ocular movement was limited, and esotropia of 15 prism diopters was detected. Exophthalmometry measurements showed a significant difference between both eyes (right eye was 18 mm, left eye was 15 mm). Computed tomography scans of the patient's orbit with contrast showed an enhancing mass in the right orbit, so a biopsy was performed. A biopsy specimen of the mass revealed non-Hodgkin's lymphoma (anaplastic variant, diffuse large B cell type). Distant metastasis was not found using abdominal and thoracic computed tomography and positron emission tomography. He underwent radiotherapy (30 Gy, 15 times). CONCLUSIONS: Orbital non-Hodgkin's lymphoma (B cell type) in a patient with AIDS is rare, but our case showed a good response to radiation therapy.

7.Reproductive Outcomes after Microsurgical Reversal of Tubal Sterilization in Women 36 Years Age or Older.

OBJECTIVE: To determine the reproductive outcomes of women undergoing microsurgical reversal of tubal sterilization at age 36 years or older. MATERIALS AND METHODS: A series of 133 patients who received microsurgical reversal of the previously sterilized fallopian tubes at Seoul National University Hospital from July, 1980 to January, 1992 was reviewed and evaluated for clinical characteristics, pregnancy rates, and factors influencing the outcome of tubal reversal. RESULTS: Of 133 patients, 78 (58.6%) had been sterilized by laparoscopic cautery. Loss of children was a leading cause for tubal reversal. The mean interval from tubal sterilization to tubal reversal was 65.0 months. The overall pregnancy rate was 52.6% (70/133), and the mean interval was 9.4 months from tubal reversal to pregnancy. Excluding 7 patients who were lost to follow-up, 76 pregnancies were confirmed in 63 patients with the delivery rate per patient of 66.7% (42/63). There were no significant differences in age, duration of tubal sterilization, postoperative tubal length between pregnant and non-pregnant groups. CONCLUSIONS: Microsurgical reversal of tubal sterilization could be a justifiable method in women 36 years age or older.

8.A case of newly developed pulmonary lesion during the antitubercular agents in tuberculous pleurisy : A paradoxical response.

Paradoxical response refers to the enlargement of old lesions or unexpected appearance of new lesions after initial improvement following treatment with antitubercular agents. Various types of paradoxical responses have been reported in the world, but they are rarely reported in Korean children. We report the case of a 17-year-old boy who was diagnosed with tuberculous pleurisy and was treated appropriately. Although the tuberculous pleurisy initially responded to medication with resolution of the pleural fluid, a new pulmonary lesion subsequently developed 3 weeks after the initiation of treatment that eventually cleared with continuation of the original drug regimen.

9.Four Cases of Intrahepatic Biliary Cystadenoma and Cystadenocarcinoma.

A biliary cystadenoma (BC) and a cystadenocarcinoma (BCA) are rare neoplasms of the liver. Among 178 patients with primary liver neoplasms who underwent surgery during the last two years at our department, there were only one case of BC and three cases of BCA. The BC case was a 57-year-old female with 15-cm-sized multilocular cystic mass containing mucin. That patient, who had undergone a simple excision of a liver cyst 6 years earlier under the diagnosis of a cystadenoma, received a repeated wedge resection and is still doing well, no recurrence, 41 months after the resection. One BCA case was a 59-year-old female with an 8-cm-sized multilocular cystic mass. She underwent an extended left lobectomy and is still alive, without recurrence, 55 months later. Another BCA case was 77-year-old female with multiple multilocular masses which had degenerated due to sclerotherapy; the masses were removed by a wedge resection. She has been alive 35 months without recurrence. The other BCA case was a 37-year-old female with a 10-cm-sized unilocular mass with lung metastasis. She underwent an extended left lobectomy and survived 22 months. Malignant transformation of a BC to a BCA is well documented, and recurrence is the rule following incomplete resection. Complete resection of a BC and radical resection of a BCA seem to offer a chance for long-term survival.

10.Liver Regeneration following Extended Liver Resection combined with Pancreatoduodenectomy.

BACKGROUND/AIMS: In patients with advanced biliary malignancies, a chance of curability is obtained by only performing extended liver resection with concomitant pancreatoduodenectomy. This hepatopancreatoduodenectomy(HPD) is known to carry high risk of hepatic failure. We evaluated the effect of pancreatoduodenectomy on liver regeneration and the risk of hepatic failure in patients having undergone HPD to prevent complications associated with liver function. METHODS: Sixteen cases of HPD with extended liver resection were reviewed in the aspects of liver regeneration and hepatic failure. Twenty cases of extended right hepatectomy were selected as a control group(ERL group) for comparison of liver regeneration. Liver volumes were measured by computed tomogram volumetry. RESULTS: Resection rates of the liver and the pancreas in the HPD group were more than 51% and about 40%, respectively. Right portal vein embolization was performed in 66% of cases and all cases with obstructive jaundice underwent percutaneous biliary drainage. Rate of liver regeneration at postoperative 1 month in HPD group was 162%, and that of the ERL group was 169%, resulting in no statistical difference. There was no occurrence of hepatic failure in the HPD group. CONCLUSIONS: Forty percent resection of pancreatic parenchyme following concomitant pancreatoduodenectomy may not compromise liver regeneration after extended liver resection. Complete external drainage of obstructive jaundice and preoperative portal vein embolization are recommended as preoperative procedures for patients undergoing extended liver resection with pancreatoduodenectomy.
