2.A simulation system of otology microsurgery based on distributed virtual reality technology.

Traditionally, a novice ear doctor acquires the skillfully operative ability through dissecting cadaveric temporal bone in ear surgical training lab. For conceivable reasons donated bodies are always short in supply, virtual reality-based simulator provides a new model for surgical training. It has been proven that the simulator is a useful tool for surgical training. And it can be used in the rehearsal of operation procedures, real-time operation guidance, and remote surgery. Our design is different from the existing virtual surgical system. It has a parallel, distributed multi-workstation with the vivid three-dimensional model of temporal bone, and it can be used not only to learn the ear surgical skills in the virtual environment, but also preoperative plan and surgical consultation.

3.The application of arterialized venous digital island flap in fingertip reconstruction.

OBJECTIVETo introduce an operation for fingertip reconstruction.
METHODSThe vascularity of the reverse dorsal island flap is augmented by performing an arteriovenous anastomosis between the dorsal vein in the flap and a digital artery at the fingertip. The flap was used in ten patients for reconstruction of their fingertip defects.
RESULTSAll the ten flaps survived and the appearance was good.
CONCLUSIONThe method is easy except for the necessity of performing standard microvascular surgery. The flap is a good option for repairing fingertip defects.

5.More stress should be laid on the application of microsurgical techniques in the repair of destructive burns and traumas, and intractable wounds.

Destructive burns and traumas which create composite tissue damages, as well as intractable wounds, usually cause the difficulties or inefficacy in the repair and management through ordinary skin grafting or tissue transplantation owing to the complex defects, unhealthy healing condition, or high requirements for the function and appearance reconstruction of the injured local tissues. The advantages of free tissue transplantation with microsurgical techniques in the tissue repair and reconstruction have been demonstrated as meeting the composite tissues restoration, improving blood supply, avoiding further damage to the wound surrounding tissues, and simplifying the distant tissue transplantation procedure, and therefore significantly decreasing the deformities with good functional and morphological outcomes, and dramatically reducing the hospitalization duration with less complications and faster restitution. In the present discussion, basing on the general literature review and the summary of our long-time clinical application of microsurgical techniques in dealing with serious injuries, we put forward the view that the microsurgical techniques should be considered as the first option for the treatment of destructive burns and traumas, and intractable wounds. And in some cases, only microsurgical techniques can be used, otherwise amputation would be unavoidable, or the optimal treatment would be abandoned or delayed. Meanwhile, it should be emphasized that the risk and the failure rate of performing microsurgical operation can be overcome through strictly hard training.

An arteriovenous fistula may be defined as a vascular abnormality in which there are one or more direct or indirect communication between an artery and a vein without any intervening capillary bed. Largely, such fistulas are divided into two groups-acquired (traumatic) or congenital in origin. There is a general agreement that congenital A-V fistula results from a failure in differentiation of the common embryonic anlage into true artery and vein. The authors have experienced a case of congenital A-V fistula formed in the 4th finger of the left hand. Such fistula is very difficult to excise completely with the conventional method. But we have obtained the excellent result by utilizing advanced meticulous microsurgical technique in excision.

7.Application of peroral endoscopic-assisted laryngeal microsurgery in children with laryngeal neurofibromas.

Objective:To investigate the clinical manifestations and the effect of peroral endoscopic-assisted laryngeal microsurgery for children with laryngeal neurofibroma, and to provide clinical reference for the diagnosis and treatment of this disease. Methods:The clinical data of 4 children with laryngeal tumors admitted to the Department of Otorhinolaryngology, Children's Hospital of Chongqing Medical University from January 2021 to June 2023 were retrospectively analyzed. Laryngeal tumors were removed by peroral endoscopic-assisted laryngeal microsurgery. One case underwent tracheotomy at the same time, and one case was simultaneously performed with laryngeal T tube placement and tracheotomy. Results:Surgical resection is the best treatment for laryngeal neurofibroma, and laryngeal microsurgery should be actively used for patients with surgical indications.This surgical method has the advantages of good efficacy, minimal invasion, aesthetics and preservation of laryngeal function, which not only ensures safety, but also improves the quality of life after surgery, and has the value of development and promotion.

9.Movement capture of knot-tying manipulation in microsurgeries.

The surgical dexterous suturing and knotting movements are photographed with binocular stereo vision and the movements' locus is captured through identifying the RGB values in a series of key-frames. Then a normal movement path of knot-tying is obtained through curve-fitting and smoothing. And meanwhile, the workspace of knot-tying manipulation in microsurgeries is got. And thus, a good foundation is laid for the design of an automatic knotting mechanism and for the establishment of a virtual surgery training system.
