3.Intracardiac Electrogram at Successful Site of Radiofrequency Catheter Ablation in Patients with Atrioventricular Nodal Reentrant Tachycardia.

BACKGROUND AND OBJECTIVES: Ablation of the slow pathway in patients with atrioventricular nodal reentrant tachycardia (AVNRT) can be performed by using a specific intracardiac electrogram findings predicting a successful radiofrequency catheter ablation. The purpose of the present study is to recognize a specific intracardiac electrogram findings predicting a successful sites of radiofrequency catheter ablation in patients with AVNRT. MATERIALS AND METHODS: The study population consisted of the 18 patients (7 males, mean age:46 yr) to undergo successful catheter ablation using radiofrequency current in order to eliminate AVNRT from January 1993 to september 1994. We have analyzed local intracardiac electrogram at successful and unsuccessful sites of radiofrequency catheter ablation before the radiofrequency application: Atrial electrogram amplitude, duration, number of peaks in atrial electrogram, atrial/ventricular (A/V) electrogram amplitude ratio, and presence of His potential and/or slow potential. RESULTS: Of 18 patients, 16 patients underwent a slow pathway ablation, the other 2 patients a fast pathway ablation. The mean A/V electrogram amplitude ratio at successful and unsuccessful sites was 0.69+/-0.91 and 1.86+/-2.03, respectively. The mean atrial electrogram duration and number of peaks at successful and unsuccessful sites was 57+/-16 msec vs 69+/-16 msec and 1.7+/-0.5 vs 2.2+/-0.7, respectively. His bundle electrogram was seen in one slow pathway ablated and one fast pathway ablated patient. No slow potential could be identified in any of these 18 patients. CONCLUSION: We think that A/V electrocardiogram amplitude ratio below 0.5 at posterior interatrial septum along tricuspid annulus is important marker indicating a successful ablation sites.

4.Clinical analysis in reconstruction of orbital blow-out fracture using the hydroxyapatite.

The blow-out fracture can be reconstructed by various autogeneous and alloplastic material. Particulate, nonresorbable hydroxyapatite is currently one of the choice of implant material available for reconstruction of blow-out fracture. Hydroxyapatite is radiopaque ceramic, physically and chemically similar to enamel and cortical bone. It is a biomaterial derived from natural corals to use as a bone graft substitute. And we looked into the clinical usefulness of 2 type of hydroxyapatite with their advantages and disadvantages in reconstruction of blow-out fracture. 183 patients with blow-out fracture who underwent surgical reconstruction with two types of hydroxyapatite from March 1933 to July 1977 have been analyzed the results of surgical reconstructions, and have been followed up for more than a year. And the condition of formerly inserted hydroxyapatite was observed in the patients who needed 2nd surgical reconstruction due to the enophthalmos. The disadvantages of hydroxyapatite are fragility, size and contour limitations. In spite of these demerits, hydroxyapatite can be one of the prospective materials to reconstruct orbital floor. Through the clinical experiences for 5 years, we have not found any of complications of exposure, infection, and foreign body reaction. Low rates of diplopia, limitation of ocular movement, and enophtalmos was observed. Hydroxyapatite was well adherent to adjacent orbital bone in most patients who needed secondary reconstruction for enopthalmos. In conclusion, our study shows that the availability of hydroxyapattite in reconstruction of blow-out fracture is recommendable, with low complication rates. Hydroxyapatite important appears to be well tolerated, and provides useful alloplastic prosthesis with few problems in reconstruction of blow-out fracture.

6.Spontaneous splenic rupture in Plasmodium vivax malaria.

Malaria can present with various clinical symptoms and complications. While a tertian malaria form that is especially prevalent in Korea is characterized by mild clinical progression, occasional splenic complications are known to occur. A 26-year-old Korean male soldier without prior medical history visited The Armed Forces Capital Hospital with left upper quadrant abdominal pain one day ago. Hemostasis under laparoscopic approach was attempted. The operation was converted into laparotomy due to friable splenic tissue and consequently poor hemostasis. Splenectomy was performed. The patient was discharged at postoperative day 17 without complication. While numerous diseases can result in splenic complications, such as splenic rupture, malarial infection is known as the most common cause. The incidence of malarial infection in Korea is increasing annually, and there are occasional reports of splenic rupture due to the infection, which requires attention.

7.Study on the Effect of Hydrosalpinx Fluid on Sperm Motility.

This study was performed to identify the effect of the hydrosalpinx fluid on sperm motility. It has been reported that the patients with hydrosalpinx show the outstandingly lower success rate than other patients having infertility by different factors. It is unclear that the cause of it is influenced by hydrosalpinx fluid directly or by secondary chronic inflammation of endometrium. We wanted to know if the hydrosalpinx fluid influences sperm motility parameters directly such that it is related to the development of infertility. Therefore, using computer assisted semen analyzer (CASA), we observed, from February to July, 1997, how sperm motility, sperm progressive motility, sperm curvilinear velocity, sperm lateral head displacement, sperm straightness and sperm linearity change after treating normal sperm with hydrosalpinx fluid to evaluate sperm function on infertility. The result was that the study group (n=32) has the tendency to differ from the control group (n=32) on sperm motility, progressive motility, curvilinear velocity, lateral head displacement, straightness and linearity. We concluded that the hydrosalpinx fluid, with varying degree, directly has the harmful effects on sperm motility parameters, that is, curvilinear velocity, lateral head displacement and linearity of sperm which are related to the hyperactivation, hence decreased capacitation.

9.Epidemiologic investigation to identify the cause of an infant methemoglobinemia.

Epidemiologic investigation was conducted on January, 1993 in Seoul to identify the cause of an infant methemoglobinemia. Field investigation of the area of outbreak, survey of household and family members, analysis of ground water, and blood tests of involved family members were performed. Following results were obtained. On analyzing the quality of the ground water on patients's household high levels of nitrate was found indicating contamination of water as the cause of a methemoglobinemia outbreak. On analyzing the quality of the ground waters on seven other places within the neighborhood five were contaminated by nitrate in concentration that exceeded the permissible limit implying presence nearby source of contamination. Sources of contamination were thought to be originating from human waste in conventional bathroom facilities, chicken manure used in nearby orchards and plant fields or fertilizers. But the results of water analysis with presence of bacteria or E.coli, concentration of potassium, phosphate and the past history of diarrhea among family members, chicken manure suggested the most possible source of contamination. To evaluate the health status of members in the neighborhood past history was reviewed revealing no prior existence of patient with cyanosis and 65 people in the neighborhood had normal levels of methemoglobin concentration in their blood. Conclusively, the ground water on patient's household was contaminated with nitrate and despite provision of adequate water supply, family members of the patient along with distrust in the water supply system had used ground water as their source of drinking water resulting of methemoglobinemia. Many suburban area of Seoul and country side thought to be having similar problems concerning contaminated ground water supply and dormant outbreak of patients as a result of the drinking of the contaminated water. Epidemiologic investigation and water analysis of ground waters are advised.
