Early diagnosis and early thrombolysis may have a better prognosis in patients with ischemic stroke.The benefits from the development of modem magnetic resonance imaging techniques in this field have received great attention,and it directly guides the 'management of the acute ischemic stroke,This article briefly reviews diffusion-weighted imaging,perfusion- weighted imaging and MR spectroscopy analysis that are more commonly used in current clini- cal practice.
3.Why is it difficult for PCR-SSP to determine some alleles at HLA-B locus?

4.Effective Way in Reducing Missing Report Rate of Nosocomial Infection

OBJECTIVE To reduce the missing report rate of nosocomial infection(NI) by establishing relevant surveillance protocol,improve the early warming system of NI,and therefore prevent the outbreak and prevalence of NI.METHODS Based on Nosocomial Infection Diagnosis Standard published by Chinese Ministry of Health in 2001,116 213 patient data from Jan 2000 to Dec 2006 had been reviewed;factors associated with NI missing report rate had been reevaluated;measures to reduce the rate had been made;and the feedback information of NI control had been collected.Meanwhile,according to high risk factors provided by the hospital information system,the prospective investigations had been made.Surveillance protocol had been established as above.RESULTS By the implementation of the protocol,the missing report rate of NI had been under control,lower than the standard rate of 20% for tertiary hospitals.In 2005,the protocol was reinforced and the NI missing report rate had been reduced to 7.4% by 2006.CONCLUSIONS The real-time monitoring of NI cases is an effective way in reducing missing report rate of NI.
5.Color doppler ultrasonography per rectum after intrarectal pouting for diagnosis of rectal tumors

Objective To discuss the characteristics of ultrasonic images obtained by color Doppler per rectum after intrarectal pouring and the advantages of this method.Methods 40 cases who were diagnosed as rectal carcinoma prior to surgeries were investigated.The rectum was cleand and then was poured with normal saline before examination.Color Doppler ultrasonography per rectum was performed with intrarectal probe touching the inferior edge of the tumor.The 2-dimensional images and color Doppler images were then analyzed.Results The accurate typing of rectal tumors(detailedly,the ulcerative,intumescent,and confined types)can be obtained with this method.Within the 40 cases,24 were ulcerative,12 were intumescent and 4 were confined.Only one case with ulcerative tumor was misdiagnosed as intumescent.The total accuracy rate was 97.5%.As with rectal tumors less than 5cm,there was no significant difference between the measurement by ultrasonography and that by pathologic methods (P>0.05).While for tumors larger than 5cm,the measurement by ultrasonography was significantly different from that by pathologic methods (P<0.05).There was no significant correlation between the bloodstream signal of color Doppler and pathological grading(P>0.05).The method can be applied for patients who have a narrow rectal duct or are suffering from agonies which prevent the passage of the colonoscope.For cases failing to detect the margin and surface of the tumor with routine method,or ultrasonic probe failing to pass the confined area leading to illegible 2-dimensional images,This method is competent as well.Conclusion Color Doppler ultrasonography per rectum after intrareetal pouring can be applied for the diagnosis of rectal tumors of all types.
6.Effect of nitric oxide inhalation on pulmonary inflammatory responses in dogs with endotoxin-induced acute respiratory distress syndrome

Objective To investigate the effects of nitric oxide (NO) inhalation on the expression of TNF-?,IL-8 and CD11b mRNA in lung tissue during acute respiratory distress syndrome (ARDS) induced by intravenous injection of endotoxin in dogs.Methods Twelve pure bred beagle dogs of both sexs weighing 8-12.5 kg were randomly divided into 2 groups: NO group received mechanical ventilation with NO inhalation (n = 6) and control group received only mechanical ventilation ( n = 6) . Sepsis and ARDS were induced by intravenous injection of endotoxin as described in detail in our previous paper. Hemodynamics and pulmonary oxygenation were monitored and shunt fraction was calculated. At the end of experiment the animals were sacrificed and lung tissue was obtained aseptically and stored in the liquid nitrogen at - 180℃ . The total RNA was extracted. Reverse transcription-polymerase chain reaction (RT-PCR) was used to detect the expression of TNF-?,IL-8 and CD11b mRNA. The total RNA was reversely transcripted to cDNA. Then the cDNA was amplified by PCR. The product of PCR was scanned by gel-image analysis system.?-action was used as internal control. Semi-quantitative method was adopted for measurement of TNF-? ,IL-8 and CD11b mRNA expression. Results The expression of TNF-?, IL-8 and CD11b mRNA in lung tissue was significantly decreased in NO group compared with those in control group.Conclusion NO inhalation reduces expression of TNF-?, IL-8 and CDllb mRNA in lung tissue during ARDS induced by intravenous endotoxin.

Wheezing is the most common respiratory disease in children.In recent years,the incidence of childhood wheezing showed an upward trend,the hot topic in the current study is how to draw up a rational and effective treatment to reduce wheezing.This paper summarized the latest research progress of pathogenic factors,clinical classifi-cation,pathogenesis and prevention strategies in childhood wheezing.It aimed to provide a theoretical basis for early diagnosis and individual treatment for children with wheezing diseases.
8.Adsorption Performance in Binding of Methylene Blue in Aqueous Solution by Orange Peel

Orange peel was used as lowcost adsorbent in binding of Methylene Blue.The effects of equilibrium time,pH,dye concentration have been studied.Carboxyl,amine and phosphonate functional groups were present in the orange peel.The equilibrium time was 1 hour,the maximum adsorption capacities of the orange peel was 370.3?31.0 mg/g at pH 10.The Langmuir and Freundlich isotherms were well fitted in this biosorption system.Results showed relatively higher rate constant and biosorption capacities.These adsorption performance indicate the orange peel as a potentially economical adsorbent for dye removal.
9.Effect of Acupuncture at Myofascial Trigger Points on the Quality of Life in Patients with Myofascial Pain Syndrome

Objective To investigate and compare the clinical efficacies of acupuncture at myofascial trigger points and extracorporeal shock wave and their effects on the quality of life in patients with myofascial pain syndrome. Method Seventy-eight patients with myofascial pain syndrome were allocated, using a random number table, to acupuncture and shock wave groups, 39 cases each. The two groups received acupuncture at myofascial trigger points and extracorporeal shock wave, respectively. The clinical therapeutic effects were compared between the two groups of patients after 20 consecutive days of treatment. The quality of life score was recorded in the two groups of patients before and after one month of treatment. Result The VAS score (1.57±0.83), the PRI score (1.87±1.06) and the PPI score (0.94±0.72) in the acupuncture group were lower than the VAS score (2.16±0.95),the PRI score (2.68±1.14) and the PPI score (1.53±0.81) in the shock wave group. All had a statistically significant difference (P<0.05). The VAS score, the PRI score and the PPI score decreased significantly in the two groups of patients after treatment compared with before. Eight scaled scores: physical functioning, physical role functioning, bodily pain, social role functioning, vitality, general health perceptions, emotional role functioning and mental health increased significantly in the two groups of patients after treatment compared with before (P<0.05) and increased more in the acupuncture group (P<0.05). The marked efficacy rate was 84.6% in the acupuncture group, which was significantly higher than 61.5% in the shock wave group (P<0.05). Conclusion Both acupuncture and extracorporeal shock wave therapy have a marked clinical effect on myofascial pain syndrome. It can further improve the quality of life in the patients.
10.Clinical pedicle internal fixation combined with debridement and bone graft through intervertebral approach as treatment of segmental lumbosacral vertebral tuberculosis

Objective To discuss and observe the clinical effect of intervertebral pedicle internal fixation and debride?ment combined with bone graft through posterior approach/trans-intervertebral space approach on the treatment of uni/multi-segmental lumbosacral vertebral tuberculosis (TB). Methods A cohort of 37 patients, with single or multiple segmental ver?tebral destruction due to TB, were treated by trans-intervertebral debridement, posterior pedicle screw system internal fixa?tion and intervertebral bone graft. All patients underwent X-ray,CT and MRI examination to observe the combination treat?ment effect. Results Most patients (n=34) enjoyed primary healing, in which include only 4 cases that presented symptom of nerve root stretch injury during operation but all recovered after 3 months. Other 3 patients underwent secondary healing due to sinus but two were rectifying with anti-TB therapy and wound dressing. The other 1 case suffered from sinus tract was healed through second debridement and rectifying therapy. X-ray, CT and MR at 6 months after operation indicated that all patients present great graft osseous fusion, good recovering of height of vertebral body without kyphosis deformity nor internal fixation loosening nor screw breakage. Conclusion Intervertebral pedicle internal fixation and debridement combined with bone graft through posterior approach/trans-intervertebral space approach is with minimum invasion but good graft fusion ef?fects, harder fixation and satisfactory clinical effects in the treatment of uni/multi-segmental lumbosacral vertebral tuberculosis.
