Objective:To investigate the current situation and correlation between organizational climate perception, professional identity, self-disclosure and transformation impact of new nurses in oncology department, and explore the chain mediating effect between organizational climate perception and professional identity between self-disclosure and transformation shock, so as to provide basis for further formulating the intervention plan of transformation impact. Methods:This study was a cross-sectional survey. From August 2021 to April 2022, a total of 252 newly graduated nurses in oncology department from nine Class ⅢGrade A hospitals in Henan Province were selected as the research objects by the convenient sampling method. General Information Questionnaire, Transformation of New Nurses Impact Evaluation Questionnaire, Distress Disclosure Index, Nursing Organization Climate Perception Scale, Professional Identity Scale were used for survey. Pearson correlation analysis was performed, and AMOS26.0 software was used to build a chain intermediary model, and the model was tested by Bootstrap method. Results:The total score of the transformation impact of new nurses in the oncology department was (96.51 ± 13.04); the total score of the Distress Disclosure Index was (33.84 ± 7.18); the total score of Nursing Organizational Climate Perception Scale was (99.16 ± 12.97); the total score of the Professional Identity Scale was (91.23 ± 19.92). Self-disclosure, organizational climate perception and professional identity were negatively correlated with transformation impact ( r = - 0.743, - 0.684, -0.631, all P<0.05). The indirect effect of self-disclosure on transformation impact was true, and the total indirect effect was 0.435. The specific mediating effect of nursing organizational climate perception accounted for 51.5% (22.4/43.5) of the total indirect effect and that of professional identity 15.9%(6.9/43.5) of the total indirect effect. The organizational climate and professional identity accounted for 32.4%(14.1/43.5) of the total indirect effects. Conclusions:Self-disclosure, organizational climate perception and professional identity of new nurses in oncology department was negatively correlated with transformation impact. Self-disclosure is positively correlated with organizational climate perception and professional identity, organizational climate perception and professional identity play a chain mediating effect between self-disclosure and transformation impact. It is suggested that managers should adopt positive and effective self-disclosure intervention programs and develop relevant training programs to improve the organizational climate perception and professional identity level of new nurses in oncology department, so as to reduce their transformation impact.

Objective:To explore the effects of professional perception based empathy training on clinical communication competency, perceived professional benefits and self-efficacy of junior nurses. Methods:The study was designed with the quasi-experimental study. From March to September 2018, a total of 67 junior nurses with low levels of self-efficacy at Henan Cancer Hospital were recruited as subjects with the way of voluntary registration. All of nurses were divided into 5 groups, 12 to 15 nurses in each group. This study carried out the face-to face empathy training taking professional perception as the main content5 times for 5 weeks. The effects were evaluated with the Nurses' Clinic Communication Competency Scale, Nurses' Perceived Profession Benefits Questionnaire and the General Self-efficacy Scale (GSES) before and after training. SPSS 22.0 statistical software was used in statistical analysis. Results:There were three nurses quitting the training and 64 nurses completed the training. Among 64 nurses, the total average scores of clinic communication competency, perceived professional benefits and general self-efficacy after training were (3.75±0.66) , (4.65±0.40) and (2.53±0.20) respectively higher than those before training with statistical differences ( t=5.058, 7.911, 25.979; P<0.01) . Conclusions:Professional perception based empathy training can improve clinical communication competency, perceived professional benefits and self-efficacy of junior nurses which could be further expanded and applied in nurses.

Objective To explore the influence of spiritual care training program on nurses' spiritual well-being and spiritual care competency. Methods From March to September 2018,a total of 80 nurses working in a provincial cancer hospital were recruited by voluntary registration to participate in this study. They were randomly divided into experimental group(n=40)and control group(n=40)by random number table. The experimental group received the specially designed spiritual care training twice a month for 6 consecutive months on the basis of regular vocational studies. The main forms included expert lectures, group interventions, providing spiritual care for cancer patients, and clinical case reporting. The control group participated in the vocational study organized by the hospital for 6 consecutive months. Before and after the training, the Nurses' Spiritual Health Scale and the Spiritual Care Cognitive Scale were used to investigate and test the nurses in two groups, and the implementation effect was compared. Results After 6 months of training, the total scores of Nurses' Spiritual Health Scale and the Spiritual Care Cognitive Scale in the experimental group were (106.30±8.72)and(185.75±16.53), which were higher than those in the control group, and the differences were statistically significant(t=5.38,4.52;P< 0.01);the scores in each dimension were also higher than those in the control group, and the differences were statistically significant(t=2.22-5.22,P< 0.05). And in the experimental group the total scores and scores in each dimension of Nurses' Spiritual Health Scale and the Spiritual Care Cognitive Scale after the intervention were higher than before the intervention, and the differences were statistically significant(t=3.18-8.99,P<0.01). Conclusions The spiritual care training program based on the concept of self-growth with patients can improve nurses' spiritual well-being and nurses' perceptions of spiritual care.

Objective:To explore the influencing factors of postoperative coping style of patients with esophageal cancer. Methods:This study was a cross-sectional study. From July 2018 to July 2020, convenience sampling was used to select 65 patients with esophageal cancer who underwent radical surgery for esophageal cancer in a ClassⅢ Grade A hospital in Henan Province as the research object. The General Information Questionnaire and the Chinese version of the Medical Coping Modes Questionnaire (MCMQ) were used to investigate the patients one day before the patients were discharged. Single factor analysis was used to analyze influencing factors. A total of 65 questionnaires were distributed and 65 valid questionnaires were returned with the valid response rate of 100%.Results:The results of single factor analysis showed that there were statistically significant differences in the confrontation dimension scores of patients with different ages, education levels, and family monthly income per capita ( P<0.05) . There were statistically significant differences in the avoidance dimension scores of patients with different genders and ages ( P<0.05) . There were statistically significant differences in the resignation dimension scores of patients with different ages, occupational status, and family monthly income per capita ( P<0.05) . Conclusions:Age, gender, occupational status, education level, and family monthly income per capita are the influencing factors of postoperative coping style of patients with esophageal cancer. Medical and nursing staff can take targeted intervention measures based on their influencing factors and guide patients to adopt a positive coping style to face the disease.
