2.A study on diagnostic value of oblique lumbar spine radiographs

A retrospective study was designed to determine the diagnostic contribution of oblique view in 740 consecutive lumbar spine examinations done in adults. In 12 cases (1.62%) there was a change in the radiographic interpretation when the oblique views were used for diagnosis. Considering the significant gonadal radiation, additional cost, and limited diagnostic value, the authors feel that the oblique view should not be a routine part of the initial lumbar spine examination in adults.

3.Percutaneous transhepatic cholangiography in obstructive jaundice

Although many invasive studies in the biliary tract disase are replaced by noninvasive techniques such asultrasonography and CT, PTC does still play an important role indiagnosis of obstructive jaundice. Authors hadperformed 42 patients of PTC with Chiba needle at the department of radiology, Han Gang Sacred Heart Hospital for4 years from Jan. 1978 to April 1982. And we reviewed literatures. The results were as follows; 1. Forty twopatients of PTC were 22 males and 20 females. The success rate was 90.5%(38/42). The failed 4 cases of PTC werenonobstructive hepatobiliary diseases. 2. The PTC diagnosis of 38 cases were biliary stone (21 cases),cholangiocarcinoma(7 cases), pancreatic carcinoma (2 cases), hepatoma (2 cases), ascariasis(2 cases), andclonorchiasis(1 case), and the remainders were normal. 3. We made 2 cases of misdiagnosis on the basis of PTCfindings. They were one case of distal CBD stone with severe cholangitis diagnosed as ampullary carcioma andanother case of proximal CBD stone with cholangitis diagnosed as pancreas carcinoma. 4. We experienced 2 cases ofcomplication such as sepsis and severe hypotension, but they were recovered.

4.Squamous Cell Carcinoma Arising from Mature Cystic Teratoma of the Ovary: A report of three cases .

Malignant transformation develops in a little less than 2% of mature cystic teratomas. A wide variety of malignant tumors may arise within benign mature cystic teratomas, and the most common of these is squamous cell carcinoma, which account for 75~85%. In general, the tumors are in an advanced stage and the prognosis is poor as most patients die within a year. However, when the tumor is confined to the ovary, they have a good prognosis and the 5-year survival rate is 63~83%. We experienced three cases of squamous cell carcinoma arising in mature cystic teratoma. Two of the carcinomas occurred in postmenopausal women: 58-(case 1) and 66-(case 2) year-old, and were confined to the ovaries. They were alive 37 months and 18 months after the operation, respectively. The third case was a 45-year-old premenopausal woman who had an extraovarian extension of the tumor and early recurrence within two months. Histologically, cases 1 and 3 were conventional well to moderately differentiated squamous cell carcinomas and case 2 showed a well-differentiated squamous cell carcinoma with exuberant proliferating trichilemmal tumor-like areas.

5.Antibiotic-Associated Diarrhea in 3 to 6 Month Old Infants with Febrile Urinary Tract Infections.

PURPOSE: This study attempted to investigate the frequency, duration, and risk factors of antibiotic-associated diarrhea (AAD) in infants hospitalized due to febrile urinary tract infection (UTI). This is a basic research on the probiotics used in the prevention and treatment of AAD in infants. METHODS: Medical records of the infants aged 3-6 months hospitalized in Gachon University Gil Hospital from January 2008 to September 2010 due to the febrile UTI were retrospectively reviewed. The episodes of loose or watery stool were investigated for frequency, onset, and duration. Those who had AAD and those who did not (non-AAD) were compared. The antibiotic regimens and the episodes of diarrhea were investigated in AAD group. RESULTS: Total 147 infants were included. Fifty-four (36.7%) showed AAD. Intravenous third-generation cephalosporin (3rd CS) single therapy was used for 102 patients (69.4%), the 3rd CS and non-3rd CS combination therapy for 24 (16.3%), and non-3rd CS combination therapy for 21 (14.3%). There was no significant difference in the dose of cefotaxime between AAD and non-AAD group (P=0.601). According to the antibiotic therapies above, in AAD group, there was no significant difference in the onset and duration of diarrhea respectively (P=0.717, P=0.830). Although the frequency of diarrhea was higher for the 3rd CS and non-3rd CS combination therapy subgroup with 9.25+/-5.30 times/day than the other two subgroups (7.58+/-2.97 times/day in 3rd CS single therapy subgroup, 6.75+/-4.40 times/day in non-3rd CS combination therapy subgroup), there was no statistical significance (P=0.078). CONCLUSION: AAD seems common to the infants aged 3-6 months with febrile UTI, regardless of regimen and amount of antibiotics in usual dosage. Further research on the effects of probiotics used in the prevention and treatment of AAD in infants is warranted.

6.Kinetic Characteristics during Initiation of Gait in Stroke Patients.

OBJECTIVE: The aim of the present study was to understand biomechanical characteristics during the process of initiation of gait (IOG) from the standing position in hemiplegic patients. METHOD: We recorded the ratio of the vertical forces of both limbs to body weight and the movement of net center of pressure (COP) on two force platforms during the process of initiation of gait (IOG) from standing in 10 normal control and 10 hemiplegic patients and processed these data using ELITE DMA acquisition program. All data were collected with uninvolved limb and involved limb and compared each other by independent samples t-test. RESULTS: 1) In 10 hemiplegic patients, nine patients began taking a step with an uninvolved limb in the first, followed by an involved limb. 2) The period of IOG of the swing limb was 0.80 sec when patients walked with an uninvolved side and those of the stance limb was 1.60 sec with involved side. When the control group walked, the period of IOG of the swing limb was 0.70 sec and those of stance limb was 1.50 sec. The period of IOG of the swing limb in uninvolved side and those of the stance limb in involved side showed significant longer than control group. 3) The latency of IOG of swing and stance limb was significantly more prolonged than control groups. 4) Anteroposterior (AP) and mediolateral (ML) distance of net COP from release to unloading with uninvolved limb showed significant longer than control groups. 5) The velocity of net COP displacement from start to release with involved limb was significantly slower than control groups. CONCLUSION: This pattern of IOG could be used as one of the tools to evaluate postural control during gait training in the hemiplegic patients with gait disabilities, and it can be used as a basis for specific therapeutic intervention, and it allows evaluation of the effectiveness of treatment.

7.Comparison of Optic Neuritis with Seropositive Myelin Oligodendrocyte Glycoprotein Antibody and Seropositive Neuromyelitis Optica Antibody

Purpose:
To compare clinical outcomes of Korean patients with seropositive myelin oligodendrocyte glycoprotein antibody (MOG-IgG)-related optic neuritis (ON) and those with neuromyelitis optica antibody (NMO-IgG)-related ON.
Methods:
Medical records were analyzed retrospectively among patients diagnosed with ON from January 2018 to June 2020, who were positive for MOG-IgG or NMO-IgG on serological tests. Clinical manifestations and outcomes were collected and compared between the two groups.
Results:
The study included 14 eyes of 10 MOG-IgG positive patients, and 38 eyes of 25 NMO-IgG positive patients. Final visual acuity differed significantly between the two groups (p = 0.002), and the proportion of patients with no light perception was higher in the NMO-ON group (11%) compared with the MOG-ON group (0%). The mean deviation value on visual field tests also showed a significant difference between NMO-ON and MOG-ON groups (-18.98 ± 12.36 dB versus -10.17 ± 10.03 dB, respectively; p = 0.018). The recurrence rate was 42% in the NMO-ON group and 79% in the MOG-ON group (p = 0.020).
Conclusions
The clinical outcomes of NMO-ON and MOG-ON patients differed significantly. MOG-ON patients were more likely to experience a relapse. Patients with NMO-ON were given a poor visual prognosis despite having a lower recurrence rate than those with MOG-ON.

9.An investigation of the current status of the new Health Technology Assessment in Korea and factors influencing assessment results

Background:
This study aims to evaluate factors influencing assessment results in the new Health Technology Assessment (nHTA) in Korea.
Methods:
We analyzed publicly available nHTA reports obtained from the program’s website. A total of 258 reports, encompassing 305 technologies, were included. Reported details were categorized into three main areas: technical characteristics, evaluation methods, and publication types. To investigate differences in evidence level (high, medium, or low) and assessment results (pass or fail) according to these categories, we employed a chi-squared test. Univariate and multivariate logistic regression analyses were further conducted to identify factors associated with evidence level and assessment results.
Results:
nHTA reports employing meta-analysis and included randomized trials for evidence synthesis exhibited a higher likelihood of achieving high evidence level (odds ratio [OR], 5.008; 95% confidence interval [CI], 1.265- 18.826 and OR, 27.052; 95% CI, 7.802-103.330, respectively). Increasing evidence level was significantly associated with a higher possibility of passing the assessment (OR 2.789; 95% CI, 1.284-6.057). However, in univariate analysis, neither performing meta-analysis nor including randomized trials, both of which were associated with evidence level, demonstrated a statistically significant association with assessment results.
Conclusion
This study represents the first systematic analysis of factors influencing nHTA assessment results in Korea. While increased evidence level was associated with positive assessment outcomes, factors affecting the evidence level itself did not directly influence assessment results. This suggests the need for further efforts to effectively integrate high-level evidence into assessment decisions within the nHTA program.
10.Transition metal induces apoptosis in MC3T3E1 osteoblast: Evidence of free radical release.

Transition metal ions including Se2+, Cd2+, Hg2+ or Mn2+ have been thought to disturb the bone metabolism directly. However, the mechanism for the bone lesion is unknown. In this study, we demonstrated that MC3T3E1 osteoblasts, exposed to various transition metal ions; selenium, cadmium, mercury or manganese, generated massive amounts of reactive oxygen species (ROS). The released ROS were completely quenched by free radical scavengers-N-acetyl cysteine (NAC), reduced glutathione (GSH), or superoxide dismutase (SOD). First, we have observed that selenium (10 micrometer), cadmium (100 micrometer), mercury (100 micrometer) or manganese (1 mM) treatment induced apoptotic phenomena like DNA fragmentation, chromatin condensation and caspase-3-like cysteine protease activation in MC3T3E1 osteoblasts. Concomitant treatment of antioxidant; N-acetyl-L-cysteine (NAC), reduced-form glutathione (GSH), or superoxide dismutase (SOD), prevented apoptosis induced by each of the transition metal ions. Catalase or dimethylsulfoxide (DMSO) has less potent inhibitory effect on the apoptosis, compared with NAC, GSH or SOD. In line with the results, nitroblue tetrazolium (NBT) stain shows that each of the transition metals is a potent source of free radicals in MC3T3E1 osteoblast. Our data show that oxidative damage is associated with the induction of apoptosis in MC3T3E1 osteoblasts following Se2+, Cd2+, Hg2+ or Mn2+ treatment.
