Objective:To explore the effect of medical alliance combined with video recording guidance on the out-of-hospital rehabilitation of elderly patients undergoing hip replacement. Methods:Using the convenient sampling method, a total of 58 aged patients who were hospitalized in the Department of Orthopedics and underwent hip replacement in the First Affiliated Hospital of Zhengzhou University from March 2018 to June 2019 were selected as research objects. They were randomly divided into the observation group and the control group, with 29 cases in each group. The control group was given conventional out-of-hospital rehabilitation management while the observation group was given medical alliance combined with video recording guidance. The serum albumin, Harris score, total body skeletal muscle mass, skeletal muscle mass of the affected limb, incidence of deep venous thrombosis (DVT) and incidence of artificial hip dislocation were compared between the two groups before and after the intervention. Results:After intervention, the serum albumin of the observation group was (36.73±1.95) g/L, the Harris score was (89.29±5.25) , the total body skeletal muscle mass was (17.91±1.18) kg and the skeletal muscle mass of the affected limb was (5.38±0.79) kg, which were all higher than those of the control group, and the differences were statistically significant ( t=3.524, 7.231, 4.584, 4.252; P<0.05) . After intervention, the incidence of DVT in the observation group was 3.45% (1/29) , and the difference was not statistically significant when compared with 13.79% (4/29) in the control group (χ 2=1.970, P>0.05) . After the intervention, the incidence of artificial hip dislocation in the observation group was 0 (0/29) , which was lower than 20.69% (6/29) in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Medical consortium combined with video guidance can effectively improve the out-of-hospital rehabilitation effect of elderly patients undergoing hip replacement surgery and reduce the occurrence of complications. Medical alliance combined with video recording guidance can effectively improve the out-of-hospital rehabilitation effect of elderly patients with artificial hip replacement and reduce the incidence of complications.

Objective:To explore effects of health education based on teach-back method on functional exercise compliance in patients with kinesophobia after total knee arthroplasty (TKA) .Methods:Using the convenient sampling method, a total of 104 patients with TKA kinesiophobia who were admitted to First Ward of Department of Orthopedics in the First Affiliated Hospital of Zhengzhou University from August 2019 to November 2020 were selected as the research objects. Among them, 52 patients admitted from August 2019 to March 2020 were set as the control group, while 52 patients admitted from April to November 2020 were set as the experimental group. They were respectively given routine health education and health education based on teach-back method. Before the intervention and 2 weeks after the intervention, the differences in Functional Exercise Compliance Scale for Orthopedic Patients, Hospital for Special Surgery Knee Score (HSS) and Modified Barthel Index (MBI) scores were compared between the two groups. Results:After implementing the health education intervention program based on the teach-back method, the total score of Functional Exercise Compliance Scale for Orthopedic Patients of patients and dimensions scores of exercise compliance related to physical aspects, the exercise compliance related to psychological aspects and the exercise compliance related to active learning in the experimental group were higher than those of the control group, and the differences were statistically significant ( P<0.05) . The scores of HSS and MBI in the experimental group were higher than those in the control group after intervention, and the differences were statistically significant ( P<0.01) . Conclusions:The health education intervention program based on the teach-back method is scientific and reliable, which can effectively improve the functional exercise compliance of TKA patients with kinesiophobia, promote recovery of knee joint function and improve their activities of daily living.

Medical Immunology is a core basic course for medical majors. In order to broaden students'horizons, cultivate students'scientific thinking, and improve students'problem-solving ability,we designed the"scientific problem-based discussion-situational teaching"module in Medical Immunology course. We create"scientific research situations"based on"cutting-edge topics"to allow students investigating in the laboratory just like scientific researchers. The course includes three parts:"Problem-Lecture-Dis-cussion","Teaching by Expert"and"Experiment Observation",allowing students to experience the research processes, including how to propose, analyze and solve the scientific questions. This teaching method effectively promotes students'understanding and ap-plication of theoretical knowledge, increases their team spirit, cultivates their scientific thinking, and stimulates their interests in sci-entific research.

Psychological factors have become significant contributors to the onset and progression of insomnia. This article explored the treatment of insomnia from the perspective of “liver governs wei qi (defensive qi)”. The concept of “liver governs wei qi (defensive qi)” is summarized in three aspects, firstly, the liver assists the spleen and stomach in transformation and transportation, governing the generation of wei qi; secondly, the liver aids lung qi diffusion and dispersion, governing the distribution of wei qi; thirdly, the liver regulates circadian rhythms, governing the circulation of wei qi. It is proposed that the clinical treatment of insomnia should focus on the following methods: for regulating the liver to harmonize the five viscera, and facilitate the circulation of wei qi, medicinals entering the liver channel include Chaihu (Bupleuri radix), Baishao (Paeoniae Radix Alba), Zhizi (Gardeniae Fructus), and Suanzaoren (Ziziphi Spinosae Semen) could be commonly used; for nourishing the liver, the treatment should align with the day-night rhythm, and herbs such as Baihe (Lilium), Hehuan (Albizia julibrissin), and Yejiaoteng (Polygoni multiflori caulis) are commonly used; for soothing the liver and address both mental and physical health to calm wei qi, treatment should advocate verbal counseling, psychological regulation, and health education. Ultimately, this treatment approach can free liver qi to flow, soothe qi movement, restore the motion of wei qi, regulate during day and night, balance yin and yang, and resolve insomnia effectively.

OBJECTIVE:
To explore the effects of taurolithocholic acid (tLCA) and chenodeoxycholic acid (CDCA) on the expression of aorexigenic neuropeptide in mouse hypothalamus GT1-7 cells.
METHODS:
Mouse hypothalamic GT1-7 cells were treated with culture medium containing 10% FBS (control group, =3) or with 10 nmol/L, 100 nmol/L, 1 μmol/L and 10 μmol/L tLCA (tLCA group, =3) or CDCA (CDCA group, =3) for 12, 24 or 48 h. Real-time PCR was performed to determine the expression levels of proopiomelanocortin (POMC) mRNA in the cells, and the production levels of α-melanocyte-stimulating hormone (α-MSH) were assessed using an ELISA kit. Signal transduction and activator of transcription 3 phosphorylation (p-STAT3), threonine kinase phosphorylation (p-AKT), suppressor of cytokine signaling 3 (SOCS3), G protein-coupled bile acid receptor-1 (TGR5) and farnesoid X receptor (FXR) protein were detected by Western blotting.
RESULTS:
Western blotting results showed that mouse hypothalamic GT1-7 cells expressed two bile acid receptors, TGR5 and FXR, whose expressions were regulated by bile acids. Real-time PCR showed that the expression of POMC mRNA was significantly increased in the cells after treatment with 10 μmol/L tLCA or CDCA for 24 h. POMC-derived anorexigenic peptide α-MSH increased significantly in GT1-7 cells after treatment with 10 μmol/L tLCA or CDCA for 24 h. Treatment of the cells with tLCA or CDCA significantly increased the expressions of intracellular signaling proteins including p-STAT3, p-AKT and SOCS3.
CONCLUSIONS
Mouse hypothalamic GT1-7 cells express bile acid receptors TGR5 and FXR. Bile acids tLCA or CDCA can promote the expression of POMC mRNA and increase the production of the anorexigenic peptide α-MSH. The intracellular signaling proteins p-AKT, p-STAT3 and SOCS3 are likely involved in bile acid-induced anorexigenic peptide production.
