Rapamycin is frequently used as an immunosuppressive agent and also plays an important role in tumor suppression.It combines to FKBP12 and forms a compound to inhibit the serine/threonine protein kinase mammalian target of rapamycin(mTOR),its activation is required for protein synthesis and cell-cycle progression.Rapamycin can block cell cycle at G1 phase and to inhibit cell proliferation.Furthermore rapamycin may inhibit cytokine production and cytokine signaling.Therefore rapamycin is used in the treatment of post-transplantation organ rejection and tumor therapy due to its dual effects of immuno-suppression and tumor inhibition.

Objective To evaluate the clinical significance of orthotopic continent detenial sigmoid neobladder.Methods 12 patients with bladder cancer were admitted and underwent radical cystectomy and detenial sigmoid neobladder.Results All the 12 patients were followed up for a mean of 18.6 months(range from 6 to 29 months).The blood Cr and BUN levels were both in the normal range without acidosis in all the cases.Only 1 had unilateral ureteral urine reflux during cystography.Of the 12 patients,11 cases(91.7%) achieved continence during daytime;2 cases(16.7%)were incontinent at night,but 1 case could control uri- nation by being woken up at night.The capacity of the neobladder was 220 to 550ml with a mean of 350ml and the maximal pressure of the neobladder during filling was 15 to 55cm H_2O(1cm H_2O=0.098kPa)with a mean of 35cm H_2O.Conclusion As a simple operation the detenial sigmoid neobladder can be performed easily with less complications and more reliable results.This operation may be generally applied in clinical practice.

Objective_To investigate whether overexpression of TRAF4 in human breast cancer may have impact on its cell proliferation.Methods_This study has two groups, MDA-MB-231 transfected with pcDNA3-TRAF4-DM-TRAF or pcDNA3 .We detected the expression and localization of TRAF4 with immunofluorescence and western blot.We detected the expression of the phosphorylation level of p70S6K and S6 with westernblot.Flow cytometry was used to detecte cell cycle.MTT Assay was used to detected cell reproductive capacity.Results_TRAF4 local-ized in both cytoplasm and nuclei in MDA-MB-231.Nuclear expression of TRAF4 in nuclei was lower than that in cytoplasm (P<0.05).After pcDNA3-TRAF4-DM-TRAF was transfected into the cells, the expression of TRAF4 in nuclei was increased (P<0.05).The phosphorylation level of p70S6K and S6 significantly increaseced (P<0.05,P<0.01).More S phase cells were recorded(P<0.01) by FCM.The cell proliferation was promoted by MTT ( P<0.01) .Conclusions_The expression of TRAF4 in nuclei may play an important role in the cell prolif-eratuion by promoting p70S6K and S6 activation.

Objective To introduce the key issues of image registration in radiotherapy and study the performances of the three kinds of registry ways based on gray-scale registration, included mean-square measure, normalized correlation measure and mutual information measure. Methods After analysis of elements of the matching algorithm, the different registration algorithm performance are evaluated by the use of a comprehensive registration error based on C ++ to be realized, which can compare with the evaluation results of traditional target registration error. Results All three kinds of measure can both match accurately in plesiotype model images, which registration accuracy and speed in the standard of mutual information measure is more stable, integrated registration error is only half of the other two measures. Conclusion The comprehensive evaluation result by using registration error is more objective, the mutual information measure is better registration measure in radiotherapy.

Objective To study the effects of total intravenous anesthesia and combined inhalation and intravenous anesthesia on arousing time and recovery quality in rectifying surgery of scoliosis. Methods Forty patients (ASAⅠ-Ⅱ) were divided randomly into total-intravenous anesthesia group and combined inhalation and intravenous anesthesia group (n=20 each). Target controlled infusion (TCI) with propofol (2-4?g/ml) was used for maintenance in total intravenous anesthesia group, while isoflurane (0.8%-1.5%) and nitrous oxide (50%) were used in combined inhalation and intravenous anesthesia group. TCI remifentanyl (2-6ng/ml) was used for maintenance in both groups. Objective To compare the influences of sulfentanyl or fentanyl combined with midazolam on respiratory function in slow induction of anesthesia.Methods Forty ASA Ⅰ-Ⅱ patients were divided into two groups.Anesthesia was induced with midazolam 0.03mg/kg in both groups,and fentanyl 2?g/kg(i.v.)was given in fentanyl group or sulfentanyl 0.2?g/kg(i.v.)in sulfentanyl group.Five minutes later,2ml of 1% decicaine was administered by cricothyroid membrane puncture to facilitate the intubation.Respiratory indexes(respiratory frequency,VT,MVV,PETCO2,SPO2),circulatory indexes(MAP,HR)and sedation level were measured before and 1,2,3,4,5 minute(s)after injection of drugs,at cricothyroid membrane puncture,and pre-and post-intubation.Results Patients showed respiratory depression(respiratory frequency was reduced)in both groups 2 minutes after injection of drugs.However,respiratory frequency was decreased more markedly in fentanyl group than that in sulfentanyl group.Ten out of 20 patients were obliged to receive breathing intervention in fentanyl group,but two in sulfentanyl group.Conclusion In combination with midazolam,sedative effect of sulfentanyl is stronger than that of fentanyl in equivalent analgesic dose,but with less respiratory depression.Sulfentanyl may be more suitable for slow induction of anesthesia.
