The cystic lesions within the knee joint are rare, contrary to those located around the knee. They usually are eniscal or ganglion cyst, reported in association with the joint capsule of the knee or the menisci and close to the peroneal tendons. The intraarticular ganglion cyst arising from the cruciate ligaments are rare and cause pain, effusion and limitation of the knee joint. We report (he case of a patient with right knee joint pain caused hy an intraarticular ganglion cyst arising from the posterior cruciate ligament with review of reference.

2.Displaced Scapula Fracture (Ideberg Type IIb) Combined with a Large Rotator Cuff Tear in Anterior Shoulder Dislocation: A Case Report.

Traumatic anterior shoulder dislocation combined with scapular fracture in elderly patients is relatively rare. In this case, a patient visited Emergency Room of Dong-A University Hospital for shoulder pain after falling off a ladder. Radiographs demonstrated anterior shoulder dislocation with displaced Ideberg type IIb scapula (glenoid fossa) fracture combined with a large rotator cuff tear on magnetic resonance imaging. We performed arthroscopic rotator cuff repair, but a large fragment in the inferior glenoid was left untreated. At the 1 year follow-up visit, the pain visual analogue scale of the patient was 2, the American Shoulder and Elbow Society score was 88 and the patient had gained nearly full range of motion without any apprehension.

4.Hypotensive Effect of Betaxolol Hydrochloride(Betoptic) In Normal Volunteers: Comparative study With Timolol.

Betaxolol Hydrochloride was compared with Timolol in view of the hypotensive effect. Thirty five normal volunteers(70 eyes) of the third decade were enrolled in this study. Thirty volunteers of experimental group were devided into three groups, -0.5% Betaxolol group, 0.25% Timolol group and 0.5% Timolol group. The remaining five volunteers(10 eyes) were used as control group. Double masked single dose study was done by instillating each eye solution into the randomly selected one eye. The intraocular pressure(IOP), Pupil size, Visual acuity, blood pressure, and pulse rate were measured before the instillation of eye solution at first and 30 minutes. 1,2,4,6,12, and 24 hours after the and the statistical significance was estimated by using the paired t-test. The following results were obtained: 1. 0.5% Betaxolol showed a significant reduction of IOP from 30 minutes to 12 hours after the instillation and maximum IOP reduction(18.9 +/- 3.4%) was noted at 4 hours after the instillation. 2. Statistically significant IOP reduction was noted in the contralateral eyes of both Betaxolol and Timolol groups. 3. After the treatment with Betaxolol, the pupil size, visual acuity, blood pressure, and pulse rate were not significantly changed. Transient mild stinging sensation was noted as a local side effect after the instillation of the Betaxolol.

6.Dorsal Capsulectomy of the Metacarpophalangeal Joints.

Extensor tendon adhesion and joint contracture after crushing injury of the hand are common complications that affect finger motion and hand function. The development of stiff joint can be associated with significant morbidity and had a profound influence on the patient's ability to function. Metacarpophalangeal joint capsulectomy is a surgical procedure used to treat patients with joint stiffness and disabling contractures. Seventeen patients with metacarpophalangeal joint contracture underwent capsulectomy with or without tenolysis from June 1997 to December 1998. The procedures were done after conservative methods had failed. The follow-up period was from 2 to 21 months(mean 12 months). After metacarpophalangeal capsulectomy and immediate postoperative exercise, an average gain of 28 degree of metacarpophalangeal flexion was achieved. In conclusion, when indicated, capsulectomy is a useful procedure for the treatment of contracture of the metacarpophalangeal joint. It is imperative for the treating physician to stress prevention in the form of timely appropriate splintage, early rehabilitation, and aggressive intervention if necessary.

7.Effects of Lovastatin(Mevacor(R)) on Lowering Plasma Lipids in Patients with Hyperlipidemia.

Lovastatin, a competitive inhibitor of the rate limiting enzyme in cholesterol biosynthesis was administered to 34 patients with primary hypertlipidemia, 20 mg once daily with the evening meal. Patients experienced mean total and LDL cholesterol reductions of 30.9% and 34.0% respectively. HDL cholesterol level was significantly increased by 15.4% and plasma triglyceride level was decreased by 11.2%. maximal hypocholesterolemic effects were evident at 8 weeks, after which the effects were stable. Adverse effects were noted in 2 patients who had mild gastrointestinal symptoms, that subsided after discontinuing the drug. We concluded that lovastatin is a well tolerated and effective agent for the treatment of primary hyperlipidemia.

9.A Case of Clear Cell Sarcoma in Left Foot.

Clear cell sarcoma of tendon and aponeuroses is slow growing and painless. It occur chiefIy in young adults, predominates in women and is most common in the region of the foot and knee. It is intimately bound to tendons or aponeuroses and is composed of srnall nest or aggregates of round or fusiform, pale staining cells showing prominent nucleoli. And it is tend to recur and to resuIt in eventual development of metastatic growth after a protracted clinical ccurs. Herein, we report a case of clear cell sarcorna of tendon anu aponeuross of metatarsophalangeal joint area between 2nd and 3rd light of left foot. The patient, 50-year-old female had a slow groving, asymptomatic, slightly eevated, emooth surfaced and normal skin colored mass for 2 years. Histopatholcigic tindings in dermis revealed nests of atypical polygonal or monotonous cells. Each cell had vesicular or hyperchrornatic nucleus with prominent nucleoli, occcasionally mitotic figure and abundant eosinophilic cytoplasm. For. treatment, transmetatarsal amputation was done.
