Objective To study the application and efficacy of hemihepatie vascular occlusion in HCC resection. Methods 90 ca-ses of HCC patients were roiled in and divided into two groups. Liver resection was performed with Pringle maneuver or hemihepatic vascular occhusion. Ischemia time, operative time, blood loss, postoperative liver function, the occurrence of complications and the recovery of gastro-intestinal function were recorded. The efficacy of the two occlusions was compared. Results There is no significant difference between hemibepatic vascular occlusion and Pringle maneuver in ischemia time, operative time, blood loss (P>0.05). However, there is signifi-cant difference in postoperative liver function, the occurrence of complications and intestinal function recovery time. The former is significant-ly superior to the latter (P<0.05). Conclusions The bemihepatic vascular occlusion and Pringle maneuver occlusion have no difference in ischemia time, operative time and blood loss. But bemihepatic vascular occlusion has less impact on liver function, lower rate of occur-rence of complications and faster recovery speed. Therefore, hemibepatie vascular occlusion is better than Pfingle maneuver occlusion in be-parle inflow occlusion in HCC surgery.

Objective To evaluate the effects of epidural infusion of different concentrations of lidocaine on the amount of propofol needed to induce anesthesia and the end-tidal isoflurane concentration needed to maintain BIS at 50-55 during operation. Methods Forty-five ASA Ⅰ-Ⅱ patients of both sexes (24 males, 21 females) aged 21-60 yrs, weighing 46-77 kg undergoing elective upper abdominal surgery were randomly divided into 3 groups with 15 patients in each group : group Ⅰ epidural normal saline ( NS); group Ⅱ epidural 1 % lidocaine (Lido) and group Ⅲ 2 % Lido. An epidural catheter was inserted at T9-10 into epidural space and advanced 3-4 cm cephalad. After a test dose of 3 ml, NS or 1% or 2% Lido 10 ml was injected into the epidural space. Anesthesia was induced with propofol infusion at 25 mg?min-1. Propofol infusion was stopped when the patients stopped responding to loud voice (OAAS/2) and the amount of propofol infused was recorded. Then fentanyl 2 ?g?kg-1 and vecuronium 0.15 mg?kg-1 were given i.v. . The patients were intubated and mechanically ventilated and PETC02 was maintained at 35 mm Hg. Anesthesia was maintained with isoflurane inhalation and epidural infusion of Lido or NS at 7 ml?h-1 . BIS was maintained at 50-55 during operation. Objective To establish an HPLC method for testing the content of icariin in Yinyangsu capsules.Methods An Lichrospher C18(4.6mm?250mm,5?m) was used as the column,and column C18 as the protecting column.The mobile phase was a mixture of acetonitrile-20g/L acetum(30∶70) in a flow rate of 1.0mL/min.The detected wavelength was 270nm.Column temperature was room temperature and the injection quantity was 10?L.The sample was prepared when we added 50%(V/V) alcohol and then made ultrasonic extraction.Measurement was made after the prepared sample was filtrated on 0.45?m filter paper.Results Icariin in the sample was well separated.The linear range was 10.0-60.0mg/L.The linear equation was C=2.042?10-5A+0.554,while the correlation coefficient was 0.9993;the average recovery was 99.8% and RSD was 0.65%(n=6).Conclusion This method can be used in content measurement of Yinyangsu capsules.

Objective To establish a method of high performance liquid chromatogram(HPLC) for measuring matrine and oxymatrine in Zhidaipian Tablets.Methods A Lichrospher-NH2 column(4.6 mm?250 mm,5?m) was used with a mobile phase consisting of acetonitrile-ethanol-0.5 mol/L phosphoric acid(80∶10∶10),and in a flow rate of 1.0 mL/min.The detective wavelength was 212 nm.Column temperature was room temperature and the injection quantity was 20 ?L.Results The samples were well separated.The linear range was the same for the target(1.0-10.0 ?g/mL).The linear equations for matrine was C=1.201?10-4A +0.161,the correlation coefficient was 0.9992,and for oxymatrine was C=1.366?10-4A +0.221,while the correlation coefficient was 0.9996,respectively.The mean recovery rate were 99.9% and 99.4%,respectively.RSD was 1.48% and 4.33%,respectively(n=6).Conclusion The method is simple,reliable,accurate and can be used to the quality control of the Zhidaipian Tablets.

Objective To explore the clinical experience of endovascular treatment for arteriosclerosis obliterans (ASO) of lower extremities.Methods Endovascular treatment were performed on 22 patients (26 limbs) suffering from ASO which were diagnosed by magnetic resonance angiography (MRA).The clinical efficacy after operation was analyzed.Results Twenty-two lower extremities of 18 patients successfully accepted endovascular treatment with 12 stents planted without major complications.Four cases failed to endovascular treatment and 2 of them converted to bypass surgery.The clinical symptoms of limb ischemia vanished or significantly improved after treatment.The ankle brachial index (ABI) of affected extremities increased from 0.35?0.13 (before operation) to 0.70?0.15 (after operation),P

Objective To detect the inhibitory effect of HSP27-siRNA on hepatocellular carcinoma(HCC) cells.Methods HCC cell QGY lines were cultured with HSP27-siRNA in a differtent range of concentration for various time periods.Cell activity was studied by MTT.The changes of cell cycle and apoptosis were analyzed by FCM.RT-PCR was used to detect the effect of HSP27-siRNA on QGY cell expression of HSP27 mRNA.Western blot was used to detect the inhibition efficiency of HSP27-siRNA on HSP27protein.Results The proliferation of QGY cell was inhibited by HSP27-siRNA,and HSP27-siRNA decreased the expression of HSP27 protein.HSP27-siRNA inhibited the proliferation of QGY cell line and induced apoptosis in vitro,and its effect was both dose-and time-dependent.Conclusions HSP27-siRNA can inhibit proliferation and induce apoptosis of HCC cancer cell lines.Thus,it may become a method for effective treatment of HCC cancer.

OBJECTIVE: To investigate the compatible stability of cefotiam hydroloride for injection with fructose injection.METHODS: The content changes of the mixture within 8h at 25℃ were determined by UV spectrophotometry,and its external appearance was observed,and the pH value as well as particle size of the mixture were measured.RESULTS: The mixture was clear within 6h,showing no visible changes in appearance,pH value and contents of cefotiam,with particles in line with the standard stated in China Pharmacopeia(2005 edition).CONCLUSION: The mixture of ceforiam hydrochloride for injection and fructose injection is stable within 6h.

Objective To investigate the relationship between the genotype /sub -genotype and antiviral treatment.Methods The clinical data of 159 patients with chronic HBV infections were collected,who were treated by adefovir dipivoxil (10mg/d)or entecavir(0.5mg/d)according to the disease′s conditions.The genotypes and subtypes of hepatitis B virus were determined by Nested polymerase chain reaction(nt PCR).The levels of serum HBVDNA replication,ALT levels and HBV serologic markers were detected pre or post -treatment 24 weeks, 48 weeks.Observed the relationship between the HBV genotypes/sub -genotypes and the antiviral efficacy of adefovir dipivoxil or entecavir treatment.Results After 24 weeks of adefovir dipivoxil therapy,ALT normalization rate of subgenotype Ba and subgenotype C2 was 66.7% vs 66.2%(χ2 =0.74,P >0.05),HBeAg negative conversion rate of two subgenotypes was 27.3% vs 23.1%(χ2 =0.10,P >0.05),HBVDNA negative conversion rate of two sub genotype Ba and subgenotype C2 was 33.3% vs 30.9%(χ2 =0.03,P >0.05),respectively.After treatment for 48 weeks,ALT normalization rates of subgenotype Ba and subgenotype C2 were 83.3% vs 82.4%(χ2 =0.01,P >0.05),HBeAg negative conversion rates of two subgenotypes were 45.5% vs 34.4%(χ2 =0.49,P >0.05 ),HBVDNA negative conversion rates of two subgenotypes were 58.3% vs 48.5%(χ2 =0.39,P >0.05).After 24 weeks of entecavir therapy, ALT normalization rates of subgenotype Ba and subgenotype C2 were 71.4% vs 69.6%(χ2 =0.02,P >0.05 ), HBeAg negative conversion rates of two subgenotypes were 33.3% vs 30.8%(χ2 =0.03,P >0.05 ),HBVDNA negative conversion rates of two subgenotypes were 42.9% vs 39.3%(χ2 =0.06,P >0.05 ),respectively.After treatment for 48 weeks,ALT normali zation rates of subgenotype Ba and subgenotype C2 were 85.7% vs 83.9%(χ2 =0.03,P >0.05),HBeAg negative conversion rates of two subgenotypes were 50.0% vs 44.9%(χ2 =0.10, P >0.05),HBVDNA negative conversion rates of two subgenotypes were 71.4%vs 67.8%(χ2 =0.07,P >0.05). Conclusion The study showed that genotype C(C2)is a predominant HBV genotype among people with chronic HBV infections in Shanxi province.HBV subgenotypes Ba and C2 have no significant difference in virologic,biochemi-cal,and immunologic response to ADV or ETV.
