1.Point Prevalence of Anxiety and Depression among Cancer Inpatients in a General Hospital: A Preliminary Study for the Development of Distress Management Strategies in Cancer Patients.

OBJECTIVES: This study aimed to examine the point prevalence of anxiety, depression and the associated risk factors in hospitalized patients with cancer. METHODS: A total of 114 patients with cancer who admitted to the Cancer Center in St. Mary's Hospital were enrolled for a day. The patients were asked to complete Hospital Anxiety and Depression Scale(HADS). Sociodemographic and cancer related clinical variables were also evaluated. RESULTS: The point prevalence of anxiety and depression was 10.5% and 34.2%, respectively, as assessed with HADS. Logistic regression models revealed that distant metastasis was significantly associated with anxiety in patients with cancer and previous chemotherapy and Low socioeconomic status were significantly associated with depression. CONCLUSIONS: Our results indicate that more than one third of patients with cancer suffer from either anxiety or depression. In addition, previous chemotherapy as well as low socioeconomic status and distant metastasis were associated risk factors of anxiety and depression, respectively. These findings suggest the need for screening and systematized psychosocial intervention for psychological distress in hospitalized patients with cancer.

2.Association between Mismatch Negativity and Functional Status in Patients with Schizophrenia.

OBJECTIVES: It has been constantly reported that mismatch negativity (MMN) is impaired in patients with schizophrenia. However, the mechanism which relates impaired MMN and schizophrenia is not clear yet. The aim of this study is to investigate the association between MMN and clinical variables including functional status in patients with schizophrenia. METHODS: The present study assessed MMN using passive auditory oddball task in 26 patients with schizophrenia and 48 healthy controls. Repeated measures Analysis of Variance with age as a covariate was carried out for comparing peak amplitude and latency of MMN at 8 central line electrodes (FPz, Fz, FCz, Cz, CPz, Pz, POz, Oz) across groups. Pearson's correlation was performed to reveal the relationship between MMN and clinical variables including neurocognitive test results and the Global Assessment of Functioning score. RESULTS: MMN amplitude was significantly reduced in patients with schizophrenia compared with healthy controls. Pearson's correlation showed that subsets of short form of Korean Wechsler Adult Intelligence Scale scores and GAF scores were associated with MMN amplitude in patients with schizophrenia. CONCLUSION: These findings suggest that MMN amplitude is associated with current functional status including cognitive function in patients with schizophrenia.

3.Studies on the Morphological Changes of Neurons in Dorsal Root Ganglion and Spinal Dorsal Horn of the Rat with an Experimental Peripheral Neuropathy.

Animal models for human chronic pain syndromes were developed and widely used for pain research. One of thsese neuropathic pain model by Kim and Chung[1992] has many advantages for operation and pain elicitation. We have examined the c-fos protein, substance P, CGRP immunoreactivity in dorsal root ganglia and dorsal horn in this neuropathic model. About 50 Sprague-Dawley rats were used for this study. L5 and L6 spinal nerve were ligated tightly to produce neuropathic pain model. After 2, 4, 8, 16, 24 hours and 1 week of surgery, rats were anesthesized and sacrificed by perfusion through the left ventricle with saline followed by 0.1M phosphate buffer[pH 7.4] containing 3% paraformaldehyde, 3% glutaraldehyde, and 0.1% picric acid. After confirmation of the roots transected by the surgery, the L5 and L6 dorsal root ganglion and spinal cord were removed and processed for immunohistochemistry. All tissue sections were immunohistochemically stained for substance P, CGRP and c-fos by using the peroxidase-antiperoxidase[PAP] method. Count the number of immunostained substance P and CGRP dorsal root ganglion cells and c-fos immunoreactive dorsal horn cells and analyzed statistically with Mann-Whitney U test. The results are as follows. 1. The number of c-fos protein immunoreactive neurons in the superficial layer of dorsal horn were increased markedly at 2 hours after operation, gradually decreased to normal level 1 week after operation. 2. The number of c-fos protein immunoreactive neurons in the deep layer of dorsal horn were gradually increased to the peak 24 hours after operation, decreased to normal level 1 week after operation. 3. The number of substance P and CGRP immunoreactive L5 and L6 dorsal root ganglion neurons were decreased markedly at 1 week after pain model operation. In conclusion, after neuropathic pain model operation, c-fos protein were immediately expressed in the superficial layer of spinal dorsal horn, thereafter c-fos protein in the deep layer of spinal dorsal horn were expressed. CGRP and substance P immunoreactive neurons were decreased markedly 1 week after neuropathic pain model operation.

4.A study on osteoblast-like cell responses to surface-modified titanium.

STATEMENT OF PROBLEM: The success of implants depends on intimate and direct contact of implant material on bone tissue and on functional relationship with soft tissue contact. Creation and maintenance of osseointegration depend on the understanding of the tissue's healing, repairing, and remodeling capacity and these capacities rely on cellular behavior. Altering the surface properties can modify cellular responses such as cell adhesion, cell motility, bone deposition. Therefore, various implant surface treatment methods are being developed for the improved bone cell responses. PURPOSE: The purpose of this study was to evaluate the responses of osteoblast-like cells to surface- modified titanium. MATERIALS AND METHODS: The experiment was composed of four groups. Group 1 represented the electropolished surface. Group 2 surfaces were machined surface. Group 3 and Group 4 were anodized surfaces. Group 3 had low roughness and Group 4 had high roughness. Physicochemical properties and microstructures of the d iscs were examined and the responses of osteoblast-like cells to the discs were investigated. The microtopography was observed by SEM. The roughness was measured by three-dimension roughness measuring system. The microstructure was analyzed by XRD, AES. To evaluate cell responses to modified titanium surfaces, osteoblasts isolated from calvaria of neonatal rat were cultured. Cell count, morphology, total protein measurement and alkaline phosphatase activities of the cultures were examined. RESULTS AND CONCLUSION: The results were as follows 1. The four groups showed specific microtopography respectively. Anodized group showed grain structure with micropores. 2. Surface roughness values were, from the lowest to the highest, electropolished group, machined group, low roughness anodized group, and high roughness anodized group. 3. Highly roughened anodized group was found to have increased surface oxide thickness and surface crystallinity. 4. The morphology of cells, flattened or spherical, were different from ach other. In the electropolished group and machined group, the cells were almost flattened. In two anodized groups, some cells were spherical and other cells were flattened. And the 14 day culture cells of all of the groups were nearly flattened due to confluency. 5. The number of attached cells was highest in low roughness anodized group. And the machined group had significantly lower cell count than any other groups(P<.05). 6. Total protein contents showed no difference among groups. 7. The level of alkaline phosphatase activities was higher in the anodized groups than electropolished and machined groups(P<.05).

5.Characteristics of Eating Behavior in Elders with Dementia residing in Long-Term Care Facilities.

PURPOSE: The purpose of this study was to explore characteristics of eating behavior according to level of functional status of elders with dementia (EWD), and to examine feeding time, change in food intake and body mass index (BMI) according to eating behavior. METHODS: Participants were 149 EWD residing in long-term care facilities located in Seoul or Gyeonggi province and evaluated using the Mini-Mental State Exam-Korean version, Korean version-Activities of Daily Living, and Eating Behavior Scale (EBS). Feeding time, change in food intake, and BMI were also measured. Data were analyzed using SPSS 17.0, specifically descriptive statistics, ANOVA, and Chi-square test. RESULTS: Participants' mean EBS score was 10.43+/-6.01 and half of them (54.4%) needed moderate or total assistance while eating. The EBS score was significantly lower for elders with severe dementia compared to those with mild or moderate dementia; and elders with severe ADL dependence compared to those with mild or moderate ADL dependence. Lower EBS scores were related to longer feeding time, a greater the rate of participants with decreased food intake and 'underweight' BMI. CONCLUSION: Nursing intervention programs which are designed for EWD are needed to maintain functional eating skills and prevent negative consequences in this population.

6.Post-Marketing Surveillance of Levocabastin Eye Droups in 1997: A Multicenter Surveillance Study.

A multicenter post-marketing surveilance study has been conducted to evaluate the efficacy and safety of levocabastine eye drops(0.5mg/ml). Total 340 patients suffering from allergic conjunctivitis(96%) or other similar ophthalmic disorders were participated. Clinical symptoms(itching, hyperemia, tearing and chemosis) were assessed according to a four point scale before treatment and at every visit. Total symptom severity score before therapy, 6.67(N=340), was remarkably decreased to 3.28(N=252) at 1 week, and to 2.39(N=197) at 2 weeks. The physicians rated the application as `very effective` 1.5% of patients. Adverse reactions(10.3%) were limited to ocular irritation like stinging and burning sensation. These events were even lower in children(age< OR =14 yrs) and elderly(age> OR =65 yrs) showing 7.0% and 9.5% respectively. Patients with gepatic, renal impairment and hypertension did not experience any special adverse effects. The overall assessment by physicians showed that the application in 80.9% of the patients was evaluated above `useful`(i.e, `very useful`; 17.1%, `useful`; 63.8%, `slighty useful`; `7.1%). therefore, it was conclued that levocabastine eye drops is a a safe and effective treatment for allergic conjnctivitis and other similar ophthalmic disorders.

8.The Relationship of Communication Competence, Professional Self-Concept and Stress in Clinical Practice of Nursing Students.

PURPOSE: This study was conducted to investigate the relationships among communication competence, professional self-concept, and stress in clinical practice of nursing students. METHODS: The study participants were 308 senior year nursing students in D city and K province. Collected data were analyzed using t-tests, analysis of variance, Pearson's correlation coefficient, and stepwise multiple regression with the SPSS Version 20.0. RESULTS: There were significant differences in participants' stress of clinical practice based on their level of satisfaction on clinical practice, self-confidence of nursing and difficulty with relationships in clinical practice. A negative correlation was detected between stress in clinical practice and the professional self-concept of nursing students. Significant predictors of the perceived stress of clinical practice were satisfaction in clinical practice and pride in nursing. These accounted for 18% of the perceived stress of clinical practice in a regression model. CONCLUSION: The results may serve as basic data in support of the need to the develop effective systematic stress management programs and clinical training courses in order to attenuate the stress experienced during clinical practice of nursing students.

9.Fluid Overload in Dialysis Patients.

End-stage renal disease(ESRD) is the fatal retention of non-volatile, metabolic waste products, salt, and water due to extensive loss of functioning nephrons. Renal replacement therapy is primarily aimed to remove retained waste products and fluid. Adequacy of dialysis is the dose of dialysis below which one observes a significant worsening of morbidity and mortality. Urea kinetic modeling, Kt/Vurea, etc. is regarded as a quantitative guideline of dialysis adequacy in both hemodialysis(HD) and peritoneal dialysis(PD). Water is one of the most important uremic toxin retained in ESRD patients. The importance of fluid overload on morbidity and/or mortality in dialysis patients is yet to be evaluated. Recent technology of HD provides adequate Kt/ Vurea for relatively short dialysis time and higher patient survival. Blood pressure control and extracellular fluid(ECF) volume are closely related with dialysis time in HD patients. Short dialysis time, 3-4 hours per session may not enough to control blood pressure. Hypertension is an important risk factor of survival in dialysis patients. Fluid overload is the most important factor of hypertension in dialysis patients. Patients with uncontrolled hypertension have higher mortality rate despite similar Kt/Vurea compared with patients with good blood pressure control. A longer dialysis time improves ECF volume and blood pressure control and decreases cardiovascular mortality. Continuous ambulatory peritoneal dialysis(CAPD) is a slow continuous therapy and is believed to maintain better control of ECF volume and blood pressure compared with hemodialysis. ECF volume and blood pressure controls are improved after initiation of CAPD, however, return to pre-CAPD levels after a few years of CAPD when residual renal function disappears. Patients transferred to hernodialysis from CAPD lose around 4kg of body weight for a few months on HD. Approximately 25% of CAPD patients are clinically fluid overloaded. Increased peritoneal permeability is an independent risk factor for patient and technique survivals in CAPD patients. Fluid overload in CAPD patients with increased peritoneal pernability is believed to be an important underlying mechanism of increased mortality and technique failure in this group. In conclusion, fluid overload is an important risk factor of mortality in dialysis patients. Sufficient dialysis time resulting good controls of ECF volume and blood pressure in hemodialysis patients and individualized dialysis prescription according to the peritoneal permeability in CAPD patients are vital to provide adequate dialysis and to decrease cardiovas-cular mortality.

10.Vitiligo-like Depigmentation Associated with Metastatic Melanoma of an Unknown Origin.

Although malignant melanoma usually occurs after the diagnosis of vitiligo-like depigmentation, the latter is rarely followed by the former. We herein report on such a case in which recognition of the vitiligo-like depigmentation preceded diagnosing the metastatic melanoma by several months. A 56-year-old woman had first developed vitiligo-like depigmentation on the forehead, eyelids, neck and back 18 months previously and thereafter she detected a hard mass in the left axilla 2 months previously. Based on the histologic findings, the axillary mass was diagnosed as metastatic melanoma. To evaluate the primary tumor focus, thorough examinations that included PET-CT, bone scan and sigmoidoscopy were performed, but we couldn`t find any the original primary tumor. Our case suggests that the vitiligo-like depigmentation could be a sign that heralds metastatic melanoma.
