1.Analysis of the test results of HBV serum markers and HBV DNA of the neonates born to HBsAg-positive mothers

Objective To observe the HBV serum markers and HBV DNA expressions of the neonates born to the HBsAg-positive mothers.Methods By detecting serum immunity markers of hepatitis B virus (5 items) and serum HBV DNA of 283 neonates (a pair of twins) born to 282 HBsAg-positive mothers.Results 12 patterns emerge from the study of the hepatitis B serum markers of 283 neonates.Topping the list is the combination of HBeAg and anti-HBc positive accounting for 48.41% (137/283),followed by the combination of anti-Hbe and anti-HBc positive accounting for 22.26% ( 62/283 ).The third highest combination is that of HBsAg,HBeAg and anti-HBc positive accounting for 12.37% (35/283).There are five combinations accounting for 16.61% (47/283),each with HBsAg-positive.No case is found of the five items all negative or only HBsAb positive.Five cases are detected of serum HBV DNA ≥ 1 ×103IU/ml accounting for 1.77%.Conclusions Neonates born to HBsAg-positive mothers display complex patterns of serum hepatitis B markers,the dominant pattern being the combination of HBeAg and anti-HBc positive.Cases of serum HBV DNA ≥ 1 × 103IU/ml are rare.

OBJECTIVETo evaluate the usefulness of new microspincolumn method for the measurement of a1pha-fetoprotein variant AFP-L3 in differentiation of benign and malignant liver disease and the warming for liver cancer.
METHODSAFP-L3 was isolated by using microspincolumn coupled with lens culinaris agglutinin (LCA), AFP and AFP-L3 were determined with chemiluminescent immunoassay, the proportion of AFP-L3 levels AFP-L3(%) were calculated, and the relationship between the elevated AFP-L3(%) levels and benign and malignant liver disease was analyzed.
RESULTSThe levels of AFP-L3(%) in serum of patients with hepatocellular carcinoma was significantly higher than those in the patients with other liver diseases (P < 0.001). Taking AFP-L3(%) >or= 10% as the diagnostic criteria, the sensitivity for diagnosis of liver cancer was 90.9%.
CONCLUSIONDetection of AFP-L3 seemed to be of clinical value in diagnosis and differential diagnosis of hepatocellular carcinoma; it may be especially important for identifying patients with hepatocellular carcinoma whose a1pha-fetoprotein level is low.

3.Clinical application Studies on AFP-L3 detected by micro-spin column method

Objective To explore the clinical value of Lens culinaris agglutinin-reactive alphafetoprotein in the differentiation diagnosis between benign and malignant liver diseases, as well as the early warning of hepatocellular carcinoma. Methods Alpha-fetoprotein variants from 300 patients with liver diseases were isolated with micro-spin column equipped lens culinaris agglutinin (LCA). The AFP and AFP-L3 were detected by the electrochemical luminescence (ECL) method, and the proportions of AFP-L3 were calculated. Results The positive rates of AFP-L3 of HCC patients and chronic liver disease patients were 95% and 64% respectively, there were significant difference in two groups (x2 = 134.72, P < 0. 01 ),the HCC incidence rates of AFP-L3 positive and negative chronic liver disease patients showed significant difference(x2 = 80. 158, P < 0.01 ). there were no correlations between the proportion of AFP-L3 and AFP consistency(r = 0.046,P >0.05). Conclusions The detection of AFP-L3 by micro-spin column assay show great clinical value in the differentiation diagnosis of benign and malignant liver diseases, as well as the early warning of hepatocellular carcinoma.
4.Study on clinical value of HBV large envelope protein detected by up-converting phoshor immune chromatography assay

Objective To evaluate the application value of the up-converting Phoshor technology immunochromatography for HBV large envelope protein (HBV-LP) quantitative determination strip in hepatitis B patients.Methods Serum HBV-LP was detected by a new UPT-based immunochromatograhpic technology,and HBV DNA was quantitively detected by real time fluorescent quantitation polymerase chain reaction (RT-PCR),HBV five serum markers were detected by chemiluminescence method.Results In 500 cases of patients with hepatitis B,HBV-LP and HBV DNA positive rates were 58.0% and 42.2% respectively,there was significant difference between the positive rate of HBV DNA and that of HBeAg(P < 0.01); In 215 cases of HBeAg negative specimens,the positive rates of HBV DNA and HBV-LP were 29.3% and 37.2% respectively,the difference was statistically significant (P > 0.05); and HBeAg positive rate was 57.0%,there was significant difference between the positive rate of HBV DNA and that of HBeAg (P < 0.01).Conclusion HBV-LP detected by UPT method can be used for the evaluation of viral replication and prognosis of patients with HBeAg negative and HBV DNA low copies patients.Combing detection of HBV DNA,HBV-LP and HBeAg is conducive to the judgment of HBV replication level and determination of antiviral treatment end point.
5.Bioluminescent imaging monitoring of a anti-angiogenesis therapeutic gene vasostatin in tumor cell PC3.

OBJECTIVETo generate a sensitive tool for noninvasive monitoring of a therapeutic gene vasostatin.
METHODSWe fused the bioluminescent reporter gene firefly luciferase to the therapeutic transgene vasostatin and ensured that these two proteins would not interrupt each other and kept their own natural character.
RESULTSWe therefore examined clones of PC3 cells stably expressing fusion gene and positive controlfluc with bioluminescence. In vivo imaging of PC3-Fluc subcutaneous tumors showed that the mean tumor bioluminescence increased in animals over several weeks.
CONCLUSIONNoninvasive monitoring facilitates the detection of gene expression in vivo and in vitro.

6.Relaxed effect of Cynomorium songaricum Rupr and its active ingredients on detrusor muscle strip tension in rats

Objective To study the relaxed effect of Cynomorium songaricum Rupr (CS) on the detrusor muscle strip tension in rats.Methods Detrusor muscle strips of rats were isolated.The effects of the CS extract and its active ingredients on the potassium chloride (KCl) or acetylcholine (Ach) induced-contraction of detrusor muscle strip were investigated in vitro respectively.Results Both KCl and Achsignificantly induced spontaneous contraction of bladder detrusor muscle strips,and CS extract inhibited both KCl and Ach-induced contraction in a dose-dependent manner.The maximum relaxation ratios of CS in response to KCl and Ach were (23.34 ± 2.40) % and (25.42 ± 5.53) % respectively at the concentration of 1 × 10-2g · mL-1.Its active ingredient Luteolin (LUT),Epicatechin (EPI) and Epigallocatechin gallate (ECG) also inhibited KCl-induced contraction of bladder detrusor muscle strips,and the relaxation ratios were (19.31 ±7.19)%,(19.03 ±6.86)% and (21.22 ± 10.89) %,respectively.Conclusion CS could relieve detrusor tension in bladder,in which LUT,EPI and ECG are involved.

Objective To monitor the susceptibility of clinical isolates to antimicrobial agents in tertiary hospitals in major regions of China in 2022.Methods Clinical isolates from 58 hospitals in China were tested for antimicrobial susceptibility using a unified protocol based on disc diffusion method or automated testing systems.Results were interpreted using the 2022 Clinical &Laboratory Standards Institute(CLSI)breakpoints.Results A total of 318 013 clinical isolates were collected from January 1,2022 to December 31,2022,of which 29.5%were gram-positive and 70.5%were gram-negative.The prevalence of methicillin-resistant strains in Staphylococcus aureus,Staphylococcus epidermidis and other coagulase-negative Staphylococcus species(excluding Staphylococcus pseudintermedius and Staphylococcus schleiferi)was 28.3%,76.7%and 77.9%,respectively.Overall,94.0%of MRSA strains were susceptible to trimethoprim-sulfamethoxazole and 90.8%of MRSE strains were susceptible to rifampicin.No vancomycin-resistant strains were found.Enterococcus faecalis showed significantly lower resistance rates to most antimicrobial agents tested than Enterococcus faecium.A few vancomycin-resistant strains were identified in both E.faecalis and E.faecium.The prevalence of penicillin-susceptible Streptococcus pneumoniae was 94.2%in the isolates from children and 95.7%in the isolates from adults.The resistance rate to carbapenems was lower than 13.1%in most Enterobacterales species except for Klebsiella,21.7%-23.1%of which were resistant to carbapenems.Most Enterobacterales isolates were highly susceptible to tigecycline,colistin and polymyxin B,with resistance rates ranging from 0.1%to 13.3%.The prevalence of meropenem-resistant strains decreased from 23.5%in 2019 to 18.0%in 2022 in Pseudomonas aeruginosa,and decreased from 79.0%in 2019 to 72.5%in 2022 in Acinetobacter baumannii.Conclusions The resistance of clinical isolates to the commonly used antimicrobial agents is still increasing in tertiary hospitals.However,the prevalence of important carbapenem-resistant organisms such as carbapenem-resistant K.pneumoniae,P.aeruginosa,and A.baumannii showed a downward trend in recent years.This finding suggests that the strategy of combining antimicrobial resistance surveillance with multidisciplinary concerted action works well in curbing the spread of resistant bacteria.
