1.Effects of Thera-Band(R) Resistance Training Including Isometric Exercise in Total Knee Replacement Patients.

BACKGROUND: The purpose of this study was to assess the effects of an exercise program using isometric and isotonic thera band exercises. METHODS: Data were collected from 30 total knee replacement patients hospitalized in the orthopedics department at 'B' hospital (control=15, experimental=15). The control group performed isometric exercises for one week while the experimental group performed three or more rounds of both isometric and isotonic exercises, including Thera-Band(R) exercises, everyday for four weeks. RESULTS: The experimental group improved significantly in muscular strength (Z=-1.99, P=0.047) and self-efficacy (Z=19.48, P=0.000), and showed lower levels of depression than the control group (Z=8.60, P=0.003). However, no difference was found in pain experience between the two groups. CONCLUSIONS: Exercise programs using the isometric and Thera-Band(R) resistance training improve muscular strength and self-efficacy and relieve depression in total knee replacement patients.

2.p53 Expression in the Head and Neck Tumor.

Mutations in the p53 gene seem to be the most common genetic changes in human malignancies. Mutation or altered p53 expression is a common occurrence in many solid neoplasms, including head and neck carcinomas. Recent studies have also shown p53 alterations in several premalignant conditions of the colon, esophagus, lung, and brain. Preliminary data have suggested that p53 mutations may be involved in tumor progression. This study was performed to determine the incidence of p53 mutations in histologically 27 squamous cell carcinomas, 19 basal cell carcinomas, 18 Schneiderian papillomas, 3 Schneiderian papillomas with malignant transformation, and 15 pleomorphic adenomas of the head and neck region. The degree of p53 gene overexpression was also evaluated according to differentiation, histologic type of tumor, and tumor progression in the head and neck carcinomas. The results were as follows; 1) Eighteen of 27 squamous cell carcinomas, and 4 of 27 dysplasias adjacent to the squamous cell carcinoma of the head and neck expressed p53 protein, but none of the normal control specimens expressed detectable p53 protein. There was no relationship between differentiation of squamous cell carcinoma and p53 protein expression. 2) Twelve of 19 basal cell carcinomas expressed p53 protein; the adenoid type especially overexpressed p53 protein. 3) Nine of 15 pleomorphic adenomas expressed p53 protein especially in the epithelial components. 4) Thirteen of 18 Schneiderian papillomas and all Schneiderian papillomas with malignant transformation expressed p53 protein. The above results indicate that the p53 protein expression is a useful tool for the prediction of tumor progression in the head and neck tumor, but there was no relationship between the differentiation of the tumor and p53 protein expression.

3.A Survey of Urban Middle-aged Women's Sexual Function and Sexual Distress.

OBJECTIVE: This survey assessed the prevalence and type of sexual dysfunction in middle-aged women regarding sexually related personal distress and examined the prevalence of perceived sexual dysfunction and perceived partners' dysfunction by women. METHODS: One-hundred ten healthy community-based middle-aged women participated voluntarily, and validated question- naires of the Female Sexual Function Index(FSFI) and the Female Sexual Distress Score(FSDS) were used. RESULTS: The percentage of women having sexual dysfunction and sexual distress were 67% and 32% respectively according to the cut-off of FSFI and FSDS. The average score of FSFI was 23.5(s.d.=5.7), which was lower than the cut-off of FSFI, while the average score of FSDS was 13.2, which was in range of a normal score. When considering the two concepts simultaneously, the women were categorized into 4 groups: sexually stressed dysfunction group (24.8%), sexually depressed group(42.2%), sexually healthy group(25.7%), and sexually hyperactive group (7.3%). The percentage of women reporting sexual problems was 24.3%, while the percentage of women reporting sexual dysfunction in their partner was 29.9%. CONCLUSIONS: The results indicate a high rate of sexual dysfunction and sexual distress in middle-aged women. However, considering the relationships between sexual dysfunction and sexual distress, almost two thirds were in the sexually depressed group among the women having sexual dysfunction.

7.Electronmicroscopic Study of Familial Benign Chronic Pemphigus.

Familial benign chronic pemphigua is a rare disease characterized by a localized recurrent eruption of grouped vesicles and exudative lesions involving the neck, groin, axillae and intertriginous areas. There are no systemic manifestations but chronicity and pruritic lesions creates a significant social and psychological handicap to those afflicted. We presented a case of familial benign chronic pemphigus in a 39 years old man He has had recurrent vesicles, fissures and crust formation in inguinal area for 10 years. Light microscopic findings showed suprabasal vesiculation with acantholysis and villi formation. And electron-microscopic findings showed as follows: Bizzare microvilli changes with widened intercellular spaces, followed by loss of desmosome and complete acantholysis. Tonofilament-desmosome complex is disrupted. Many of acantholytic cells are associated with abnormal keratinization which is repreaented by perinuclear thickened bundles of tonofilament arranged in whorls or lumps. Authors diagnosed with clinical symptoms, light and electron-microscopic findings as Hailey-Hailey disease. The lesions were improved by systemic antibiotics and steroid with cold soaking.

8.Immunohistochemical Findings in 10 Cases of Inflammatory Myofibroblastic Tumor.

A wide range of denomination has been used for inflammatory myofibroblastic tumor (IMT). IMT is not entirely homogeneous, even though it shows some overlapping histologic features such as haphazard proliferation of spindle cell and polymorphic chronic inflammatory cell infiltraion. The spindle cell is considered to be of myofibroblastic origin but follicular dendritic cell origin was reported recently. IMT is known as nonneoplastic, aberrant inflammatory response. However, IMT could show local invasion, recurrence, vascular invasion, and malignant transformation, and clonal characteristics and aneuploidy of IMT support the hypothesis that IMT may be a neoplastic process. In order to define the nature of spindle cell of IMT, immunohistochemical stains for smooth muscle actin (SMA), vimentin (VMT), lysozyme, S-100 protein, cytokeratin, CD21 were done. Additional immunohistochemical stains for MIB-1 for proliferating activity and LMP (latent membrane protein) for Epstein-Barr virus (EBV) were done. IMTs were composed of each 2 cases from lung, liver and lymph node and one case from common bile duct, maxillary sinus, bladder and thigh, and were histologically subclassified according to Coffin et al. Nine cases (90%) were positive for SMA and VMT, but no correlation between SMA and VMT immunoreactivity and histologic types was identified. Five cases (50%) were positive for lysozyme and S-100 protein, and histologic type III was negative for lysozyme and S-100 protein, and immunoreactivity for S-100 protein was different according to the histologic subtypes. All 11 cases were negative for CD21 and EBV LMP. MIB-1 labelling index was less than 1% in all cases. In summary, the spindle cell is regarded as myofibroblastic origin rather than follicular dendritic cell origin. Relationship with EBV is not clear, and negligible MIB-1 reaction suggests that IMT might have a good prognosis.

9.Calcium Status and Bone Mineral Density by the Level of Sodium Intake in Young Women.

Previous studies have shown that sodium excretion is positively related to calcium excretion in the urine. As excessive sodium intake is a common nutritional problem in Korea, we intended to investigate associations among sodium intake levels and calcium status, evaluated by 24 hour recall method and urinary excretion, and bone status. We collected dietary information for non-consecutive three days from 139 young adult women 19~29 years. After classifying the subjects into 4 groups based on the dietary sodium levels by daily total sodium intake (mg) and sodium density (sodium intake per 1000 kcal energy intake), we compared the bone status, nutrient intakes, urinary calcium and sodium excretions. The results showed a positive association between total daily sodium intake and intake of other nutrients. However, no significant differences in nutrients intakes were observed among subject groups classified by sodium density levels. There were no significant differences of bone density among groups by total daily sodium intake as well as by sodium density. While total daily sodium intake showed significantly positive relationship with urinary sodium (p < 0.05) and calcium (p < 0.05), sodium density was not related to urinary excretion of calcium and sodium. Our results suggested that promoting balanced meals providing appropriate amounts of energy intake is the essential component of nutrition education for improving calcium status of young Korean women with excessive sodium intake.

10.Affecting Factors of Deviant Behaviors of Korean High School Students.

PURPOSE: The purpose of this study is to analyze the factors affecting Deviant Behaviors of Korean High school Students. METHOD: Data was collected from October 8 to 31, 2002. The subjects for this study were 697 Korean High school Students(boys 347, girls 350), recruited from two High School located in Seoul. Data collection was conducted through the use of 6 Questionnaire that modified by the investigator. The data was analyzed by the SPSS win 10.0 program using Descriptive statistics, Pearson Correlation coefficient and stepwise multiple regression. RESULT: 1) The mean of total item score the Deviant Behaviors scales was 1.59, which was slightly low. 2) There was a significant correlation between Deviant Behaviors, Type A Personality, Aggression, Impulsivity, Stress and Social Support(gamma= .11 ~ .65, p < .001), but It was no significant correlation Type A Personality and Stress(gamma= -.01). 3) Stepwise multiple regression analysis showed that (1) Impulsivity, Social Support and Type A Personality were the predictors of Deviant Behaviors and account for 18.6% of the variance in Deviant Behaviors. (2) Impulsivity account for 3.6% of the variance in Deviant Behaviors in Subjects with a lower degree of score the Deviant Behaviors. Impulsivity and Social Support account for 23.2% of the variance in Deviant Behaviors in subjects with higher degree of score the Deviant Behaviors. (3) Impulsivity account for 18.3% of the variance in Deviant Behaviors in High school girls (n=350). Impulsivity and Social Support account for 20.1% of the variance in Deviant Behaviors in High school boys (n=347). CONCLUSION: Impulsivity and Social Support account for Deviant Behaviors of High school Students. Therefore it is necessary to develop nursing intervention to reduce the level of Impulsivity, to increase the Social Support in order to decrease the Deviant Behaviors.
