2.In Vitro High-Resolution Sonography of Benign and Malignant Lymph Nodes: A Sonographic-Pathologic Correlation.

PURPOSE: In vitro sonography of lymph nodes was performed to ascertain the differential point between benignancy and malignancy. MATERIALS AND METHODS: Ninty-one lymph nodes were obtained at gastric cancer surgery and pathologically proved as benignancy in 68 and malignancy in 23 cases. Sohographic and pathologic correlation of resected ninty one lymph nodes was done in terms of size, the ratio of longest diameter versus shortest diameter, central hyperechogenicity, and peripheral cortical echo. RESULTS: Sonographic findings of benign lymph node were small size, oval shape(85%), narrow central high echogenicity(44%), and symmetric cortical thickening(41%). Sonographic findings of malignant lymph node were large size, round contour(39%), loss of central high echogenicity(82%), and diffuse increase of cortical thickness(82%). CONCLUSION: The most reliable sonographic criterion for differentiation of benignancy from malignancy was size of a lymphnode.

4.Malignant Fibrous Histiocytoma of the Heart: A case report.

A case of a 58-year-old man with malignant fibrous histiocytoma (MFH) of the right ventricle is reported light and electron microscopically. This is the first case of MFH of the heart in the Korean literature. A tendency for malignant fibrous histiocytoma of the heart to occur in the left atrium of young women is suggested; this sarcoma's usual location is in the soft tissue of elderly men. The tumor consisted of spindle cells arranged in a focal storiform patterns, clusters or sheets of histocyte-like cells, benign and malignant giant cells, inflammatory cells, scattered mitotic figures and anaplasia of stromal cells. In ultrastructure the constituent of cells of the tumor are primitive mesenchymal cells, histiocytoid cells and fibroblast like cells including giant cells.

7.A Study on Patients' Perception of Nurse's Behavior in Protecting Patient Privacy.

PURPOSE: The purpose of this study was to investigate the patient's perception of the nurse's behavior in protecting patient privacy and to make suggestions for medical facilities to increase protection of patient privacy. METHOD: The research was a survey study. The data on protection of privacy in primary nursing, for physical privacy, of patient information and in private conversations were collected in October 2005 from 187 patients in a university hospital. Frequencies, means, t-test, ANOVA, and chi-square-test were used to analyze the data. The SPSS 13.0 for Windows program was used. RESULTS: The mean score for patients' perception of nurse protection of their privacy was 3.33. There were significant differences in perception of protecting patients' privacy according to gender for private conversation, according to level of education for all but physical nursing and for number of admissions for total score and for primary nursing. CONCLUSION: The results of this study suggest the following: a) Institutional polices and nursing guideline should be clearly stated as to the nurses' duty to protect patient privacy. b) Medical facilities should be arranged in a way that allows for the protection of patients' medical information, and should focus on patient privacy. c) It is necessary of nurses to receive periodic in-service education on protecting patient privacy.

8.Usefulness of Routine Renal Sonography Screening for Healthy Infants with Isolated Single Umbilical Artery .

PURPOSE: It is known that single umbilical artery is frequently associated with gastrointestinal or urogenital anomaly, however, routine renal sonography has been debated in healthy neonate with isolated single umbilical artery. This study is designed to determine the usefulness of routine renal sonography in apparently healthy infants with an isolated single umbilical artery. METHODS: Thirty healthy neonates with a single umbilical artery without a major anomaly from January 1995 to July 2002 were enrolled. The authors investigated the clinical background of babies and their mothers after renal sonography after 72 hours of age. When the abnormalities were found at the first renal sonography, the severity of hydronephrosis and degree of obstruction and renal function were analyzed by follow up renal sonography, voiding cystourethrography(VCUG) and technetium-99m-dimercaptosuccinic acid(DMSA) scan or technetium-99m-mercaptoacetyl-triglycerine (MAG3) scan. RESULTS: Among the 30 healthy patients with isolated single umbilical artery, five patients(16.7%) showed abnormalities on first renal sonography with one major(3.3%) and four(13.4%) minor renal anomaly(minimal or mild hydroneohrosis). One major renal anomaly(severe hydronephrosis) showed severe decreased renal function on MAG3 scan without reflux, and the other four minor regressed spontaneously on follow up study. CONCLUSION: The value of routine early renal sonograpy for detecting renal anomaly in healthy infants with an isolated single umbilical artery remained unclear because most of the anomalies would regress spontaneously in the follow up study.

9.Distribution of Pathogenic Genes and Molecular Typing of Yersinia pseudotuberculosis Isolated from Spring Water in Seoul.

In order to investigate pathogenic genes and genetic relationships of Y. pseudotuberculosis strains, We isolated 9 strains of Y. pseudotuberculosis from 380 spring water sites in Seoul from 2000 to 2003. All isolates were distributed to the northeast area in Seoul. The isloates were analyzed for chromosomal virulence gene (inv) and plasmid-borne genes (yadA and lcrF) using PCR to assume pathogenicity. As a result, all isolates were positive for the inv gene, but only five isolates (55.6%) were positive for the yadA and lcrF genes. RAPD and PCR-ribotyping were tested and all isolates were grouped with 90% similarity. RAPD revealed 4 clusters and PCR-ribotyping revealed 2 clusters. The result of this experiments confirmed the view that RAPD had better powerful discrimination than PCR-ribotyping and RAPD typing was effective to distinguish between various strains of Y. pseudotuberculosis from spring water.

10.A Study on Motivation for Pregnancy of Married Women in Korea.

PURPOSE: This study is to define the structural patterns of pregnancy motivation of married women. METHOD: Q-methodology was used on 110 statements collected from interviews with women and a literature review. 38 Q-samples were selected. The Q-samples were administerd to 30 randomized P-sample. RESULT: Data analysis of Q-type obtained by QUANL program shows three types of subjectivity in the perception of pregnancy motivation. Type 1, "Family-connection strengthening", Pregnancy motivation is characterized by the tendency to make home, to tie family member, to connect couple firmly. Type 2,"Self-satisfaction", Women of this type take pleasure and happiness from child rearing in terms of being a mother and having maternal affection. Type 3, "Self-centered", This type of pregnancy motivation puts on high emphasis on self achievement and they make a family planning suitable for the maternal role. CONCLUSION: The stronger was women's pregnancy motivation, the more positive were women's pregnancy experience and maternal role attainment. Therefore, When nurses take care of pregnancy women, they should understand the women's psycho-social aspects and then apply a program of nursing intervention to the pregnant women, It is necessary to continue to study on social and psychological problems related to pregnancy.
